Tip-Functionalized Gold Nanorod Micropillar Arrays for Millisecond-Resolved Photothermal Neuromodulation Toward Retinal Prosthetic Applications.

Photothermal stimulation using plasmonic nanomaterials presents a promising alternative to electrical stimulation in retinal prostheses, addressing various functional limitations associated with electrical methods. However, many photothermal strategies utilize dispersed nanoparticles randomly accumulated on target locations and do not demonstrate millisecond-scale thermal dynamics. Herein, a flexible polydimethylsiloxane (PDMS) micropillar array with gold nanorods (GNRs), immobilized at the pillar tips, is introduced to create a deterministic and conformal platform that spatially localizes pho

Galvanic vestibular stimulation to rehabilitate postural instability in Parkinson's disease.

BACKGROUND: Galvanic vestibular stimulation (GVS) is a non-invasive technique employed to rehabilitate balance by delivering low-intensity, short-duration electrical stimulation to the mastoid bones, effectively activating the vestibulospinal tract. OBJECTIVE: To evaluate the effects of GVS on balance in patients with Parkinson's disease (PD) and postural instability. METHODS: In this clinical study, 25 PD patients with postural instability in the ON phase (best effect of dopaminergic medication) underwent GVS. Balance was assessed using the Berg Balance Scale (BBS), the Timed Up and Go (TUG

Repetitive Transcranial Magnetic Stimulation (rTMS) in Major Depression.

Major depressive disorder (MDD) is a psychiatric disorder with several effective therapeutic approaches, being antidepressants and psychotherapies the first-line treatments. Nonetheless, due to side effects, limited efficacy, and contraindications for these treatments, alternative treatment options are required. Neurostimulation is a non-pharmacological and non-psychotherapeutic approach that has been under study for diverse neuropsychiatric conditions in the form of electrical or magnetic stimulation of the brain. Repetitive transcranial magnetic stimulation (rTMS) is a neurostimulation metho

Diabetic Peripheral Neuropathy: Prevention and Treatment.

Diabetic peripheral neuropathy occurs in up to 50% of patients with diabetes mellitus and increases the risk of diabetic foot ulcers and infections. Consistent screening and clear communication are essential to decrease disparities in assessment of neuropathic symptoms and diagnosis. Physicians should address underlying risk factors such as poor glycemic control, vitamin B12 deficiency, elevated blood pressure, and obesity to reduce the likelihood of developing neuropathy. First-line drug therapy for painful diabetic peripheral neuropathy includes duloxetine, gabapentin, amitriptyline, and pre

Evaluation of Quantitative and Selective Sensory Fiber Dysfunction in Patients with Cirrhosis.

BACKGROUND: Chronic liver disease has been reported to be associated with peripheral neuropathy. However, which sensory fibers are affected remains unknown. The objective of this study was to examine the function of sensory nerve fibers in patients with cirrhosis using the current perception threshold (CPT) test, as well as the correlation between blood biochemical indicators related to cirrhosis and CPT values. METHODS: We recruited 44 patients with liver cirrhosis and 37 healthy controls of the same age and gender. The Neurometer® system for the CPT test was used to stimulate the median ner

Treatment of Dysphagia in Parkinson's Disease: A Systematic Review.

The incidence of oropharyngeal dysphagia in Parkinson's disease (PD) is very high. It is necessary to search for effective therapies that could prevent pneumonia. Previous results should be interpreted cautiously as there is a lack of evidence to support the use of compensatory or rehabilitative approaches to dysphagia. We reviewed the scientific literature to describe the treatments of dysphagia in PD. A systematic review was performed in PubMed, Scopus, Elsevier, and Medline according to PRISMA standards in 2018. The articles that did not mention dysphagia secondary to PD or used surgical tr

tACS-mediated modulation of the auditory steady-state response as seen with MEG.

BACKGROUND: Previous studies have shown that transcranial electrical stimulation can be successfully applied during simultaneous MEG measurements. In particular, using beamforming they have established that changes of stimulus induced as well as evoked activity can be inspected during transcranial alternating current stimulation (tACS). OBJECTIVE/HYPOTHESIS: We studied tACS-mediated changes of the auditory steady-state response (ASSR), hypothesizing that-due to the putatively inhibitory role of alpha oscillations-these evoked responses would be diminished. METHODS: We compared ASSRs in condi

Vagus nerve stimulation in pregnant rats and effects on inflammatory markers in the brainstem of neonates.

BackgroundVagus nerve stimulation (VNS) is an Food and Drug Administration-approved method delivering electrical impulses for treatment of depression and epilepsy in adults. The vagus nerve innervates the majority of visceral organs and cervix, but potential impacts of VNS on the progress of pregnancy and the fetus are not well studied.MethodsWe tested the hypothesis that VNS in pregnant dams does not induce inflammatory changes in the cardio-respiratory control regions of the pups' brainstem, potentially impacting the morbidity and mortality of offspring. Pregnant dams were implanted with sti

Assessing temporal modulation sensitivity using electrically evoked auditory steady state responses.

Temporal cues are important for cochlear implant (CI) users when listening to speech. Users with greater sensitivity to temporal modulations show better speech recognition and modifications to stimulation parameters based on modulation sensitivity have resulted in improved speech understanding. Behavioural measures of temporal sensitivity require cooperative participants and a large amount of time. These limitations have motivated the desire for an objective measure with which to appraise temporal sensitivity for CI users. Electrically evoked auditory steady state responses (EASSRs) are neural

Deep brain stimulation and behavioural changes: is comedication the most important factor?

