Understanding social inequalities in Zika infection and its consequences: A model of pathways and policy entry-points.

The health consequences of arbovirus infections such as dengue fever (DENV), Chikungunya (CHIKV) and Zika (ZIKV) has in recent years become a public health challenge, due to failure of prevention followed by increased incidence and pronounced social inequality in occurrence and consequences. This motivates a more systematic analysis of the potential mechanisms and pathways that generate these inequalities. We present in the paper a model that delineates five possible mechanisms driving the inequality of ZIKV and congenital Zika syndrome (CZS). They include differential exposure to bad housing

Parental Opioid Abuse: Barriers to Care, Policy, and Implications for Primary Care Pediatric Providers.

Parental opioid use is affecting the physical, developmental, and mental health of the pediatric population nationwide and raises questions of safety when these children remain in the care of opioid-addicted parents. Pediatric providers face many barriers to identifying and caring for children beyond the neonatal period who have been affected by parental opioid abuse both in utero and in the home. These barriers include communication between providers and services, identification of intrauterine exposure, parental opioid abuse screening, and knowledge of child protective services involvement.

Notes from the Field: Expanded Chemoprophylaxis Offered in Response to a Case of Meningococcal Meningitis in an Elementary School - Indiana, 2015.

On December 11, 2015, the Fort Wayne-Allen County (Indiana) Department of Health was notified by a local hospital laboratory of a suspected case of meningococcal meningitis based on Gram stain results of cerebrospinal fluid. The county health department interviewed close family members and friends of the patient to establish an infectious period, timeline of events, and possible exposures. Close medical and household contacts were offered chemoprophylaxis (1). This case was associated with an elementary school. The patient had intermittent, close, potentially face-to-face contact with many stu

Psychological morbidity and caregiver burden in parents of children with Tourette's disorder and psychiatric comorbidity.

OBJECTIVE: To investigate the mental health and caregiver burden in parents of children with Tourette's disorder (TD) compared with parents of children with asthma. METHOD: A cross-sectional cohort survey was conducted at TD and pediatric asthma hospital outpatient clinics over a 6-month period. Main outcome measures were parent mental health (General Health Questionnaire [GHQ]-28) and caregiver burden (Child and Adolescent Impact Assessment) scores. RESULTS: The response rate achieved was 89.7%. Of the parents of children with TD, 76.9% achieved caseness on the GHQ-28 compared with 34.6% of

Nutrition scenario in India--implications for clinical practice.

India has the highest prevalence (and largest share) of malnourished children, low birth weight babies and anaemia levels amongst children and women in the world. Other micronutrient deficiencies (vitamin A and iodine) also constitute serious public health problems. Various government schemes set up to combat these problems have not had the expected impact in reducing malnutrtion or micronutrient deficiencies. This is not only due to low utilisation or inadequacy of the schemes but also due to the population not adopting appropriate behaviours to improve their health and nutrition. Providing c

Research priorities for data science and artificial intelligence in global health: an international consensus exercise.

Applications of data science and artificial intelligence (AI) in global health are expanding, yet research remains fragmented and often misaligned with the needs of low-income and middle-income countries (LMICs). To address this misalignment, we conducted a global research priority-setting exercise using the Child Health and Nutrition Research Initiative (CHNRI) method. 155 research ideas were scored by 51 experts based on feasibility, potential impact on disease burden, paradigm shift potential, implementation potential, and equity. Top-ranked priorities focused on epidemic preparedness, incl…

FIGIJ Advocacy Statement on Female Genital Mutilation/Cutting FGM/C, Endorsed by NASPAG.

Female genital mutilation/cutting (FGM/C) is a global problem affecting millions of girls and women. It involves cutting or removing parts of the external genitalia for no medical reason, often without consent, and has no health benefits. It is deeply rooted in tradition and often undertaken by lay practitioners. It is known to have immediate risks (of hemorrhage, infection, death) and long-term sexual, gynecological, obstetric, urological, and mental health sequelae including post-traumatic stress disorder. Health professionals worldwide need to be aware that girls and women from high-prevale

Perinatal mental health around the world: priorities for research and service development in Africa.

Africa is a diverse and changing continent with a rapidly growing population, and the mental health of mothers is a key health priority. Recent studies have shown that: perinatal common mental disorders (depression and anxiety) are at least as prevalent in Africa as in high-income and other low- and middle-income regions; key risk factors include intimate partner violence, food insecurity and physical illness; and poor maternal mental health is associated with impairment of infant health and development. Psychological interventions can be integrated into routine maternal and child healthcare i…

Validity and reliability of the Early Childhood Caries Perceptions Scale (ECCPS) to assess health beliefs related to early childhood caries prevention among primary caregivers of children under 5 years of age.

