The glycine receptor.

The inhibitory glycine receptor (GlyR) is a member of the ligand-gated ion channel receptor superfamily. The GlyR comprises a pentameric complex that forms a chloride-selective transmembrane channel, which is predominantly expressed in the spinal cord and brain stem. We review the pharmacological and physiological properties of the GlyR and relate this information to more recent insights that have been obtained through the cloning and recombinant expression of the GlyR subunits. We also discuss insights into our understanding of GlyR structure and function that have been obtained by the geneti…

Combined supratentorial and infratentorial approach to large pineal-region meningioma.

Our case report is used to describe a combined supratentorial and infratentorial approach to a giant pineal-region meningioma. The procedure involves section of the less dominent transverse sinus and the tentorium. After the cerebellum and the occipital lobe are retracted away from each other, the operative procedure is carried out. The brain stem and the large veins of this region are better exposed with minimal retraction of the cerebellum and occipital lobe. The transverse sinus is resutured at the end of the procedure to reestablish the blood circulation. The operative procedure is ideal f…

Visual hallucinations on eye closure associated with atropine toxicity. A neurological analysis and comparison with other visual hallucinations.

Visual hallucinations of remarkable intensity began shortly after intravenous atropine and persisted for 11 days. They were present only when the eyes were closed and were associated with heightened dreaming and disturbed sleep. The patient remained lucid and described his experiences to his attendants. Our patient's hallucinations bore some resemblance to hypnagogic hallucinations and this became the basis for the hypothesis that the hallucinations originated in the sleep-dream system of the brain stem. It is speculated that a similar site--a metabolic locus minoris resistentiae may play a pa…

[Ophthalmologic manifestations of the Pierre Robin syndrome. Report of a case of microphthalmia].

The classical description of the Pierre Robin syndrome includes micrognathia, glossoptosis, airway obstruction, and usual presence of a cleft palate. The Pierre Robin syndrome is currently defined as the combination of retrognathia, cleft palate, and respiratory distress. This last is mixed, with a peripheral component due to glossoptosis and a central component due to brain stem immaturity. The main ocular manifestations found in the Pierre Robin syndrome are congenital glaucoma and severe congenital mypopia responsible for retinal detachment. Microphthalmia is infrequent. We report the case …

Trichloroethylene cranial neuropathy: is it really a toxic neuropathy or does it activate latent herpes virus?

The mechanism of the cranial neuropathy associated with heavy exposure to trichloroethylene (or dichloroethylene) is unknown. In severe cases there is destructive spread of the neuropathic process from the Vth cranial nerve nuclei up and down the brain stem in a manner that is difficult to explain on accepted neurotoxicological principles. However, there is a close association reported of this form of trigeminal neuropathy with reactivation of orofacial herpes simplex that suggests the possibility that the chemical, which readily gains entrance into the nervous system, may be responsible for r…

Diffuse axonal swellings in a case of acquired immunodeficiency syndrome.

We report a case of a 25-year-old woman affected by the acquired immunodeficiency syndrome in whom the neuropathologic examination disclosed the presence of diffuse axonal swellings in the brain stem and in selected areas of cerebral hemispheres. These lesions, although microscopically similar to those previously described as focal pontine leukoencephalopathy, differ in that their distribution was wider, there was an absence of associated necrosis of the adjacent nervous tissue, and there was a lack of calcification. It is suggested that axonal swellings represent a nonspecific reaction of the…

Cerebral astrocytoma as a complication of acute lymphoblastic leukaemia.

A boy who developed acute lymphoblastic leukaemia at the age of two years and 10 months had several central nervous system relapses, and received two courses of cranial irradiation, each of 24 Gy, in addition to systemic and intrathecal chemotherapy. Nearly 10 years after the onset of his leukaemia he developed a brain stem astrocytoma from which he died. This appears to be a rare occurrence, and the role of the treatment in the induction of his second tumour is discussed. The need for the long-term follow-up of cancer patients after apparent cure is emphasized.

Laryngeal stridor associated with the Chiari II malformation.

Twelve patients with Chiari II malformation developed laryngeal stridor and respiratory distress. This may develop when there is acute downward or upward displacement of the malformed brain stem secondary to increased or decreased intracranial pressure. Rapidly progressive hydrocephalus and compression of the lower portion of the IV ventricle are more important factors than the size of the head at birth. For the treatment of these symptoms relief of the increase in intracranial pressure, relief of the pressure difference between intracranial and intraspinal compartments, and selection of a shu…

[Radiologic diagnosis of acoustic neurinoma. Apropos of 130 patients].

Diagnostic value of radiologic examinations was assessed in a homogeneous series of 130 cases of neurinoma of acoustic nerve seen over the last 10 years. Radiologic exploration is conceived and used at the present time for two purposes. Firstly, it enables selection of patients with suspected acoustic nerve neurinoma. Radiotomography appears to be currently an accessory examination because of the discriminative performance of early auditory evoked potentials tests on the brain stem. Secondly, radiology is necessary for confirmation of diagnosis. Visualization of the tumoral syndrome can be obt…

[Gastrointestinal hemorrhage in neurosurgical patients].

