SPECT/CT unequivocally depicts dilated cystic duct sign on hepatobiliary scintigraphy in acute cholecystitis.

The dilated cystic duct sign (DCDS) on hepatobiliary scintigraphy can lead to a false-negative test in patients with acute cholecystitis. Presented is a case of acute gangrenous cholecystitis with probable DCDS on the planar study that was unequivocally delineated on SPECT/CT imaging. SPECT/CT may be useful when planar findings suggest DCDS.

Congenital posterior pole cataract and adult onset dilating cardiomyopathy: expanding the phenotype of αB-crystallinopathies.

Mutations in the αB-crystallin gene (CRYAB) have been reported in desmin-related myopathies, with or without cardiac involvement. Mutations in this gene have also been documented in large multi-generation families with autosomal dominant congenital posterior pole cataract (CPPC). In these congenital cataract families no cardiac or muscular phenotype was reported. This report describes a family with an unusual read-through mutation in CRYAB, leading to the elongation of the normal αB-crystallin protein with 19 amino acid residues. Affected family members combine a CPPC with an adult onset dilat

Acute disseminated intravascular coagulation developed after dilation and curettage in an adenomyosis patient: a case report.

We reported a rare case of acute disseminated intravascular coagulation (DIC) after dilation and curettage in an adenomyosis and missed abortion patient. The clinical performance improved after treatment with tranexamic acid, blood transfusions and subtotal hysterectomy. Hemorrhage, degeneration and necrosis were found in the myometrium. Pregnancy-related thrombotic tendency, accelerated uterus tissue injury after curettage, activation of coagulation system, microthrombosis formation, myometrium necrosis, exhaustion of coagulation factors and hyperfibrinolysis might play the crucial roles in t

Nav 1.5 mutations linked to dilated cardiomyopathy phenotypes: Is the gating pore current the missing link?

BACKGROUND: Periodontitis triggers systemic inflammatory reactions that may be associated with different oncological diseases, such as prostate cancer (PC), although little information exists on the possibility of this association. To determine whether periodontitis is associated with PC and other non-metastatic urological cancers in patients over 50. MATERIAL AND METHODS: This cross-sectional study was conducted between August and September 2024 at the outpatient clinic of the Belén de Trujillo Hospital (HBT) and the Regional Institute of Neoplastic Diseases "Dr. Luis Pinillos Ganoza" - IREN

Gastric pouch resizing for Roux-en-Y gastric bypass failure in patients with a dilated pouch.

BACKGROUND: Insufficient weight loss or weight regain a few years after laparoscopic Roux-en-Y gastric bypass is becoming a serious problem given the large diffusion of this procedure. In the present study, we analyzed the feasibility and safety of pouch resizing for Roux-en-Y gastric bypass failure in a consecutive series of 20 patients at a university hospital. METHODS: A prospectively maintained database was queried regarding patient demographics, the indication for revision morbidity, the percentage of excess weight loss, and the evolution of co-morbidities. RESULTS: A total of 20 patien

Percutaneous balloon dilation of Carpentier-Edwards porcine-valved right ventricle-to-pulmonary artery conduits.

Right ventricular outflow tract (RVOT) conduit stenosis remains a significant problem for patients with right ventricle-to-pulmonary artery (RV-to-PA) conduits placed as palliation for congenital heart disease. Previous reports on balloon dilation of RVOT conduits all describe small series with varying levels of success during limited follow-up evaluation. This study reviewed all patients with RV-to-PA conduits who underwent percutaneous balloon dilation for conduit stenosis at the authors' institution from 2000 to 2011. Patients with Carpentier-Edwards (CE) model 4300 porcine-valved conduits

Benign post-radiation rectal stricture treated with endoscopic balloon dilation and intralesional triamcinolone injection.

Post-radiation stricture is a rare complication after pelvis irradiation, but must be in the mind of the clinician evaluating a lower gastrointestinal obstruction. Endoscopy has gained an important role in chronic radiation proctitis with several therapeutic options for management of intestinal strictures. The treatment of rectal strictures has been limited to surgery with high morbidity and mortality. Therefore, a less invasive therapeutic approach for benign rectal strictures, endoscopic balloon dilation with or without intralesional steroid injection, has become a common treatment modality.

Postpartum dilated cardiomyopathy in a patient with systemic lupus erythematosus, nephritis and lupus anticoagulant: a diagnostic dilemma.