Primary caregivers' child oral health care beliefs and practices are major factors in the prevention of Early Childhood Caries (ECC). This study assessed the validity and reliability of a newly-developed scale--the Early Childhood Caries Perceptions Scale (ECCPS)--used to measure beliefs regarding ECC preventive practices among primary caregivers of young children. The ECCPS was developed based on the Health Belief Model. The construct validity and reliability of the ECCPS were examined among 254 low-socioeconomic status primary caregivers with children under five years old, recruifed from 4 B

Somatic consequences of violence against women.

The rapidly growing literature on the somatic, nonpsychiatric effects of violence on women's health is reviewed, including rape, battery, and the adult consequences of child sexual abuse. The sequelae of these victimizations are summarized with consideration of acute effects (genital and nongenital injuries, sexually transmitted disease, and pregnancy), late consequences (chronic pelvic pain and other forms of chronic pain, gastrointestinal symptoms, premenstrual symptoms, and negative health behaviors), and long-term increases in the use of medical services. A recurrent theme across the liter

Can trust in healthcare be restored after armed violence? Evidence from a randomized field experiment in north-eastern Nigeria.

Armed violence undermines public health not only by destroying infrastructure but also by eroding trust in government institutions, including the health system. Prior research shows that victimized individuals often withdraw from public health services, with particularly adverse consequences for child health. Whether such distrust can be restored, however, remains an open question. We address this question in northeastern Nigeria, a region heavily affected by armed violence, through a randomized field experiment embedded in a representative household survey of caregivers of young children (n =…

EEG activation in preschool children: Characteristics and predictive value for current and future mental health status.

BACKGROUND: Previous research has characterised EEG changes associated with resting activation in primary school children and adults, while task-related activation has only been considered in adults. The current study characterises physiological activation in preschool children and examines the potential value of activation indices for predicting mental health status at two time points. AIMS: To investigate how resting activation and task-related activation are represented in 4- to 5-year-old preschool children and examine if these activation indices can predict current and future mental heal

Causes and differentials of childhood mortality in Iraq.

BACKGROUND: Limited information is available in Iraq regarding the causes of under-five mortality. The vital registration system is deficient in its coverage, particularly from rural areas where access to health services is limited and most deaths occur at home, i.e. outside the health system, and hence the cause of death goes unreported. Knowledge of patterns and trends in causes of under-five mortality is essential for decision-makers in assessing programmatic needs, prioritizing interventions, and monitoring progress. The aim of this study was to identify causes of under-five children death

Parenting interventions to support parent/child attachment and psychosocial adjustment in foster and adoptive parents and children: A systematic review.

BACKGROUND: Adopted children and children placed in foster care are at increased risk of developing a range of mental health, behavioural, and psychosocial adjustment problems. Previous studies suggest that due to early experiences of separation and loss some children may have difficulties forming a secure attachment relationship with the adoptive/foster parents. OBJECTIVES: The objectives of the present review were: (1) to assess the efficacy of attachment-based interventions on measures of favourable parent/child outcomes (attachment security, dyadic interaction, parent/child psychosocial a

Assessing the Respect of Children's Rights in Pediatric Hospitals.

Background and Objectives: In 1989, the United Nations (UN) General Assembly adopted the United Nations Convention on the Rights of the Child (UNCRC), with a considerable number of the Articles of the Convention being related to the health status of children. Therefore, adhering to and assessing the implementation of the rights of children during hospitalization is a very important step towards child protection. Herein, we attempt to highlight the depth of knowledge of employees working in children's hospitals with regard to children's rights as well as the degree of adherence to the UNCRC wit

Primary hydatid cyst of the small intestine masquerading as intestinal duplication in a child.

Hydatid disease or hydatidosis is a worldwide zoonosis disease caused by the tapeworm of Echinococcus granulosus and still widely endemic in Tunisia especially in rural areas where the sheep-dog cycle is dominant. It is an important public health problem in the pediatric age group causing significant morbidity and mortality. We report a case of primary hydatid cyst of the small intestine in a child and we want to highlight the difficulty that we meet in the diagnosis despite the contribution of imaging.

[Neonatal subgaleal haemorrhage causing fatal course after vacuum-assisted extraction].

We present a case that emphasizes the importance of early recognition of neonatal subgaleal haemorrhage through knowledge of risk factors and adequate communication within the health team treating the woman in labour. Especially vacuum-assisted extraction of the child significantly multiplies the incidents of subgaleal haemorrhage. The high mortality is directly associated with disseminated intravascular coagulation and bleeding. Early monitoring and treatment with replacement of blood volume is crucial. Education concerning this condition is therefore of great importance.

Ocular tuberculosis misdiagnosed as retinoblastoma: an interesting case.

Tuberculosis is a ubiquitous disease and a public health problem of major importance in almost all developing and underdeveloped countries. It can involve any part of the body, including the eye. We report a case of a young child presenting with a painful blind eye with leucocoria, where a misdiagnosis of retinoblastoma was made clinically. The eye was enucleated, and subsequent pathological examination led to a final diagnosis of ocular tuberculosis. The patient was put on anti-tubercular treatment, and responded well.