Analysis of 445 fatal cases among which 21 were marked by profuse gastroduodenal hemorrhage showed that this complication develops in ulceration of the mucous-membrane in the upper parts of the digestive tract and occurs most frequently in affection of the hypothalamic region and brain stem. Gastrointestinal hemorrhages are caused to a certain measure by medication with massive doses of corticosteroids in the stage of tension of the general adaptation syndrome, hypoxia of the mucous membrane, azotemia, and other disorders of homeostasis. Gastrointestinal hemorrhages greatly aggravate the condi…

Eye movements in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is characterized by degeneration of the upper motor neurons in the frontal cortex and the motor neurons of the brain stem and spinal cord. Cranial nerve nuclei III, IV, VI are usually normal histopathologically, as are the eye movements clinically. Using electro-oculography, we found decreased saccadic or smooth pursuit velocities in four of ten patients unidirectional saccadic pursuit in one of these four, and progressively decreasing saccadic velocities in another. We believe the defect is supranuclear and that the ocular motor system is more often abnorma…

Syndromes of vertebrobasilar insufficiency and their possible surgical treatment.

Occlusive disease of the vertebrobasilar system may cause a variety of syndromes depending on the artery or arteries involved and the available collateral circulation. Based on the clinical picture and the angiographic findings, some of the patients may benefit from a gross reconstructive surgical procedure in the vertebral or carotid arteries. When these arteries are not correctable by gross surgical techniques, an occipital artery-posterior inferior cerebellular artery microanastomosis may also prove beneficial. An early clinical diagnosis of the brain-stem and occipital lobes ischemia and a

Inhibition of the central hypotensive effect of clonidine by trazodone, a novel antidepressant.

Trazodone, a non-tricyclic drug with antidepressant properties, diminished the central hypotensive response to clonidine in a dose-dependent manner. Both drugs were infused into the left vertebral artery of chloralose-anaesthetized cats. Intravenously injected trazodone did not reduce the central hypotensive effect of clonidine. The antagonism probably occurs at the level of central alpha-adrenoreceptors in the brain stem, where clonidine is the alpha-agonist and trazodone the alpha-antagonist. Such an antagonism, already known for clonidine and the tricyclic drugs also holds for non-tricyclic…

Congenital trigeminal neuropathy in oculoauriculovertebral dysplasia-hemifacial microsomia (Goldenhar-Gorlin syndrome).

A 2 1/2 year old child with clinical features of Goldenhar-Gorlin syndrome showed diminished pinprick sensation over the right half of the face. After surgery for the cleft lip, the child died. Neuropathological investigations showed agenesis of the right trigeminal nerve and hypoplasia of the right trigeminal brain-stem nuclei. Nosological aspects of the Goldenhar-Gorlin syndrome and previously reported cases of congenital trigeminal anaesthesia in this disorder are discussed. It is suggested that the hypoplasia of the trigeminal nerve is responsible for the diminished facial sensation seen i

Age-Trajectories of Higher-Order Diffusion Properties of Major Brain Metabolites in Cerebral and Cerebellar Gray Matter Using In Vivo Diffusion-Weighted MR Spectroscopy at 3T.

Healthy brain aging involves changes in both brain structure and function, including alterations in cellular composition and microstructure across brain regions. Unlike diffusion-weighted MRI (dMRI), diffusion-weighted MR spectroscopy (dMRS) can assess cell-type specific microstructural changes, providing indirect information on both cell composition and microstructure through the quantification and interpretation of metabolites' diffusion properties. This work investigates age-related changes in the higher-order diffusion properties of total N-Acetyl-aspartate (neuronal biomarker), total chol

Stiffness pulsation of the human brain detected by non-invasive time-harmonic elastography.

Introduction: Cerebral pulsation is a vital aspect of cerebral hemodynamics. Interplay of SOX transcription factors and microRNAs in the brain under physiological and pathological conditions.

Precise tuning of gene expression, accomplished by regulatory networks of transcription factors, epigenetic modifiers, and microRNAs, is crucial for the proper neural development and function of the brain cells. The SOX transcription factors are involved in regulating diverse cellular processes during embryonic and adult neurogenesis, such as maintaining the cell stemness, cell proliferation, cell fate decisions, and terminal differentiation into neurons and glial cells. MicroRNAs represent a class of small non-coding RNAs that play important roles in the regulation of gene expression. Togethe

Examining structure-activity relationships of ManNAc analogs used in the metabolic glycoengineering of human neural stem cells.

This study defines biochemical mechanisms that contribute to novel neural-regenerative activities we recently demonstrated for thiol-modified ManNAc analogs in human neural stem cells (hNSCs) by comparing our lead drug candidate for brain repair, "TProp," to a "size-matched" N-alkyl control analog, "But." These analogs biosynthetically install non-natural sialic acids into cell surface glycans, altering cell surface receptor activity and adhesive properties of cells. In this study, TProp modulated sialic acid-related biology in hNSCs to promote neuronal differentiation through modulation of ce
