Dermatology for the practicing allergist: Tinea pedis and its complications.

Tinea pedis is a chronic fungal infection of the feet, very often observed in patients who are immuno-suppressed or have diabetes mellitus. The practicing allergist may be called upon to treat this disease for various reasons. Sometimes tinea infection may be mistaken for atopic dermatitis or allergic eczema. In other patients, tinea pedis may complicate allergy and asthma and may contribute to refractory atopic disease. Patients with recurrent cellulitis may be referred to the allergist/immunologist for an immune evaluation and discovered to have tinea pedis as a predisposing factor. From a m

Systematic review of treatments for atopic eczema.

BACKGROUND: Atopic eczema is the commonest inflammatory skin disease of childhood, affecting 15-20% of children in the UK at any one time. Adults make up about one-third of all community cases. Moderate-to-severe atopic eczema can have a profound effect on the quality of life for both sufferers and their families. In addition to the effects of intractable itching, skin damage, soreness, sleep loss and the social stigma of a visible skin disease, other factors such as frequent visits to doctors, special clothing and¿the need to constantly apply messy topical applications all add to the burden o

Microbiota-Host Interactions: Exploring Their Dynamics and Contributions to Human Diseases.

Dysbiosis is the imbalance of bacterial composition, which would otherwise change the human host's metabolic activities and usual microbiota distribution. The outcomes would be as clear as day: losing beneficial bacteria in exchange for the overgrowth of potentially pathogenic bacteria, leading to diseases. It is crucial to unravel the dynamic roles of bacteria in maintaining human health to prevent and alleviate the said dysbiosis. To date, diet, lifestyle, age, and chemical exposures were cited as the leading cause of bacterial dysbiosis atop of genetic factors. This review aims to shed ligh

Regulation of Tissue Immune Responses by Local Glucocorticoids at Epithelial Barriers and Their Impact on Interorgan Crosstalk.

The anti-inflammatory role of extra-adrenal glucocorticoid (GC) synthesis at epithelial barriers is of increasing interest with regard to the search for alternatives to synthetic corticosteroids in the therapy of inflammatory disorders. Despite being very effective in many situations the use of synthetic corticosteroids is often controversial, as exemplified in the treatment of influenza patients and only recently in the current COVID-19 pandemic. Exploring the regulatory capacity of locally produced GCs in balancing immune responses in barrier tissues and in pathogenic disorders that lead to

Proceedings of a symposium on pediatric food allergy. April 20, 2002.

Food allergy seems to be increasing in prevalence, significantly decreases the quality of life for patients and their families, and has become a common diagnostic and management issue for the pediatrician. Studies now a decade old showed that 6% to 8% of children younger than 3 years experience documented adverse reactions to foods. Several studies have defined the prevalence of allergy to specific foods in childhood. Population-based studies document a prevalence of cow milk allergy in 1.9% to 3.2% of infants and young children, egg allergy in 2.6% of children by age 2.5 years, and peanut all

[Clinical evaluation of urinary leukotriene e4 levels in children with respiratory syncytial virus infection].

The levels of leukotriene E4 (LTE4) of the urine were determined in 24 pediatric patients with infectious diseases due to respiratory syncytial virus (RSV), i.e., bronchitis, pneumonia, and bronchiolitis, and compared with those in controls without allergic disease. The level for LTE4 of the acute-phase urine was 620+/-562 pg/mg. cr in the pediatric patients infected with RSV, being significantly higher than 190+/-67 pg/mg. cr in controls (P<0.005). The levels for LTE4 of the urine in the recovery phase showed a tendency toward decrease, as compared to those in the acute phase. However, there

Janus Kinase Inhibitors' Effect on Patch Testing: A Systematic Review.

Janus Kinase (JAK) inhibitors are a growing class of medications with expanding FDA approvals for dermatologic conditions. This class of medications' immunosuppressive effects are well documented in the literature, as well as the associated side effects. Patients with atopic dermatitis (AD) and psoriasis have been shown to have increased rates of allergic contact dermatitis (ACD), traditionally diagnosed via cutaneous patch testing. With the increasing use of JAK inhibitors in this population, their interaction with patch testing is vital to ensuring accurate diagnosis of ACD and other allergi

Allergic Contact Dermatitis Reaction to Permanent Tattoo Containing Paraphenylenediamine: A Case Report.

Paraphenylenediamine (PPD) is a well-known culprit allergen in the literature and clinical practice. Although this has been described in temporary tattoos, the definite implication of PPD in permanent tattoos has not been described. We report a patient who developed severe allergic contact dermatitis (ACD) requiring skin grafting after receiving a permanent tattoo with ink containing PPD. A 30-year-old female with a past history of atopic dermatitis and psoriasis presented with a 2-week history of cutaneous reaction to a recent tattoo. The patient noticed inflammation and irritation of the tat

COVID-19 reopening causes high risk of irritant contact dermatitis in children.

INTRODUCTION: Childhood irritant contact dermatitis (ICD) is the most common cause for developing chronic hand eczema as an adult. The COVID-19 reopening in Denmark included regulations introducing frequent hand washing. The aim of the present study was to evaluate if frequent hand washing increases the incidence of ICD in children. METHODS: We conducted an observational study in Denmark during the reopening of schools and daycare facilities for children aged 0-12 years (April 22nd to May 1st 2020). A questionnaire was sent out to parents in four municipalities consisting of 20 questions abou

mSphere of Influence: Understanding atopic disease through the dynamics of skin-microbe interactions.

In this mSphere of Influence article, Flavia G. Costa explores dynamics of host-microbe interactions on the skin through three papers that have shaped her perspectives in this field. She first discusses a publication by Radhouani et al., which showed how S. aureus skin colonization promotes lung inflammation through eosinophil reprogramming. She then highlights the potential protective benefits of understudied skin commensals as described by Myles et al., finally concluding with a recent study demonstrating how byproducts from industrialization can disrupt these beneficial interactions as desc
