Patterns of Online Medical Direction Utilization in an Urban EMS System: A Retrospective Analysis of 9,944 Consultations.

OBJECTIVES: Emergency medical services (EMS) clinicians primarily rely on standing protocols for prehospital care but may seek real-time physician guidance through online medical direction (OLMD) when situations fall outside established protocols. This study aimed to characterize patterns of OLMD consultation, including indications, timing, and physician decision-making within an urban EMS system and identify potential opportunities for protocol refinement. METHODS: We used an observational study design and retrospective descriptive analysis of 9,944 OLMD consultation records from July 2016 t

Should I Target the Blood Pressure from the Arterial Line or the Cuff? A Practical Approach for Dealing with Widely Discordant Measurements.

Blood pressure (BP) monitoring is essential in managing critically ill patients in the intensive care unit (ICU), particularly for ensuring … physiologic contributors before deciding which value should guide treatment. We present a stepwise clinical decision-making algorithm that helps practitioners (1) recognize when a discrepancy is large enough to matter, (2) evaluate perfusion using bedside and laboratory markers, (3) identify technical or anatomic reasons for discordant readings, and (4) determine when mo …

How much are radiological parameters related to clinical symptoms and function in osteoarthritis of the shoulder?

Loss of joint space, formation of osteophytes and deformation are common features of osteoarthritis. Little information exists about the radiological features of arthrosis in relation to clinical findings and the radiological appearance in degenerative shoulder joint disease especially with regard to decision making about the timing of joint replacement. We retrospectively examined 120 standardised X-rays of patients with advanced osteoarthritis of the shoulder. Exclusion criteria included rotator cuff tear, severe glenoid erosion or protrusion. Measurements of joint space width at three level

Neural signatures of risk-taking adaptions across health, bipolar disorder, and lithium treatment.

Cognitive and neural mechanisms underlying bipolar disorder (BD) and its treatment are still poorly understood. Here we examined the role of adaptations in risk-taking using a reward-guided decision-making task. We recruited volunteers with high (n = 40) scores on the Mood Disorder Questionnaire, MDQ, suspected of high risk for bipolar disorder and those with low-risk scores (n = 37). We also recruited patients diagnosed with BD who were assigned (randomized, double-blind) to six weeks of lithium (n = 19) or placebo (n = 16) after a two-week baseline period (n = 22 for FMRI). Participants comp

Step-by-step causal analysis of EHRs to ground decision-making.

Causal inference enables machine learning methods to estimate treatment effects of medical interventions from electronic health records (EHRs). The prevalence of such observational data and the difficulty for randomized controlled trials (RCT) to cover all population/treatment relationships make these methods increasingly attractive for studying causal effects. However, researchers should be wary of many pitfalls. We propose and illustrate a framework for causal inference estimating the effect of albumin on mortality in sepsis using an Intensive Care database (MIMIC-IV) and comparing various s
