A case of bilateral breast infiltration as the initial manifestation of FAB-L3 leukemia during pregnancy is reported. This rare presentation is usually associated with Burkitt's lymphoma or FAB-L3 leukemia. A high suspicion of the presence of hematologic malignancy and appropriate evaluation are indicated in the presence of this clinical manifestation.

A case-control study of 67 cases of breast cancer and 157 controls was conducted to investigate the role of different behavioral, reproductive, and hormonal factors and to develop a unifying indicator of breast cancer risk. The results confirm previous reports of the influence of smoking on the risk of breast cancer. Age at menarche was found to be a risk factor among the premenopausal women. Most of the physicians and residents involved began performing and teaching breast examination to patients in their teens. Thirty patients presenting through the Breast Screening Programme with impalpable breast abnormalities clearly visible on ultrasound, underwent non-invasive ultrasound localization. The patient was scanned so that the ultrasound abnormality lay below the probe and the skin at this site marked with a skin marker. The purpose of this article is to determine whether or not the transverse rectus abdominis musculocutaneous (TRAM) flap procedure is a practical operation for immediate breast reconstruction. Our series reports 128 consecutive patients who underwent immediate breast reconstruction with the TRAM flap from 1985 to 1990. Retrospective analysis was performed for 56 breast masses with mixed cystic and solid sonographic patterns encountered in the last 5 years at Keio University Hospital, with the objective of elucidating their significance. The lesions were classified into two major groups: tumor with cyst (TWC--solid mass with cystic component) and intracystic tumor (ICT--cystic mass with solid component). The value of whole body positron emission tomography using F-18 2-deoxy-2-fluoro-d-glucose in primary work-up and follow-up was prospectively evaluated in 37 patients with primary or metastatic breast cancer. From 20 primary breast masses 15 from 16 malignant and 4 from 4 benign lesions confirmed by biopsy, were detected. In 3 out of 21 patients in correlation to morphologic imaging, respectively biopsy, no metastatic disease was not identified. Generally speaking, whole body positron emission tomography appears to be a suitable diagnostic staging tool in breast cancer.

Applying standardized and sound operative techniques, impaired wound healing following reduction mammoplasty is rare. A postoperative case of pyoderma gangraenosum is reported. This necrotizing skin disorder seemed at first to be a postoperative bacterial infection, but correct diagnosis was finally made histopathologically. The clinical course, etiology, diagnostic findings, and therapy are discussed and a review of the literature is presented.

Human breast cyst fluid (BCF) proteins were evaluated by sodium dodecylsulfate polyacrylamide gel electrophoresis (SDS-PAGE) after protein denaturation in the presence or absence of 2-mercaptoethanol. Both electrophoretic techniques yielded characteristic and reproducible banding patterns and achieved a high degree of resolution. One hundred two patients with mammographically suspicious, nonpalpable lesions underwent stereotactic breast biopsy with a biopsy gun and an automated 14-gauge cutting needle. After biopsy, a localization wire was placed and surgical biopsy performed. Immunoreactive endothelin has been detected in 21 of 43 samples of breast cyst fluid (21 cases; 3.5 +/- 0.6 pmol/l, mean +/- SEM. Other 22 cases; not detectable, less than 0.5 pmol/l). Fast protein liquid chromatographic analysis of the immunoreactive endothelin of pooled breast cyst fluid showed two immunoreactive peaks; one in the void volume and the other in the position of endothelin-1. It is probable that endothelin-1 is produced by the epithelial cells lining breast cysts, but significance of the presence of endothelin-1 in breast cyst fluid remains to be elucidated.

A retrospective study of the records of xeromammographic examinations of 940 patients with invasive carcinoma of the breast diagnosed during an 11-year period in an out-patient facility yielded four cases of invasive papillary carcinoma.
