BACKGROUND: Anterior cervical foraminotomy (ACF) is a motion-preserving technique that directly addresses anterior and lateral pathologies without requiring fusion. Despite its theoretical advantages, there are few reported series in the literature and its long-term impact of spinal alignment and degeneration remains unexplored. This is the first Australian case series reporting our experience with ACF.
METHODS: We retrospectively reviewed 49 patients (79 levels) who underwent ACF. Clinical outcomes were measured using validated scoring systems. Radiological measurements were made from pre- a

OBJECTIVE: Posterior cervical foraminotomy (PCF) is an established motion-preserving alternative to anterior cervical discectomy and fusion (ACDF) for cervical radiculopathy. However, concerns remain regarding the efficacy of PCF in patients with vertically narrow foramina, where decompression in the craniocaudal direction is limited. This study was conducted to evaluate the impact of narrow foraminal height (FH; < 6 mm) on clinical and radiographic outcomes following PCF.
METHODS: This retrospective cohort study reviewed the records of 82 patients who underwent PCF with ≥ 2 years of follow-u

Cervical spine injuries in mixed martial arts athletes present unique challenges, especially regarding return to play after anterior cervical discectomy and fusion. While single-level anterior cervical discectomy and fusion has shown favorable return to play outcomes in collision sports, mixed martial arts involves higher cervical impact forces, increasing the risk of adjacent segment disease, muscle imbalances, and proprioceptive deficits. Return to play following multilevel anterior cervical discectomy and fusion remains controversial, with limited data and no standardized guidelines. Rehabi

The atlantoaxial segment of the cervical spine is unique for its distinctive anatomical features. Given the variation in the anatomy of this region, surgeons must possess a profound knowledge of its anatomy to be able to combine multiple surgical approaches tailored to the specific anatomical variants of the patient's condition. Insertion of C1 lateral mass screws can be technically demanding, particularly in cases where dissection leads to iatrogenic injury of the C1/C2 venous sinus and causes significant bleeding or in patients with dysplastic C1 anatomy or vertebral artery anomalies. PURPOSE: Grisel Syndrome (GS) describes a rare, non-traumatic atlantoaxial rotatory subluxation in children typically associated with upper respiratory tract infection, or otorhinolaryngological surgery. The aim of this study was to systematically review the literature regarding interdisciplinary treatment approaches, consequences of delayed or misdiagnosed GS, and upper cervical deformities resulting from misdiagnosed GS. Additionally, we share our institutional experience with this pathology.
METHODS: We queried our institutional database for patients suffering from GS which were treated in

OBJECTIVES: Traumatic cervical spine injuries in pediatric populations are uncommon, accounting for 1% to 10% of spinal trauma cases. Cervical facet dislocations are particularly rare and usually result from high-energy trauma. This study presents a rare instance of a C5/C6 facet dislocation caused by a low-energy mechanism during judo and discusses management strategies based on a literature review.
METHODS: This work includes a case report of a 15-year-old male with a C5/C6 facet dislocation sustained during judo practice. Imaging confirmed vertebral malalignment and a herniated disc withou

Cervical spondylosis is a broad term that describes pathological degeneration of the cervical spine, leading to axial neck pain, cervical myelopathy, and radiculopathy. Surgical treatments have been developed to address cervical radiculopathy and myelopathy, with the gold standard being anterior cervical diskectomy and fusion (ACDF). However, cervical fusion is associated with many well-known complications, such as pseudarthrosis and adjacent segment disease (ASD). As a result, cervical disk arthroplasty (CDA) was developed as an alternative to fusion for addressing cervical pathology without

STUDY DESIGN: A meta-analysis.
OBJECTIVE: To evaluate the impact of anterior versus posterior surgical approaches on outcomes in MCSM.
BACKGROUND: Multilevel cervical spondylotic myelopathy (MCSM) is the most common cause of spinal cord dysfunction, and the clinical effectiveness of anterior versus posterior surgical approaches remains elusive.
METHODS: A comprehensive search was conducted across electronic databases, including MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials, to evaluate the effects of anterior surgery versus posterior surgery on outcomes, such as Ja

This study investigates the neural electrophysiological and functional effects of cervical spinal ganglia and sympathetic ganglia in cervical vertigo. Twenty-eight rabbits were randomized into superior cervical sympathetic ganglia (SCSG) group (n = 8), inferior cervical sympathetic ganglia (ICSG) group (n = 8) and control groups for both SCSG and ICSG (n = 6 each). Calcium indicator CaMPARI2 was injected into the ganglia. Four weeks later, both SCSG and ICSG groups underwent electrical stimulation followed by UV irradiation. Control groups did not receive electrical stimulation but were also i

BACKGROUND: Chordomas are rare tumors that arise from notochord remnants and are typically located in the axial skeleton. Chordomas arising in the lower cervical spine are rare.
METHOD: We report a case of a 79-year-old man who presented with a lump on the left side of his neck, indicating an unusual presentation of a chordoma in the lower cervical region. This report describes the clinical, radiological, and histological results of this patient who sought medical attention for a left-sided neck mass. Additional diagnostic tests, including immunohistochemistry and biopsy, confirmed the existe

BACKGROUND: In the setting of cervical open-door laminoplasty, the question of whether or not every opened laminar level should be instrumented has not been sufficiently investigated. We postulated that the surgical outcomes of open-door laminoplasty with instrumentation of every second opened level (skip-fixation) might not be inferior to those of laminoplasty with instrumentation of every opened level (all-fixation). The purpose of the present study was to test the noninferiority of laminoplasty with skip-fixation in improving myelopathy at 2 years postoperatively compared with all-fixation.

BACKGROUND AND OBJECTIVES: We present an illustrative case of spontaneous intracranial hypotension (SIH) in the setting of a suspected C1-2 cerebrospinal fluid (CSF) leak that was successfully treated with muscle, collagen, and epidural blood patch. We examined the literature to identify similar cases reporting Cl-2 retrospinal fluid collections identified on imaging in the setting of SIH and quantified the success of targeted treatment to this area despite previous reports that caution about a "C1-2 false localizing sign."
METHODS: A systematic search was performed identifying cases of SIH r

Neural tube defects (NTD), caused by a disturbance in the neurulation process, are easily diagnosed and treated in the early years of life. Although early repair of NTD is advocated, there is lacking information on its natural course. There are only 11 cases, including this one, reported in literature of a cervical meningocele diagnosed and treated in an adult. A 64-year-old male patient was aware of a skin lesion on his neck but had not consulted a physician until 3 years ago when he started experiencing neuropathic pain in his arms. Cervical MRI revealed a posterior arch fusion defect, a syr
