[Value of the intracranial pressue measurement in the craniosynostoses detected after the age of 1 year].

Notwithstanding the fact that there is a general agreement on the necessity of surgery in the first year of life in craniosynostosis, the problem is more difficult for older children. Deterioration of clinical status is, in general, in relation with an increase in intracranial pressure (ICP), which may happen abruptly following different causes (slight head injury, for example). The purpose of this study was to determine, in different varieties of synostosis, if a high intracranial pressure could exist without clinical signs and consequently, if a surgical opening of the sutures would be neces

Syringobulbia in pediatric patients with Chiari malformation type I.

OBJECTIVE Syringobulbia (SB) is a rare entity, with few cases associated with Chiari malformation type I (CM-I) in the pediatric population. The authors reviewed all pediatric cases of CM-I-associated SB managed at their institution in order to better understand the presentation, treatment, and surgical outcomes of this condition. METHODS A prospectively maintained institutional database of craniovertebral junction abnormalities was analyzed to identify all cases of CM-I and SB from the MRI era (i.e., after 1984). The authors recorded presenting symptoms, physical examination findings, radiolo

Emergency surgical management for complicated schistosomal massive splenomegaly with portomesenteric thrombosis and bowel ischemia: a case report.

INTRODUCTION: Schistosomal periportal fibrosis is a major cause of non-cirrhotic portal hypertension and massive splenomegaly. Rarely, superimposed portomesenteric venous thrombosis (PMVT) may precipitate bowel ischemia. Managing this combination in non-endemic settings presents significant diagnostic and therapeutic challenges. We report successful bowel salvage and definitive portal decompression during the same surgical course. CASE PRESENTATION: A 43-year-old man with schistosomal periportal fibrosis and massive splenomegaly presented with partial PMVT-associated bowel ischemia. Following

Translumbar Endovascular Coiling of an Abdominal Aortic Aneurysm Complicated by Lumbar Epidural Extravasation: A Novel Case.

Abdominal aortic aneurysms (AAAs) require complex surgical management. Currently, endovascular aneurysm repair (EVAR), including the placement of stent grafts and endovascular coils, offers early survival benefits over open surgery but carries its own distinct set of complications. We report the case of a 68-year-old man with a 35-mm saccular AAA who initially underwent stent graft placement and later required additional endovascular treatment for a type 2 endoleak. Intraoperatively, part of the coil mass appeared to deviate from the expected contour of the aneurysm. Postoperative CT imaging r

Keep your attention closer to the ureters: Ureterolysis in deep endometriosis surgery.

Endometriosis surgery involving the ureter poses significant challenges requiring meticulous surgical techniques and vigilant postoperative care. This chapter addresses key aspects of ureterolysis techniques, intraoperative management of ureteral injuries, and postoperative care in the context of endometriosis surgery. Ureterolysis methods aim to isolate and mobilize the ureter while preserving its vascularity. Cold instruments and careful dissection are recommended to prevent thermal injury during surgery. Intraoperative tools such as indocyanine green (ICG) show promise in assessing for vasc

Intraoperative image guidance for cervical spine surgery.

Intraoperative image-guidance in spinal surgery has been influenced by various technological developments in imaging science since the early 1990s. The technology has evolved from simple fluoroscopic-based guidance to state-of-art intraoperative computed tomography (iCT)-based navigation systems. Although the intraoperative navigation is more commonly used in thoracolumbar spine surgery, this newer imaging platform has rapidly gained popularity in cervical approaches. The purpose of this manuscript is to address the applications of advanced image-guidance in cervical spine surgery and to descr

Missing Disc Fragment: A Rare Surgical Experience.

About 35%-72% of lumbar disc herniations are associated with fragment migration. However, the posterior epidural migration is rare. We present a strange situation encountered during surgical decompression of the posterior migrated fragment. A 72-year-old male presented with a history of pain radiating to the left lower limb and Grade 3 power of the extensor hallucis longus. Magnetic resonance imaging revealed a prolapsed intervertebral disc and a possible posterior epidural migration of disc fragment. Routine surgical steps for microdiscectomy were followed after confirmation of level using fl

Transmaxillary approach to the cranial base: an evaluation of 11 cases.

UNLABELLED: Surgical access to the skull base is always difficult, especially because of the noble anatomic structures present there. Maxillary osteotomy provides direct view to the clivus region and the neck spine, and it also bears less morbidity when compared to the many other accesses described in the literature. AIM: to assess 11 patients submitted to transmaxillary osteotomy, describing the surgical technique and postoperative results and complications. MATERIALS AND METHODS: A retrospective study involving eleven patients submitted to transmaxillary approach to the brainstem. We studi

Intraosseous schwannoma of D12 thoracic vertebra: diagnosis and surgical management with 5-year follow-up.

Intraosseous schwannomas or neurilemomas are rare benign neoplasms that account for less than 0.2% of primary bone tumours. Very rarely they have been observed in lumbar vertebrae. We report a neurilemoma involving the lower thoracic spine and present the clinical, radiological and histological findings with surgical management and 5-year follow-up. An 18-year-old-male presented with back pain and deteriorating locomotor function. Neurological examination revealed wasting of both calves and weakness in plantar flexion and dorsiflexion bilaterally. X-rays showed a D12 vertebral body abnormality

Astragalus polysaccharide ameliorates spinal cord ischemia-reperfusion injury by inhibiting apoptosis and ferroptosis through the Nrf2/HO-1 signaling pathway.

