Effect of emergency department opioid prescribing on health outcomes.

BACKGROUND: The relation between emergency department opioid prescribing and subsequent harm is complex and poorly studied. We sought to quantify adverse outcomes, incremental risk, and rates of prolonged opioid use among emergency department patients receiving an opioid prescription and propensity-matched controls. METHODS: We used administrative data to sample all Alberta emergency department visits over 10 years, excluding patients with cancer, palliative care, or concurrent opioid use. Treated patients filled an opioid prescription within 72 hours after their index visit; untreated patien

Identifying the Local Emergence of Medetomidine Through Prehospital Collaborations.

OBJECTIVES: Medetomidine, an alpha-two receptor agonist, has been recently detected as an adulterant of the illicit opioid supply in multiple states and observed to complicate clinical management of opioid overdoses through sedation, hypotension, and/or bradycardia. Our goal is to describe the process through which a novel adulterant of illicit opioids was identified in our community, and describe the process through which this information was disseminated to key stakeholders, including emergency medical services, emergency medicine and internal medicine providers, local and state departments

Endoplasmic reticulum stress: an emerging therapeutic target for preeclampsia†.

Preeclampsia (PE), a common obstetric complication during pregnancy, has a global prevalence of 5-8%, and it is one of the leading causes of adverse maternal and fetal outcomes, which has a lack of effective treatment other than termination of pregnancy. Endoplasmic reticulum stress (ERS) is a self-stress response of cells to alleviate misfolded and unfolded protein aggregation and calcium ion homeostasis disorders in the endoplasmic reticulum (ER) lumen by activating the unfolded protein response. Many studies have demonstrated a potential link between ERS and PE pathogenesis by mediating gen

Emerging Role of Leptin in Vascular and Placental Dysfunction in Preeclampsia.

Leptin is a well-known metabolic hormone that plays diverse roles in various body functions, including growth, reproduction, and blood pressure regulation. In pregnancy, leptin produced from the placenta is crucial for ensuring proper fetal development and angiogenesis; however, pathological increases in leptin in maternal circulation are strongly associated with vascular endothelial dysfunction and preeclampsia. Leptin has a strong role in fertility and healthy pregnancy; however, numerous clinical reports over the last 2 decades show that leptin levels pathologically increase in patients wit

Delirium Prevention and Management in Older Adults in the Emergency Department.

Delirium, acute brain dysfunction, is present in 10% to 35% of older adults in the emergency department (ED) but unrecognized in ∼80% of cases leading to significant adverse outcomes. Thus, routine screening for delirium is vital to improve prevention and management in the ED. The treatment of delirium focuses on addressing the underlying cause. For agitation, nonpharmacologic measures using the Tolerate, Anticipate, and Don't Agitate (TADA) approach and the Assess, Diagnose, Evaluate, Prevent, and Treat (ADEPT) tool are prioritized for management. If unsuccessful, only the lowest effective do

Emerging role of alternative splicing in oral and maxillofacial development.

The excision of introns and subsequent ligation of exons in precursor messenger RNA (pre-mRNA) is a fundamental mechanism requisite for the expression of eukaryotic genes. Alternative splicing (AS) serves as a potent amplifying factor, augmenting the spectrum of protein isoforms that can emanate from a singular genetic locus, thereby bolstering proteomic diversity. Perturbations in the regulatory framework of pre-mRNA splicing have been associated with an extensive array of pathological conditions. Oral and maxillofacial morphogenesis, a multifaceted process governed by intricate molecular int

Penetrating Orbital Injury: A Narrative Review for Emergency Clinicians.

Penetrating orbit injury is a rare but complex and life-threatening occurrence that may easily be overlooked. Management in the emergency department requires an early multidisciplinary approach but still lacks standard guidelines. This narrative review aims to provide a systematic approach to the management of penetrating orbital injuries for emergency clinicians. Mortality and morbidity are significant due to the orbit's proximity to numerous anatomical structures. Complications may be infectious, ocular, or cerebro-vascular. Their incidence depends on the mechanism of injury, entry point, an

Literacy legislation in practice: implementation, impact, and emerging lessons.

In the past decade, states across the United States have enacted an unprecedented number of literacy laws, many of which have been spurred by growing awareness of dyslexia and calls to align instruction with the science of reading. These laws have reshaped policies on screening, curriculum adoption, educator preparation, and intervention. This Special Issue of Annals of Dyslexia contains eleven articles that collectively examine how literacy laws are being implemented, the opportunities and challenges they present, and their consequences for educators and students. Contributions include high-l

Emerging Therapies in Autosomal Dominant Polycystic Kidney Disease.

Autosomal dominant polycystic kidney disease is the most common inherited kidney disorder and a leading monogenic cause of kidney failure. Reduction or loss of polycystin-1 and polycystin-2 function disrupts ciliary calcium signaling, elevates cyclic AMP (cAMP), reprograms cellular metabolism, and activates proliferative cascades that drive cyst expansion. Tolvaptan, a vasopressin V2 receptor antagonist, established cAMP modulation as a disease-modifying strategy but is limited by aquaretic effects and hepatotoxicity risk. This review highlights emerging therapeutic strategies in clinical tria

Current and emerging therapies for fungal biofilms and systemic infections.

Fungal pathogens pose a global public health risk, driven by the alarming rise of antifungal resistance. The current antifungal pipeline remains limited to three main classes (azoles, polyene, and echinocandins). Additionally, fungal biofilms, with its extracellular matrix, further complicates the antifungal therapeutics. Despite the persistent challenges posed by biofilms in clinical medicine, advancements in research have led to the development of numerous antifungal approaches aimed at inhibiting the fungal growth, disrupting biofilm integrity and overcoming resistance mechanisms. This revi

The emerging role of anoctamin-1 in cardiac and cerebrovascular diseases.

Cardiocerebral vascular disease has long been the leading cause of morbidity and mortality worldwide. Although there are many effective avenues for preventing and treating cardiocerebral vascular disease, further research is still needed to identify more novel molecular targets for therapeutic intervention. Anoctamin-1 (ANO1), also known as transmembrane protein 16A (TMEM16A), is the molecular identity of calcium-activated chloride channels (CaCCs) and is widely distributed in myocardial cells and the vasculature, including but not limited to the thoracic aorta, mesenteric artery, cerebral art

Emergency Management of Intoxication-Type Inherited Metabolic Disorders.

In many intoxication-type inherited metabolic disorders, the accumulation of the toxic chemical can cause acute life-threatening emergencies. Sometimes this is the inevitable consequence of a severe metabolic defect, but it is often triggered by catabolism. In this article, we consider the acute management when these conditions cause encephalopathy, seizures, stroke-like episodes, thromboses, liver failure, cardiac failure, arrhythmias and rhabdomyolysis. Treatment is available for most intoxication-type disorders, though it is seldom entirely satisfactory. The emergency management involves ge…
