3D modelling and x-ray depth analysis map of the pulp with computer software via digital periapical radiography and cone beam computed tomography.

OBJECTIVE: Periapical radiographs (PAR) offer information about the pulp and periodontal health of teeth. However, intraoral radiographs are insufficient for diagnosing buccolingual anomalies and variations such as bifid canals due to their two-dimensional nature. Cone beam computed tomography (CBCT) is the gold standard for 3D imaging in the clinic but requires additional radiation. The aim of this study was to create a software (XPAR) which obtains x-ray depth analysis and 3D modelling of the pulps of single-rooted teeth by converting the grey values in the original radiographs into numerica

Reducing the prosthesis modulus by inclusion of an open space lattice improves osteogenic response in a sheep model of extraarticular defect.

Introduction: Stress shielding is a common complication following endoprosthetic reconstruction surgery. The resulting periprosthetic osteopenia often manifests as catastrophic fractures and can significantly limit future treatment options. It has been long known that bone plates with lower elastic moduli are key to reducing the risk of stress shielding in orthopedics. Inclusion of open space lattices in metal endoprostheses is believed to reduce the prosthesis modulus potentially improving stress shielding. However, no in vivo data is currently available to support this assumption in long bon

Effects of teriparatide and loading modality on modeling-based and remodeling-based bone formation in the human femoral neck.

PURPOSE: We have previously shown that a brief course of teriparatide (TPTD) stimulates bone formation in the cancellous and endocortical envelopes of the human femoral neck, and the regions of tension and compression respond differently. The purpose of the present study was to determine how much of the new bone was formed by modeling-based formation (MBF) or remodeling-based formation (RBF). METHODS: We performed a double-blind trial of TPTD vs. placebo (PBO) in patients about to undergo a total hip replacement (THR) for osteoarthritis. Participants were randomized to receive daily TPTD 20 μ

Diaphyseal and Metaphyseal Modeling Defects-Clinical Findings and Identification of WRAP53 Deficiency in Craniometadiaphyseal Dysplasia.

Background: Diaphyseal and metaphyseal modeling defects lead to severe changes in bone mass and shape, which are common features in osteoporosis that linked to non-vertebral fractures. Original mechanism of diaphyseal and metaphyseal modeling defects has proved elusive. Studying rare syndromes can elucidate mechanisms of common disorders and identify potential therapeutic targets. Methods: We evaluated a family pedigree with craniometadiaphyseal dysplasia (CRMDD, OMIM 269300), a genetic disorder that is characterized by cortical-bone thinning, limb deformity, and absent of normal metaphyseal f

Radial bending breaking resistance derived by densitometric evaluation predicts femoral neck fracture.

Bone mineral density (BMD) measurement by hip dual-energy X-ray absorptiometry (DXA) is considered the best predictor of osteoporotic fracture risk. BMD takes into account only in part the bone cross-sectional area that is an important determinant of both bone compression strength and of bending breaking resistance. From DXA measurements of proximal radius (Osteoplan, NIM, Verona, Italy) we obtained the projected outer diameter (D) and the mean diameter of the medulla (d), by an algorithm based on the assumption of a constant cortical volumetric density of 1050 g/cm3. The algorithm was validat

From Diagnosis to Treatment: Challenges in Scaphoid Imaging.

The scaphoid is the largest and most significant bone in the proximal carpal row and is essential for wrist biomechanics and stability. Its intricate anatomy and unique blood supply render it vulnerable to overlooked fractures and postfracture complications, often making for an exceptionally challenging radiologic appraisal. Radiographic diagnosis of a scaphoid fracture is particularly difficult because of the bone's complex geometric structure. Its numerous sulci, apices, ridges, and facets can resemble fractures, which may lead to unnecessary immobilization. A significant portion of the scap

Differential effects of romosozumab across central and peripheral skeletal sites: a prospective DXA and HR-pQCT study in high-risk postmenopausal osteoporosis.

