What should be done with antisocial personality disorder in the new edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-V)?

Antisocial personality disorder, psychopathy, dissocial personality disorder and sociopathy are constructs that have generally been used to predict recidivism and dangerousness, alongside being used to exclude patients from treatment services. However, 'antisocial personality disorder' has recently begun to emerge as a treatment diagnosis, a development reflected within cognitive behaviour therapy and mentalisation-based psychotherapy. Many of the behaviour characteristics of antisocial personality disorder are, at the same time, being targeted by interventions at criminal justice settings. A

Consensus criteria for the diagnosis of frontotemporal cognitive and behavioural syndromes in amyotrophic lateral sclerosis.

Amyotrophic lateral sclerosis (ALS) is increasingly recognized to be a multisystem disorder which includes both clinical and neuropathological features of a frontotemporal lobar degeneration (FTLD). In order to provide a common framework within which to discuss the characteristics of the cognitive and behavioural syndromes of ALS, and with which to conduct clinical and neuropathological research, an international research workshop on frontotemporal dementia (FTD) and ALS was held in London, Canada in June 2007. The recommendations arising from this research workshop address the requirement for

Visuospatial memory in healthy elderly, AD and MCI: a review.

In the literature it is commonly reported that several spatial abilities decline with normal aging, even though such a decline is not uniform. So far, it is not yet clear which spatial components present a normal age-related decline, which ones are preserved and at what point the deficit is so severe to represent an index of mild cognitive impairment (MCI) or a symptom of potential degenerative progression as in the early-stage Alzheimer's disease (AD). In particular, AD (from early onset) is characterised by impairments in constructive abilities, visuospatial intelligence, spatial short-term

Reversible parkinsonism in a patient with progressive multifocal leucoencephalopathy.

A case of pathologically confirmed progressive multifocal leucoencephalopathy presenting with unilateral parkinsonism and cognitive decline that significantly improved over a 12-month period without any treatment is described. The patient had a background of chronic lymphocytic leukaemia, but had been in complete remission for 4 years at the time of diagnosis. This case is highly unusual not only in terms of the mode of clinical presentation in an apparently immunocompetent patient but also in that the patient spontaneously improved without any intervention. Progressive multifocal leucoencepha

Cerebrovascular disease and dementia.

Cerebrovascular disease is a major contributor to later-life dementia, accounting for up to 20% of cases of dementia. Atherosclerotic and arteriolosclerotic mechanisms account for most of the burden of disease. Cerebrovascular disease may take several forms. Macrovascular disease in the form of large vessel and larger arteriole infarcts produce a wide spectrum of clinical syndromes. Single strategic infarctions, multiple bilateral infarctions and multiple lacunar infarctions can lead to cognitive dysfunction that spans a large range of both severity and type of cognitive deficits. [Higher brain dysfunction--from the administrative viewpoint].

The "higher brain dysfunction" illustrates various cognitive and behavioral consequences resulted from organic brain damage--attention deficits, memory disorders, executive dysfunction, and disorders of emotion and social communication. These deficits are hard to be visible externally, so patients with such "higher brain problems" are difficult to be appropriately evaluated and treated even in psychiatry service. The patients who have reached to a chronic phase and whose treatments are terminated have been able to live some stable life at home, but exhibit many difficulties in social life, so

Internet-guided self-help with or without exposure therapy for phobic and panic disorders.

BACKGROUND: As many sufferers from phobic and panic (phobia/panic) disorders cannot get to suitable therapists, routine aspects of therapy were delegated to internet-accessed computer-aided self-help with or without exposure instructions. METHODS: Phobia/panic referrals were randomised to computer-aided self-help via the internet at home in a 2:1 ratio either by self-exposure cognitive behaviour therapy (CBT) [FearFighter (FF), n = 45] or by minimal CBT without exposure [Managing Anxiety (MA), n = 23]. All had brief backup phone advice from a clinician concerning their computer guidance. RES

Understanding sexual offending in schizophrenia.

BACKGROUND: Studies have found an elevated incidence of violent sexual offences in males with schizophrenia. The relationship between sexual offending and psychiatric illness is, however, complex and poorly defined. AIMS: The aim of the present article is to delineate possible mechanisms that underlie offensive sexual behaviour in schizophrenia that can be used as a framework for assessing and treating these behaviours. A review of research pertaining to the aetiology of sexual deviance in schizophrenia was conducted, focusing in particular on the role of early childhood experiences, deviant

Posterior cortical atrophy: a review of the literature.

