Benign lipoblastomalike tumor of the vulva: report of a case affecting a young patient.

BACKGROUND: Mesenchymal vulvar tumors are rare, and a benign lipoblastomalike tumor may be confused with malignancy. CASE: A 15-year-old patient consulted for a right labium tumefaction measuring 15 cm. After excision, histologic examination revealed a well-circumscribed and lobulated tumor. The lobules were composed of slender spindle cells showing slightly eosinophilic cytoplasm with indistinct boundaries, uniform nuclei with finely granular chromatin and no nucleoli. Scattered Signet-ring-type lipoblasts were observed. The background was myxoid, with a "chickenwire" capillary vascular netw

Strangulation of a prolapsed rectal cancer in labor: a case report.

BACKGROUND: Colorectal cancer during pregnancy is uncommon. Most patients present in late pregnancy, and the tumor is localized to rectum in up to 85% of cases. Delayed diagnosis due to confusing significant lower gastrointestinal symptoms with pregnancy-associated gastrointestinal changes is a common feature. From the increasing intraabdominal pressure during delivery, a tumor can prolapse throu the anus and develop incarceration and strangulation, but that is seen a extremely rarely, CASE: A 33-year-old woman was found to have a prolapsing rectal cancer through the anus during delivery, and

[Vesical schistosomiasis: an uncommon cause of bladder wall thickening].

Schistosomiasis is typically a tropical disease; however, given the large influx of immigrants to our country and the boom in tourism to exotic locations, schistosomiasis should be taken into account in the differential diagnosis of certain clinical entities. The flatworm Schistosoma haematobium can cause a large variety of lesions in the urinary tract. The most common sites of involvement are the bladder and the lower third of the ureters, where it can cause lesions ranging from lithiasis and ureteral stenosis to the formation of masses inside the bladder. Ultrasonography is the imaging techn

Calcific retropharyngeal tendinitis: a frequently missed diagnosis. Case report.

Neck pain is a common presentation in emergency departments that requires the exclusion of serious underlying pathological entities. Acute calcific retropharyngeal tendinitis is a rare and underrecognized cause of atraumatic neck pain and stiffness. Although this entity has been reported previously, it is often poorly recognized and is often confused with other more serious lesions, such as infectious, traumatic, and neoplastic conditions. Clinically, it can be very difficult to distinguish from these three disorders, and a definitive diagnosis can only be established using imaging studies. A

Reclassification of a tubal leiomyosarcoma as an eGIST by molecular evaluation of c-KIT.

BACKGROUND: Extragastrointestinal stromal tumors (eGISTs) are rare mesenchymal-derived tumors arising outside of the GI tract. eGISTs are often histologically confused with leiomyosarcoma. Distinction between eGIST and leiomyosarcoma is critical because of the unique responsiveness of eGISTs to the molecularly targeted agent imatinib. CASE: A woman presented with a history of tubal spindle cell tumor that was initially diagnosed and treated as a leiomyosarcoma. Because of minimal response to sarcoma directed chemotherapy, the possibility that the tumor was in fact an eGIST was investigated an

[Stab injury of the brain by kitchen knife].

A 32-year-old male attempted suicide by stabbing his forehead with a kitchen knife, and was sent to our hospital. On admission, he was confused (20-30/JCS), but was able to move his both extremities. Skull X-p, CT scan indicated that the kitchen knife penetrated the frontal bone, reached the cerebrum by way of the frontal sinus. Cerebral angiography was performed, and showed no vascular lesions. Emergent craniotomy was performed and the kitchen knife was removed with minimum movement. Traumatic intracerebral hematoma was removed, and injured frontal sinus, dura mater and frontal bone were repa

[Gastric Crohn's disease as a cause of gastrointestinal bleeding after gastroscopy--diagnostic difficulties].

Crohn's disease is a chronic, idiopathic inflammatory bowel disease localized most commonly in the terminal ileum, but it may involve any site of the gastrointestinal tract. Stomach location is extremely rare. Usually this process is confused by the presence of fistulas and intraabdominal abscesses in the typical emplacement, which are the consequences of penetration of the inflammatory process through the whole thickness of the large intestine's wall. In the case of ithe upper part of the gastro-intestinal tract location (commonly distal part of stomach or proximal of duodenum) bleeding is th

Inflammatory pseudotumor of the nasal cavity.