INTRODUCTION: Romosozumab is known to markedly increase areal BMD (aBMD) at the LS and proximal femur assessed by DXA; however, its effects on peripheral volumetric BMD (vBMD) and bone microarchitecture measured by HR-pQCT remain incompletely characterized in postmenopausal women at very high fracture risk. MATERIALS AND METHODS: In this single-center, prospective longitudinal observational study, postmenopausal women with primary osteoporosis and a history of fragility fractures received romosozumab for 12 mo. DXA (LS, proximal femur, and radius) and HR-pQCT (distal radius and tibia) were pe

Biomechanical evaluation of cerclage wiring in plated tibia fractures using human and synthetic specimens.

PURPOSE: Supplemental cerclage wiring at the distal tibia has shown to improve the mechanical stabilization of the fracture. However, its clinical use remains controversial partly because of concerns about implant loosening as well as frictional or constrictive interference between the cerclage and cortical surface. The aim of this study was to investigate in a distal tibia fracture model possible loosening of the cerclage and interference with the bone surface. METHODS: Fourteen distal tibia oblique fractures (AO/OTA 42-A2) in seven human and synthetic bone samples, respectively were treated

The mandibular bone structure in children by fractal dimension and its correlation with pixel intensity values: a pilot study.

OBJECTIVES: To identify a normal pattern of mandibular trabecular bone in children based on the fractal dimension (FD), and its possible correlation with pixel intensity (PI) values, to facilitate the early diagnosis of possible diseases and/or future bone alterations. MATERIALS AND METHODS: The 50 panoramic images were selected and divided into two groups, according to the children's age: 8-9 (Group 1; n = 25) and 6-7 (Group 2; n = 25). For FD and PI analyses, three regions of interest (ROIs) were selected, and their mean values were evaluated for each ROI, according to each group, using the

Scapular Angiomatoid Fibrous Histiocytoma with EWSR1-CREB1 Fusion in an Adult Patient.

BACKGROUND: Angiomatoid fibrous histiocytoma (AFH) is a rare intermediate malignant tumor that arises mainly in soft tissues, especially in the superficial extremities of patients younger than 30 years. There have been a few reports of AFH arising from sites other than soft tissue, including bone, and unusual site and age make it difficult to diagnose this rare tumor. Case Presentation. Here, we present a case of a 54-year-old man who was examined for chest pain, and computed tomography (CT) incidentally detected a bone tumor at the scapula with destruction of cortical bone and invasion into s…

Surgical Orthodontic Treatment of Skeletal Mandibular Protrusion with Multiple Impacted Ankylosed Teeth Treated with Alveolar Corticotomy.

AIM: To present a case of skeletal mandibular protrusion with multiple impacted ankylosed teeth, which were treated with conventional orthognathic surgery and alveolar corticotomy, respectively. BACKGROUND: Tooth ankylosis is characterized by the fusion of a root surface with the surrounding alveolar bone. Various treatment modalities for tooth ankylosis have been developed and are used commonly in the clinic. Corticotomy is defined as the application of intentional surgical injury to the cortical bone to mobilize a tooth with the adjacent bone and soft tissues. The corticotomy technique has …

Finite element analysis of an intramedulary anatomical strut for proximal humeral fractures with disrupted medial column instability: A cohort study.

BACKGROUND: Lateral locking plate (LLP) fixation has gained popularity for the treatment of proximal humeral fractures (PHFs); however, complications can occur due to loss of the medial cortical buttress from fracture comminution. MATERIALS AND METHODS: We designed a novel intramedullary anatomical medial strut with allograft bone (IAMSAB) using MIMICS software to specifically fill the intramedullary canal of the proximal humeral bone. We used finite element analysis to evaluate the biomechanical characteristics of a LLP, LLP-intramedullary fixation system (IFS), LLP-anatomical medial locking

Correcting maxillary sinus deficiencies.

The use of hydraulic force to condense bone particulate into the maxillary sinus is a predictable and less invasive method of site preparation for many, but not all, dental implant candidates. It is our approach in the primary indication for HSC--patients with advanced periodontitis in whom insufficient cortical bone at the alveolar ridge of the posterior maxilla stands as a detriment to effective implantation. A second indication for HSC is for those patients who suffer blood disorders or sensitivity to certain medications, such as vasoconstrictors, and in whom it is crucial to manage bleedin…
