Pelvic fracture related hematoma - emergency treatment and bleeding control.

PURPOSE: Unstable pelvic fractures present significant challenges in trauma care due to the risk of massive haemorrhage from arterial, venous, and osseous sources. Rapid identification of the bleeding origin is critical for targeted intervention. This multicentre retrospective study evaluated the use of the Pelvic Vascular Injury Score (P-VIS) and CT-based volumetric assessment (Kothari's method) for early detection and classification of pelvic vascular injury (PVI) in severely injured patients. METHODS: Data from January 2021 to December 2022 were collected from three German Level I Trauma C

Glenoid defects in revision shoulder arthroplasty may be treated with coracoid autograft with reliable short-term results.

BACKGROUND: Glenoid bone defects are common after failed arthroplasty or fracture fixation. Allograft or autograft reconstruction may be used to fill defects, however, may have increased failure rates or add to patient morbidity, respectively. We hypothesize that coracoid autograft can effectively fill contained glenoid defects and secondarily hypothesize that coracoid autograft has a low failure rate in early post-operative follow-up. METHODS: A retrospective chart review was performed of all patients receiving coracoid autograft for glenoid bone defects during revision shoulder arthroplasty

Multidisciplinary perioperative management of cloacal exstrophy bladder closure: A single institution's approach.

INTRODUCTION: Cloacal Exstrophy (CE), also known as OEIS complex (Omphalocele, Exstrophy of the cloaca, Imperforate anus, and Spinal defects), is the rarest and most severe congenital malformation of the bladder exstrophy-epispadias spectrum, characterized by an open bladder and exposed portions of the large intestine, along with bony anomalies. The objective of this study is to describe the experience of a high-volume exstrophy center's perioperative pathway, with a focus on analgesic use as well as urologic, orthopedic, and dietary interventions, for bladder repair of cloacal exstrophy. The Type of Lateral Hinge Fracture in Medial Open-Wedge High Tibial Osteotomy Determines Its Stability: A Biomechanical Study.

BACKGROUND: Lateral hinge fractures (LHFs) are considered risk factors for delayed union or nonunion after medial open-wedge high tibial osteotomies (MOWHTOs). However, there is limited evidence on the extent to which the morphology of the hinge fracture influences the stability of a MOWHTO. PURPOSE/HYPOTHESIS: The purpose of this study was to validate the Takeuchi classification under axial and torsional loading to identify the LHF types requiring surgical treatment. It was hypothesized that (1) LHFs would reduce construct stiffness and increase interfragmentary instability across the osteot

Clinical and Radiological Outcomes of Patients With Anterior Acetabulum Fractures Treated by the Modified Stoppa Approach.

Introduction Acetabular fractures are intra-articular fractures involving the lower extremity's weight-bearing dome. These fractures require an anatomical reduction of the fracture fragments. This aim can be accomplished by the selection of an appropriate surgical approach. This study aimed to analyze the clinical and radiological outcomes of patients with fractures in the anterior part of the acetabulum who were treated by the modified Stoppa approach. Methods This prospective observational study was conducted from April 2022 to September 2023. The inclusion criteria were: (i) age between 18

Wartime Soft Tissue Coverage Techniques for the Deployed Surgeon.

BACKGROUND: Since the start of the conflict in the Middle East in 2001, military orthopedic surgeons have faced complex orthopedic injuries including high-energy soft tissue wounds, traumatic amputations, and open fractures. Although orthopedic surgeons are well trained in the management of osseous injuries, the treatment of soft tissue injuries can be technically challenging and unfamiliar. Early washout, debridement of devitalized tissue, external fixation of bony injuries, and antibiotic therapy remain the foundation of early wound management. However, these unique extremity injuries have n

Acute compartment syndrome in children: a case series in 24 patients and review of the literature.

Trauma-associated acute compartment syndrome (ACS) of the extremities is a well-known complication in adults. There are only a handful of articles that describe the symptoms, the diagnostic procedure and treatment of ACS in children. The aim of this study was to analyse the diagnostic procedures in children compared to adolescents with ACS to obtain evidence for the diagnosis, treatment and outcome of children with ACS. Twenty-four children and adolescents with ACS have been treated at the Department of Trauma Surgery of the Medical University of Vienna, Austria. Two age-related groups were in

[The articular fracture of the lower limb].

AIMS: The combination of a distal tibia fracture with an additional upper ankle joint injury is a challenge. Apart from various implants the intraoperative problem is the handling of these two injuries with appropriate reduction and retention. The existing and further developing soft tissue damage has to be taken into consideration. The aim of this study is to evaluate the surgical management of this type of fracture. PATIENTS AND METHODS: Between 2000 and 2007 we treated 22 patients with tibia fractures and coexisting upper ankle fractures. All patients could be traced for follow-up examinat

Iliac bone-block autograft for posterior shoulder instability.

INTRODUCTION: Posterior shoulder instability is a rare condition, representing only 4% of all shoulder-joint instabilities. Numerous surgical techniques are used to treat it when conservative functional treatment proves to be insufficient. This retrospective study relates to 8 patients, presenting recurrent posterior shoulder instability, all treated with a posterior iliac bone-block procedure. The results were assessed both clinically and with contemporary imaging techniques. MATERIALS AND METHODS: A unique identical surgical technique was used in all these cases including a posterior deltoi

Remodeling of the aortic neck with a balloon-expandable stent graft in patients with complicated neck morphology.

Graft migration and other device-related problems are more frequent in abdominal aortic aneurysm (AAA) patients with a complicated neck. We wanted to evaluate the performance of a balloon-expandable stent graft in these cases. Complicated aortic neck morphology was defined as a combination of short (<15 mm) and angulated (>45 degrees) necks with or without circumferential thrombus. Severe aortic angulation was defined as less than 120 degrees. During a 24-month period, 18 consecutive patients with complicated neck anatomy were treated with the Vascular Innovations (VI)-Datascope balloon-expand