Degenerative cervical myelopathy (DCM) is a challenging neurological disorder characterized by chronic spinal cord compression, and surgical decompression is the only established treatment to relieve this compression. However, post-decompression spinal cord ischemia-reperfusion (IR) injury paradoxically worsens neurological function, and effective therapeutic treatments remain lacking. In this study, we purified a homogeneous and structurally well-defined Astragalus polysaccharide (APS) from Astragali Radix through sequential water extraction, alcohol precipitation, ion-exchange, and gel-filtr

Inside-Out Lumbar Endoscopy Revisited: Disc-Centered Surgery in Patients Requiring Limited Discectomy to Minimize Burden From Epidural Trauma, Irrigation, Nerve Manipulation, and Scarring.

BACKGROUND: Lumbar spinal endoscopy originated as a disc-centered inside-out technique shaped by early technological constraints. As endoscopic platforms advanced, operative capability expanded toward extradiscal and epidural decompression within the spinal canal, broadening the range of pathology that can be addressed endoscopically. OBJECTIVE: To re-examine the historical rationale and contemporary relevance of inside-out lumbar endoscopy and to consider its role as a proportional, biologically contained operative corridor in selected patients with disc-dominant pathology. METHODS: Narrati

Prognostic Value of the Goutallier Scale for Paravertebral Muscle Atrophy in Predicting Disability and Pain Outcomes in Degenerative Lumbar Spinal Stenosis: A Longitudinal Cohort Study of 100 Patients.

Background/Objectives: Degenerative lumbar spinal stenosis (LSS) is a prevalent cause of disability in elderly populations, often treated with decompressive surgery. However, postoperative functional outcomes are variable and influenced by factors beyond neural compression alone. This study aimed to investigate the prognostic significance of the Goutallier Classification System (GS), a radiological index of paravertebral muscle fatty degeneration, in predicting long-term postoperative disability and pain in elderly patients undergoing decompression for LSS. Methods: A retrospective cohort stud

A Mini-Thoracotomy Retropleural Microdiscectomy for Thoracic Disc Herniation: A Case Report.

Symptomatic thoracic disc herniation (TDH) is relatively rare, most commonly occurring below the T8 level due to increased spinal mobility. A small herniation could cause significant spinal cord compression due to the narrow thoracic spinal canal. A 62-year-old Malay man presented with thoracic myelopathy and was unable to walk. Magnetic resonance imaging (MRI) demonstrated a large paracentral disc herniation at T9/T10 compressing the spinal cord. He underwent a lateral mini-thoracotomy retropleural microdiscectomy. Both lungs were ventilated throughout the procedure. A lateral mini-open incis

Efficacy of the erector spinae plane block with sedation for unilateral biportal endoscopic spine surgery and comparison with other anesthetic methods.

BACKGROUND: Erector spinae plane block (ESPB) is a new regional anesthesia. Unilateral biportal endoscopic (UBE) spine surgery, a minimal invasive technique, has been performed under not only general anesthesia (GA) but regional anesthesia including spinal anesthesia (SA). The aims of this study were to evaluate the efficacy of ESPB with sedation for UBE lumbar decompression and compare it with GA and SA. METHOD: A retrospective age matched case-control study design was performed. Three groups (20 patients in each group) of patients who underwent UBE lumbar decompressions under each anestheti

Management of tracheostomized patients after poor grade subarachnoid hemorrhage: Disease related and pulmonary risk factors for failed and delayed decannulation.

OBJECTIVE: Tracheostomy is often indicated in patients with spontaneous subarachnoid hemorrhage (sSAH). Decannulation is a major goal of neurorehabilitation, but cannot be achieved in all patients. The aim of this study was to describe the course of decannulation and to identify associated risk factors in a single-center collective. PATIENTS AND METHODS: We retrospectively reviewed 87 sSAH patients with WFNS (World Federation of Neurosurgical Societies) grade III-IV, who received tracheostomy. Decannulation events and the time from tracheostomy to decannulation were recorded in a 200-days fol

Temporary axial rotation stabilization for lumbar disc herniation surgery with the ARO® spinal system: a prospective analysis of safety and clinical efficacy.

BACKGROUND: Decompressive surgery has a failure rate of between 25% and 32% based on patient reported improvement in clinical symptoms. Significant back pain is associated with 53% of failures of decompressive surgery, while also being associated with abnormal axial rotation motion. We report on the clinical performance of subjects receiving a novel axial rotation stabilization implant (ARO Spinal System, ARO Medical), while undergoing a surgical decompression for a herniated lumbar disc, a condition associated with low back pain and abnormal movement. METHODS: This Danish Medicines Agency an

Commentary on: Laminectomy plus fusion versus laminectomy alone for lumbar spondylolisthesis by Ghogawala Z, Dziura J, Butler WE, Dai F, Terrin N, Magge SN, et al. NEJM 2016;374 (15):1424-34.

BACKGROUND: How does an article involving only 66 patients randomized into two spinal surgical groups get into the New England Journal of Medicine? Nevertheless, this one did. The article by Ghogawala et al. entitled Laminectomy plus fusion versus laminectomy alone for lumbar spondylolisthesis, compared the efficacy/outcomes of pedicle/screw/rod instrumented posterolateral lumbar fusions vs. decompressions alone for treating lumbar stenosis with grade I degenerative spondylolisthesis (DS). METHODS: They designed a randomized controlled study involving only 66 patients between the ages of 50-8