Inflammatory pseudotumor is usually found in the orbits and lungs, but rarely in the sinonasal area. We present a 59-year-old woman with a right nasal mass. This lesion caused nasal bleeding and blockage. Image study showed that the solid mass eroded the bony structure and pushed the nasal septum toward the left side. Wide excision was done via lateral rhinotomy. Although its clinical picture mimicked a malignant tumor, histological examination showed an inflammatory process composed of a mixture of lymphoplasmacytic infiltrate, histiocytic cells, and spindled fibroblastic/myofibroblastic cell

Henoch-Schönlein purpura in an adult Filipino man: a case report and literature review.

Henoch-Schönlein purpura (HSP) is a form of cutaneous small vessel vasculitis that can involve visceral organs and is associated with deposition of immunoglobulin A (IgA)-containing immune complexes. HSP may appear after a remote history of infection (often an upper respiratory tract infection) as a rash with palpable petechiae or purpura. In addition, a patient with HSP usually complains of arthralgia and abdominal pain. Renal involvement also is common. HSP may be confused with other systemic autoimmune diseases because they all can present with similar symptoms. Prognosis is good and recove

Renal cell carcinoma metastatic to the choroid mimicking intraventricular meningioma.

BACKGROUND: Metastatic renal cell carcinoma to the choroid plexus is a rare condition and can be easily confused with meningioma. METHODS: A 61-year-old female presented with progressive neurologic deterioration and MRI findings of obstructive hydrocephalus and a homogeneously contrast enhancing 3 cm oval mass in the trigone of the left lateral ventricle. RESULTS: Despite radiologic, intraoperative gross features, and frozen pathology all consistent with meningioma, the final pathology revealed metastatic renal cell carcinoma. CONCLUSION: Renal cell carcinoma metastatic to the choroid plexu

Testicular torsion revisited.

Ultrasound is routinely used for the assessment and diagnosis of testicular torsion. It has proven to be a reliable and necessary diagnostic tool for the urologist. Problems, however, can arise in the form of misdiagnosis when the basic pathophysiology and clinical presentation of testicular torsion are not clearly understood. In this pictorial review, we present the spectrum of the appearance of testes with testicular torsion. This includes both complete and incomplete torsion. The critical use of Doppler and the interpretation of the Doppler findings will be presented. Other abnormalities th

Granulocytic sarcoma of the small intestine in a child without leukemia: report of a case with cytologic findings and immunophenotyping pitfalls.

BACKGROUND: Granulocytic sarcoma is a rare tumor that is often misdiagnosed as it can be confused with lymphoma. It has unique cytologic features independent of the site of the tumor and can be identified on fine needle aspiration. CASE: A 13-year old girl without a relevant medical history presented with an abdominal mass. Investigation revealed a tumor infiltrate in the small intestine and mesentery. The fine needle aspirate contained myeloid blasts with cytoplasmic granules. Immunohistochemistry on subsequent biopsy confirmed myeloid differentiation. There was no evidence of blood or bone

Fine-needle aspiration of Riedel's disease: report of a case and review of the literature.

The cytomorphological features of a case of Riedel's thyroiditis (Riedel's disease) in a 37-yr-old woman are reviewed. The patient presented with a diffusely enlarged thyroid gland with extension to carotid and jugular vessels bilaterally. A fine-needle aspiration of the right lobe of the thyroid demonstrated moderate cellularity with fragments of fibrous tissue with bland spindle-shaped cells and myofibroblasts. The patient subsequently underwent a bilateral subtotal thyroidectomy with removal of two-thirds of both lobes of the thyroid. A frozen section diagnosis of Riedel's disease was later

Differential diagnosis of Charcot-Marie-Tooth disease and related neuropathies.