BACKGROUND: Adverse effects of dopaminergic medication (DA; levodopa and dopamine agonists) on impulsive behaviour and decision-making in patients with Parkinson's disease (PD) have been repeatedly reported. Deep brain stimulation (DBS) is increasingly used for the treatment of parkinsonian motor symptoms, but the excellent efficacy of DBS contrasts with a growing number of reports that the treatment may result in behavioural complications. AIMS: We investigated impulsive behaviour under different therapeutic treatments. METHODS: Fifteen patients with PD with DBS (PD-DBS) were assessed with

The endogenous opioid system is not involved in modulation of opioid-induced hyperalgesia.

UNLABELLED: Some recent studies suggested a role of the endogenous opioid system in modulating opioid-induced hyperalgesia (OIH). In order to test this hypothesis, we conducted a prospective randomized, placebo-controlled, 2-way crossover study in healthy human volunteers. We utilized a well-established model of inducing OIH after a brief exposure to the μ-opioid agonist remifentanil using intradermal electrical stimulation. Patients were exposed to a randomized 90-minute infusion of remifentanil or saline placebo during 2 separate occasions. Development of OIH was quantified using changes in

Stimulated smooth muscle neosphincter in male intrinsic sphincter deficiency: Proof of principle studies in a rabbit model.

AIMS: Intrinsic sphincter deficiency (ISD) causes significant disability and impairment of quality of life despite a range of treatment options. We investigated a novel method to improve sphincter function that does not appear to have been previously attempted, that is, transplantation to create a smooth muscle cuff, that subsequently becomes innervated, around the urethra. METHODS: Bladder pressure and passage of urine were measured in conscious, sedated rabbits of three groups: 6 control (unoperated) rabbits, 8 rabbits rendered incontinent by incision of their urethral wall, and 12 lesioned

Parasympathetic reflex vasodilatation in the masseter muscle compensates for carotid hypoperfusion during the vagus-mediated depressor response.

Parasympathetic vasodilatation in the orofacial area is thought to be an important factor in the regulation of blood flow in the common carotid artery (CABF), and disturbances in parasympathetic vasodilatations may be related to impairment of the CABF inducing craniofacial ischemia. We hypothesized that the parasympathetic vasodilatation in the masseter muscle evoked by a vagus-mediated reflex is involved in the maintenance of the CABF during the vagus-mediated depressor response. In the present study, we compared changes in blood flow in the masseter muscle (MBF) and CABF, and systemic arteri

Voluntary activation and central activation failure in the knee extensors in young women and men.

Quadriceps muscle weakness is common after knee injuries. This weakness is caused, in part, by reduced voluntary activation (VA) because of central activation failure (CAF). Superimposed electrical stimulation techniques are used to assess VA and to detect CAF. The aim of this study was to assess VA during knee extension in young healthy women and men, and to evaluate subjective discomfort from the electrical stimulation. The quadriceps muscle in six young healthy women (mean age 22 years) and six young healthy men (mean age 29 years) was stimulated during maximal voluntary contractions using

Electro-acupuncture stimulation effects on duodenal motility in anesthetized rats.

The effect of electro-acupuncture stimulation (EAS) on duodenal motility was examined in anesthetized, artificially ventilated rats. EAS was applied to the abdominal area or to a hindpaw for 30 s at stimulus intensities of 0.1-10.0 mA with a stimulus frequency of 20 Hz. The duodenal motility was measured using the balloon method at a position about 1.5 cm caudal from the pylorus. Duodenal motility was inhibited by EAS at intensities of more than 5.0 mA (suprathreshold of group IV afferent excitation) when applied to the abdominal area. The duodenal inhibitory response existed after bilateral v

Convergent carotid sinus nerve and superior laryngeal nerve afferent inputs to neurons in the NTS.

A population of 43 neurons in the nucleus of the solitary tract (NTS) was identified in pentobarbital sodium anesthetized, paralyzed, and artificially ventilated cats that received convergent inputs from carotid sinus nerve (CSN) and superior laryngeal nerve (SLN) afferents. In 21 neurons, electrical stimulation of the CSN and SLN each evoked an excitatory postsynaptic potential (EPSP; mean onset latency +/- SE of CSN-evoked input = 7.2 +/- 0.8 ms, range 2.1-14.1 ms; of SLN-evoked input = 10.3 +/- 2.1 ms, range 2.8-46.8 ms). In 22 neurons, electrical stimulation of either the CSN or SLN evoked

Diffuse noxious inhibitory controls in humans: a neurophysiological investigation of a patient with a form of Brown-Séquard syndrome.

In normal subjects, the application of heterotopic painful stimuli induces simultaneous and parallel decrease in the sensation of pain and of the spinal nociceptive flexion (RIII) reflex evoked by electrical stimulation of the sural nerve. This inhibition of the RIII reflex is absent in tetraplegic patient with clinically complete spinal cord transections and can be triggered only from the analgesic hand in patients with Wallenberg's syndrome. These findings suggest that the inhibitory phenomena observed in normal subjects are likely to be examples of diffuse noxious inhibitory controls (DNICs

Suppression of evoked potentials with continued ion transport during anoxia in turtle brain.

A key to turtle brain survival during anoxia is continued ion transport and avoidance of anoxic depolarization. Previous findings that ATP concentration remained constant during prolonged anoxia and calculations that ATP production decreased indicate that compensatory processes, other than consumption of energy stores or increased anaerobic glycolysis, must also contribute to ion homeostasis and brain survival. To determine whether preservation of ion transport is associated with changes in electrophysiology during loss of oxidative metabolism, the brains of pentobarbital sodium-anesthetized t