The past three decades have seen an accumulation of reports of neurodegenerative disease with disruption to visual processing. Disorders of both visuospatial processing and visual recognition have been described, though the former predominate. But the conundrum of posterior cortical atrophy (PCA) is that while the clinical presentation is relatively homogeneous, the nosological status remains something of a puzzle. Many cases coming to pathology have had Alzheimer's Disease (AD) but in a different distribution from that seen in typical AD (tAD). Patients with non-specific subcortical gliosis a

Differences in cognitive distortions between problem and social gamblers.

The cognitive model of gambling argues that irrational beliefs and erroneous perceptions including illusions of control, superstitious beliefs, expectancies of winning, attributional biases, selective memory, and entrapment play a pivotal role in the development and maintenance of impaired control over patterns of gambling behaviours. Assessment strategies using self-report, behavioural inferences, and verbalizations produced by 'thinking aloud' techniques, and the effectiveness of treatment interventions designed to correct cognitive distortions have provided substantive empirical evidence su

[Alzheimer's disease and depression].

Alzheimer's disease is the most frequent cause of dementia (60% of all dementias) and affects nearly 300,000 people in France. Alzheimer's disease is a disease of the elderly which generally begins after 60 years and whose prevalence increases markedly after age 75 years. The elderly population is increasing in all Western countries. Alzheimer's disease thus constitutes a veritable emergent public health problem. The rapid inflation of the epidemiological and etiopathogenetic data have contributed to enhanced nosographic definition and finer semiological characterization of the disease. Thus,

Schizophrenia--a disorder of the corollary discharge systems that integrate the motor systems of thought with the sensory systems of consciousness.

BACKGROUND: In spite of intensive research, no causal anatomical lesion has been found in schizophrenia. It may instead be caused by malfunctioning circuits in the corollary discharge, feed forward (CD-FF) systems of thought. AIMS: To integrate with the CD-FF hypothesis recent data showing that subcortical motor systems participate in thinking. METHODS: We review CD-FF concepts in relation to recent evidence that 'motor' brain structures participate in cognitive processing. RESULTS: Malfunctioning of CD-FF systems that integrate thinking and consciousness could produce auditory hallucinatio

A dual-task analysis of resource allocation in dysthymia and anhedonia.

Research has shown dysthymic individuals to be hyporesponsive at various stages of information processing, yet it is not clear whether dysthymics are deficient in the amount of available attentional resources for information processing or, instead, in the allocation of those resources. To distinguish between these possibilities, the authors compared dysthymics to anhedonic and normal control Ss during the performance of memory tasks, under conditions of varying task priority and difficulty. Although there were no performance differences, dysthymics and anhedonics exhibited a consistently small

Identification and management of cognitive and perceptual deficits in the rehabilitation patient.

The rehabilitation nurse frequently encounters patients who display cognitive/perceptual impairments as a result of a head injury or cerebrovascular accident. Deficits in attention, perception, memory, sequencing, and initiation may prevent the patient from reaching goals set by nurses or other rehabilitation professionals. When nurses recognize cognitive or perceptual impairments and have techniques available that will compensate for such deficits, they can more readily help patients meet rehabilitation goals.

Hallucinations in the months after a trauma: An investigation of the role of cognitive processing of a physical assault in the occurrence of hallucinatory experiences.

The role that cognitive processing of a recent trauma has in the occurrence of hallucinations has not been examined longitudinally. This study investigated trauma-related cognitive predictors of hallucinations in the months following an interpersonal assault. Four weeks after treatment at an emergency department for interpersonal assault injuries, 106 participants were assessed for peri-traumatic cognitive processing, cognitive responses to trauma memories, negative beliefs about the self, Posttraumatic-stress disorder (PTSD), and hallucinatory experiences. Hallucinatory experiences were reass

Cognitive Complaints and Comorbidities in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is highly prevalent sleep disorder that causes sleep fragmentation, frequent awakenings, and intermittent hypoxia. Both OSA and cognitive decline increase in prevalence with factors such as increasing age and body mass. Multiple areas of cognition can be affected, including attention, executive function, memory, as well as emotional functioning through direct effects on brain health. Although positive airway pressure therapy has shown to improve some aspects of cognitive functioning, it does not fully alleviate all cognitive complaints. Inclusion of complementary

Cognitive Impairment in Multiple Sclerosis.

Multiple sclerosis (MS) is a socially significant immune-mediated disease, characterized by demyelination, axonal transection and oligodendropathy in the central nervous system. Inflammatory demyelination and neurodegeneration lead to brain atrophy and cognitive deficit in up to 75% of the patients. Cognitive dysfunctions impact significantly patients' quality of life, independently from the course and phase of the disease. The relationship between pathological brain findings and cognitive impairment is a subject of intensive research. Summarizing recent data about prevalence, clinical specifi