A 32-year-old Caucasian woman presented with shortness of breath four weeks postpartum. She was known to suffer from systemic lupus erythematosus with cutaneous, joint and minor renal involvement. During pregnancy, the patient had developed nephrotic syndrome for which she was managed with prophylactic anticoagulation and corticosteroid therapy. A leg deep vein thrombosis had arisen following caesarean section following antepartum haemorrhage. Examination revealed a heart murmur, and pulmonary signs. Computed tomography pulmonary angiogram showed cardiomegaly and bilateral pleural effusions bu

Silicone catheters may be superior to latex catheters in difficult urethral catheterization after urethral dilation.

BACKGROUND AND PURPOSE: Urethral dilation in the setting of difficult urethral catheterization is sometimes necessary to avoid suprapubic catheterization. Anecdotally, we have observed that less dilation is needed when advancing a silicone catheter over a Glidewire compared with a latex catheter of the same size. Our aim was to quantify the difference in the resistance to buckling between silicone and latex catheters. MATERIALS AND METHODS: A BOSE Electroforce load testing device was used to test 12F and 16F silicone and latex catheters under tensile and compressive forces. This information w

Impaired brachial flow-mediated dilation is a predictor of a new-onset vascular event after stroke.

BACKGROUND AND OBJECTIVE: Brachial arterial flow-mediated dilation (FMD) is associated with an increased risk of vascular events. Our aim was to investigate the relationship between FMD measured in the acute phase of ischemic stroke and a new major adverse vascular event in a consecutive cohort of patients followed up for 48 months after an acute first-ever stroke. METHODS: We measured FMD in 120 consecutive patients with acute ischemic stroke using high-resolution ultrasonography. FMD was calculated as the relationship between the basal diameter of the brachial artery before and after transi

[Usefulness of intracoronary therapy with progenitor cells in patients with dilated cardiomyopathy: Bridge or alternative to heart transplantation?].

INTRODUCTION AND OBJECTIVES: Some paediatric publications have recently raised the value of intracoronary therapy with autologous bone marrow-derived progenitor cells (APCs) in children with dilated cardiomyopathy (DCM) and heart failure. We describe the usefulness of this treatment in two infants with severe DCM and heart failure, who had been transferred to our hospital for cardiac transplant evaluation. PATIENTS AND METHODS: The first patient was a 3 months old male weighing 4 kg. The second was a 4 months old male weighing 5 kg. At the time of admission, both were in poor clinical conditi

Long-term survival in idiopathic pulmonary arterial hypertension associated with massive pulmonary artery dilation.

The present report describes two patients with long-term survival after being diagnosed with idiopathic pulmonary arterial hypertension more than 20 years earlier. Both patients were treated with calcium channel blockers for several years and are currently maintained on bosentan, an oral endothelin receptor antagonist. Severe dilation of the main pulmonary arteries is present in both patients and may be related to long-term survival with idiopathic pulmonary arterial hypertension.

Acidosis dilates brain parenchymal arterioles by conversion of calcium waves to sparks to activate BK channels.

RATIONALE: Acidosis is a powerful vasodilator signal in the brain circulation. However, the mechanisms by which this response occurs are not well understood, particularly in the cerebral microcirculation. One important mechanism to dilate cerebral (pial) arteries is by activation of large-conductance, calcium-sensitive potassium (BK(Ca)) channels by local Ca(2+) signals (Ca(2+) sparks) through ryanodine receptors (RyRs). However, the role of this pathway in the brain microcirculation is not known. OBJECTIVE: The objectives of this study were to determine the mechanism by which acidosis dilate

A rare calcified thrombosis of the dilated epidural venous plexus presenting with lumbar radiculopathy: a case report.

BACKGROUND CONTEXT: Lumbar radiculopathies may occur in the course of many diseases but are rarely caused by abnormalities or pathologic changes in the epidural venous plexus. We describe a patient with lumbar radiculopathy attributable to a thrombolith of the epidural venous plexus, as diagnosed by computed tomography (CT) and magnetic resonance imaging (MRI), and who was treated by hemilaminectomy and removal of the thrombolith. PURPOSE: To document the first report of thrombolith in the dilated vertebral epidural venous plexus presenting as a lumbar radiculopathy. STUDY DESIGN/SETTING: A
