Meningococcal septicemia presenting as bilateral endophthalmitis.

We present a patient with bilateral endophthalmitis as the presenting sign of meningococcal septicemia. Systematic examination and vitreous tap conclusively identified the microbe, and appropriate treatment was administered, with good recovery of vision.

Adverse corneal events associated with corneal reshaping: a case series.

PURPOSE: This case series presents the first documented cases of infectious ulcers associated with overnight orthokeratology in North America and other less serious complications associated with overnight corneal reshaping. CASE REPORTS: Five cases of adverse corneal events associated with corneal refractive therapy are described: two cases of microbial keratitis, one case of infiltrates, one case of toxic keratitis, and one corneal abrasion. CONCLUSIONS: Corneal compromise and poor compliance can cause adverse events with corneal reshaping. The need for ongoing patient education is importan

[Mycotic dacryocystitis related to naso-lacrimal stent. Case report].

CASE REPORT: A nasolacrimal stent was placed in a diabetic woman with a ten-year history of epiphora. Three months later the non-functional stent was removed and taken for histological study. A presumed diagnosis of fungical dacryocystitis was then established. An external dacryocystorhinostomy was performed and the obtained cultures confirmed Candida albicans as the infective agent. DISCUSSION: Only 1.2% of all dacryocystitis are due to fungal agents. This possibility must be considered when evaluating a routine chronic dacryocystitis. Cultures should be obtained when removing a nasolacrimal

Medical management of Beauveria bassiana keratitis.

PURPOSE: To describe a case of Beauveria bassiana keratitis and to discuss the management of this rare condition. METHODS: An 82-year-old woman underwent surgical repair of a graft wound dehiscence. Seven months later, shortly after the removal of sutures, the patient developed a fungal keratitis. B. bassiana was identified as the infecting organism. The patient was treated with topical natamycin and oral fluconazole. RESULTS: Following antifungal therapy, the corneal ulcer was eradicated, but the patient underwent repeat penetrating keratoplasty for decreased vision due to corneal edema. Th

The antimicrobial susceptibility of Mycobacterium chelonae isolated from corneal ulcer.

PURPOSE: To determine the in vitro susceptibility of Mycobacterium chelonae isolates from corneal ulcers to various traditional and newly-developed antimicrobial agents, alone or in combination. METHODS: Fifteen strains of M. chelonae isolated from corneal ulcers were collected at the National Taiwan University Hospital from 1989 to 1993. Susceptibility to antimicrobial agents was tested by the broth microdilution method to determine the minimum inhibitory concentration (MIC). The antimicrobial effects of combinations of antimicrobial agents were assessed by the checkerboard titration method

Optic nerve sheath decompression for visual loss in patients with acquired immunodeficiency syndrome and cryptococcal meningitis with papilledema.

Visual dysfunction developing in association with acquired immunodeficiency syndrome (AIDS) can be multifactorial. Two patients with this syndrome and cryptococcal meningitis had papilledema and visual loss. Both were treated by optic nerve sheath fenestration. One patient had bilateral nonsimultaneous optic nerve sheath fenestrations; visual function improved in one eye. The other patient had bilateral visual improvement after a unilateral optic nerve sheath fenestration. Cryptococcal organisms were present in the dural sheath specimens of both patients despite ongoing therapy with antifungal

Postoperative endophthalmitis in children following cataract surgery.

Three children developed endophthalmitis after cataract surgery. Each had signs and symptoms of either nasolacrimal duct obstruction or upper respiratory infection at the time of surgery. The causative organisms were Staphylococcus aureus, Staphylococcus epidermidis, and Streptococcus pneumoniae. Final visual acuities were 6/24, LP, and NLP, respectively. Endophthalmitis after cataract surgery in infants has never been reported. These three cases drawn from two pediatric ophthalmology practices represent an incidence of postoperative endophthalmitis of 0.45%. Although this incidence report is

A Highly Promiscuous Integron, Plasmids, Extended Spectrum Beta Lactamases and Efflux Pumps as Factors Governing Multidrug Resistance in a Highly Drug Resistant Vibrio fluvialis Isolate BD146 from Kolkata, India.

Patients with diabetes mellitus (DM) are prone to infection because glucose in the skin, urine, mucous membranes, and tears promotes growth of microorganisms. Conjunctival flora develops soon after birth, and some saprophytic conjunctival flora play a pathogenic role when immune function is compromised, which can lead to serious infection. DM is one condition that may compromise immune status. In lacrimal function tests of DM patients, a decrease in breakup time (BUT) of lacrimal film and a decrease in Schirmer's test results were seen. In the present study, conjunctival flora in patients with

Incidence of post-traumatic endophthalmitis following repaired open globe injury: impact of prophylactic intracameral moxifloxacin.

BACKGROUND/OBJECTIVES: To assess the effect of intracameral moxifloxacin (ICM) after repaired open globe injury (OGI) in reducing the incidence of post-traumatic endophthalmitis. SUBJECTS/METHODS: Ambispective, consecutive, comparative study of patients operated of OGI repair. Patients were divided into two groups: those without intracameral antibiotic prophylaxis (No-ICM) (operated between 2002 and 2012) and those with ICM prophylaxis at the end of the procedure (ICM) (operated between 2013 and 2023). Both received standard povidone-iodine antisepsis. Endophthalmitis was defined as severe in

A technique for capturing migratory periocular worms: a case series and review of literature.

PURPOSE: To describe the clinical presentation of migratory nematodes and to outline a simple strategy to ensure capture. METHODS: Retrospective case series. RESULTS: Two consecutive patients with suspected migratory nematodes were treated promptly by strategic placement of a pharmacological barrier in the forniceal conjunctiva using 1% lidocaine with epinephrine to block the routes of retreat and to immobilize the worms for controlled retrieval. Two live nematode worms, one subcutaneous dirofilaria and one subconjunctival Loa loa, were successfully removed. CONCLUSIONS: A sense of urgency

Chronic intraocular copper foreign body and candida: a unique combination.

PURPOSE: To describe a case of unilateral intermediate uveitis, retained copper intraocular foreign body (IOFB) and fungus. METHODS: Interventional case report. RESULTS: Brown-orange discoloration of the iris, lens and opacified vitreous was observed. Computed tomography revealed a retained IOFB, which was composed of copper. IOFB was removed during vitrectomy and cataract surgery. Copper deposition was demonstrated on the anterior capsule. Incidentally, dimorphic fungus were found in the vitreous. CONCLUSIONS: The presence of intraocular copper might have reduced the virulence of the fungu
