BACKGROUND: Fracture-dislocation of the midfoot with collapse of the longitudinal arch is common in patients with neuropathic arthropathy of the foot. In this study, we describe a technique of midfoot arthrodesis with use of intramedullary axial screw fixation and review the results and complications following use of this technique.

The term "failure of differentiation" describes the phenotypes of a large number of otherwise unrelated conditions. The six conditions described here (arthrogryposis, camptodactyly, clinodactyly, Madelung deformity, trigger finger, and trigger thumb) are believed to occur because various structures failed to differentiate normally; however, they have neither common features nor a common cause. We have included information about the history and diagnosis of these conditions, the cause (if known), and the current concepts of treatment and expected outcomes.

Dupuytren's disease predominantly occurs among Europeans, with higher reported prevalence in Northern than in Mediterranean European countries. We evaluated the recurrence and extension rates among Caucasian patients from Primorsko-goranska County in Croatia who underwent partial fasciectomy as treatment for Dupuytren's disease. The Pirani scoring system, together with the Ponseti method of club foot management, was assessed for its predictive value. The data on 70 idiopathic club feet successfully treated by the Ponseti method and scored by Pirani's system between February 2002 and May 2004 were analysed. Juvenile hyaline fibromatosis (JHF) is a rare autosomal recessive disease of the connective tissue. Dupuytren's disease (DD) is a common progressive fibrotic condition affecting the palmar and digital fascia. Although its management is undertaken by hand surgeons, it is commonly seen by other doctors as an incidental finding. In many cases it is believed to be associated with other medical conditions, although the evidence for such associations is not always clear. This review considers the evidence behind these associations and discusses the aetiology of DD. By doing so, it is hoped that this review will permit a better understanding of the relevance of DD as a clinical sign.

We compared the incidence of significant Dupuytren's disease in men across occupational social classes in England and Wales, using data from the National Morbidity Survey. We found that manual occupational social class was not associated with an increased incidence of Dupuytren's disease. In fact, the incidence rates of Dupuytren's disease in the elderly were higher in non-manual than in manual social classes.

We studied 135 hands in 77 Japanese patients to assess the frequency of radial involvement and its association with recurrence and the Dupuytren's diathesis. Radiographic spinal evaluations were done on 305 patients with myelodysplasia. Asymptomatic spondylolisthesis, which averaged 37.1% translation, was seen in 5.9% of the patients. The prevalence of spondylolisthesis increased to 15.6% of patients with L5-S1 motor levels. Patients with myelodysplasia do develop spondylolisthesis. Increased lumbar lordosis and concomitant diminished thoracic kyphosis were commonly seen in the patients with spondylolisthesis.

The present status of adjudication for workers claiming compensation for Dupuytren's disease is inconsistent and, therefore, unfair to both workers and employers. In a 50-year-old patient with unilateral pain in the buttocks and in the sciatica, a radicular compression syndrome was diagnosed. Because conservative therapy failed, surgical intervention for herniated disc was suggested. Clinical examination revealed a painfully contracted piriform muscle and irritation of the sciatic nerve. Through specific stretch exercises, quick recovery was effected. Based on this case report, typical clinical diagnoses and therapeutical possibilities for the piriformis syndrome are discussed.