The diagnosis of Charcot-Marie-Tooth disease (CMT) and related neuropathies (e. g. Déjèrine-Sottas disease; hereditary neuropathy with liability to pressure palsies) appears to be easy. However, the incredible advances in molecular genetics have greatly complicated the classification of these disorders, and the proper diagnosis of the CMT subtype may be important for correct genetic counselling and prognosis. Moreover, these diseases may be confused with potentially treatable acquired and inherited neuropathies, such as dysimmune neuropathies, familial amyloid polyneuropathy, and Refsum's dise

Sexually transmitted diseases in the HIV care setting: what's really going on down there?

Syphilis epidemics are occurring in a number of cities in the United States and worldwide. In New York City, the number of cases primary or secondary syphilis has increased from 130 in 1999 to 434 in 2002. The majority of new cases occur in men, with most of these occurring in men who have sex with men. The presentations of primary and secondary syphilis are varied and can be overlooked or confused with other diseases. Health care providers should be aware of atypical presentations of sexually transmitted diseases (STDs), screen for nonapparent STDs, and know how to treat such diseases once th

Chronic short-lasting headaches: clinical features and differential diagnosis.

Chronic short-lasting headaches are relatively rare, poorly recognized syndromes. The chronic short-lasting headaches are a group of chronic disorders in which the headaches occur for one year or more, either without remission or with remission periods less than two weeks; each attack lasts less than 4 hours. These headache syndromes are divided into those with autonomic activation and those without autonomic activation. This article presents the clinical features and differential diagnosis of the relatively rare and clinically challenging trigeminal autonomic cephalgias (TACs). The article al

Talc pleurodesis mimics pleural metastases: differentiation with positron emission tomography/computed tomography.

Talc pleurodesis is a technique used in the treatment of patients with persistent pleural effusions or pneumothorax not amenable to other treatment. These are commonly seen in patients with malignant thoracic neoplasms. Radiographic abnormalities resulting from prior talc pleurodesis could be confused with progression of the underlying neoplastic process. Positron emission tomography with F-18 fluorodeoxyglucose (FDG-PET) might be unable to distinguish between malignant and benign inflammatory processes. This report demonstrates the use of combined positron emission tomography/computed tomogra

[Neuralgia of the trigeminal nerve as first symptom of a primary central nervous system lymphoma of non-Hodgkin's type].

Primary central nervous system lymphoma of non-Hodgkin's type are distinguished from systemic non-Hodgkin's lymphoma because of their different pathobiological behaviour. Predisposing factors are pharmacologically induced immunosuppression and those referring to diseases. We present the case of a patient who was sent to hospital in case of emergency due to pain of one half of the face. The symptoms corresponded essentially to neuralgia of the trigeminal nerve. During course other neurological symptoms showed up. The right diagnoses of a primary central nervous system lymphoma of non-hodgkin's

Endometrial stromal sarcoma of the sigmoid colon arising in endometriosis: a case report with a review of literatures.

Most of malignant tumors arising in ovarian and extraovarian endometriosis are carcinomas. Mixed mullerian tumor and endometrial stromal sarcoma arising in intestinal endometriosis are rarely described, but its clinicopathologic features have not been well characterized. Here we report a case of endometrial stromal sarcoma of the sigmoid colon arising in endometriosis with a review of six additional cases of endometrial stromal sarcoma arising in intestinal endometriosis found in English literatures. The patients ranged in age from 36 to 64 yr. Presenting symptoms were pain, bloody diarrhea, a

Chromosomal imbalances in meningeal solitary fibrous tumors.

We present the results of a comparative genomic hybridization (CGH) analysis of three meningeal solitary fibrous tumors (SFT). One case showed loss of chromosome 3 and two tumors had deletions of the region 3p21-p26. Other chromosomal losses included 4p15, 8q22-q24, 10, 11q14-q25, 17q11- q23, 20, and 21 in one case each. In addition, there were gains of 18p11-p13 in one case, and 1p11-p36 and 20q11-q13 in another. To our knowledge, there are no previous CGH or cytogenetic data on meningeal SFT, and loss of material on chromosome 3 has not been described in SFT at other sites. Our findings are
