Surgical morbidity of the extradural anterior petrosal approach: the Lariboisière experience.

OBJECTIVE: Concerns about the approach-related morbidity of the extradural anterior petrosal approach (EAPA) have been raised, especially regarding temporal lobe and venous injuries, hearing impairment, facial nerve palsy, cerebrospinal fluid fistula, and seizures. There is lack in the literature of studies with detailed analysis of surgical complications. The authors have presented a large series of patients who were treated with EAPA, focusing on complications and their avoidance. METHODS: The authors carried out a retrospective review of patients who underwent EAPA at their institution bet

Trends in maxillofacial fractures in Otago-Southland, New Zealand: 2009 to 2020.

AIMS: Fractures to the maxillofacial region can have a serious impact on quality of life. The over-representation of males in the occurrence of almost all types and mechanisms of these injuries has tended to divert attention away from maxillofacial fractures in females. This study aimed to describe trends in maxillofacial trauma in a New Zealand tertiary trauma centre over a 12 year period, with a particular focus on gender differences. METHOD: A retrospective audit was undertaken of records for maxillofacial fracture cases referred to Dunedin Hospital and Southland Hospital Maxillofacial Uni

Mandibular Fractures Epidemiology and Treatment Plans in the Center of Italy: A Retrospective Study.

The epidemiology of maxillofacial fractures is variable in different geographic regions. Economic and social conditions, laws and types of behavior can be considered the most important factors influencing these differences.Mandibular fractures were first cited in 1650 BC in an Egyptian papyrus. Today, these fractures are one of the most prevalent facial skeletal injuries.A 4-year retrospective study was performed and the trauma - related data were collected from medical and radiological archives. The analysis comprised patients admitted for mandibular fracture at Sapienza University of Rome -

Effects of the COVID-19 Pandemic on Pediatric Nasal Fractures.

OBJECTIVES: Nasal bone fracture is a frequent entity consulted to the otolaryngologists, approximately accounting for 39% of all facial bone fractures. The most frequent mechanisms of injury consist of assault, sport-related injuries, falls, and motor vehicle accidents. In this study, we examined the effects of the COVID-19 pandemic on pediatric nasal fractures. METHODS: Children with nasal fracture who applied to Malatya Training and Research Hospital during the year before the first case and the following year were included in this study. Data of 172 patients for the pre-pandemic period and

Blunt cerebrovascular injury: The case for universal screening.

BACKGROUND: Current evidence-based screening algorithms for blunt cerebrovascular injury (BCVI) may miss more than 30% of carotid or vertebral artery injuries. We implemented universal screening for BCVI with computed tomography angiography of the neck at our level 1 trauma center, hypothesizing that only universal screening would identify all clinically relevant BCVIs. METHODS: Adult blunt trauma activations from July 2017 to August 2019 underwent full-body computed tomography scan including computed tomography angiography neck with a 128-slice computed tomography scanner. We calculated sens

Ocular Complications in Assault-Related Blowout Fracture.

BACKGROUND: Blowout fracture is one of the most common facial fractures, and patients usually present with accompanying ocular complications. Many studies have looked into the frequency of persistent ocular symptoms, but there is no study on assault patients and related ocular symptoms. We evaluated the incidence of residual ocular symptoms in blow-out fractures between assaulted and non-assaulted patients, and sought to identify any connection among the degree of enophthalmos, defect size, and assault-related injury. METHODS: A retrospective review was performed for any patient who sustained

[Role of fibula in replacement of mandible].

The mandible and the fibula are two totally different bones of the human skeleton. The fibula is a long straight bone of the lower leg playing secondary role compared with the tibia. The mandible, or jaw bone, is the only facial bone that moves and has complex spatial structure. The blood supply of the mandible is mainly endosteal, the inferior mandibular artery, which is one of the more important branch of the maxillary artery is responsible for its arterial supply. The fibula shows the uniform pattern of periosteal blood supply receiving many small branches from the peroneal artery, and havi

Knowledge, Attitude, and Skills of Clinical Decision Making on Pain in Emergency and Trauma Nurses: A Cross-Sectional Study.

INTRODUCTION: Emergency and trauma nurses have important responsibilities in pain management. Emergency units are the first point of admission to the hospital for patients owing to trauma, burns, acute abdominal pain, and various injuries, and these patient groups experience acute pain of varying severity. The pain experienced by emergency and trauma patients plays an active role in the deterioration of their clinical conditions. This study aimed to describe the knowledge, attitudes, and clinical decision-making skills of emergency and trauma nurses regarding pain. METHODS: The data for this

Associations Between G8 Geriatric Screening Score, Charlson Comorbidity Index, AI-Based Age Phenotype, and Overall Survival in Older Adults With Stage I-II Non-Small Cell Lung Cancer.

PURPOSE: Comprehensive geriatric assessment can identify older adult oncology patients at high risk for adverse outcomes, but is variably feasible. Therefore, we assessed whether an abridged geriatric vulnerability model incorporating abstracted G8 score (G8), Charlson Comorbidity Index (CCI), and FaceAge (an AI-based aging measure) was associated with all-cause mortality or falls risk in patients undergoing stereotactic body radiation therapy (SBRT) for early-stage non-small cell lung cancer (NSCLC). METHODS: We reviewed the records of 708 patients aged ≥65 years with stage I-II NSCLC treate

Application of morphological characteristics of radiofrequency lesions to individual parameters of thermal rhizolysis for trigeminal neuralgia.

BACKGROUND: Thermic rhizolysis is a reliable therapy for pharmaco-resistant trigeminal neuralgia (TN). Temperature, duration of electrocautery and needle location can influence the efficacy and complications of the therapy. METHODS: We performed experimental thermocautery on egg white with increasing parameters of time (30-120 s) and temperature (60-95°C); we analysed the shape, size and volume of the thermic lesions produced. We developed a surgical procedure to assess peroperatively the probable thermocoagulation field and its geometric relations with the trigeminal roots and other anatomic

Driving intoxicated: is hospital admission protective against legal ramifications?

BACKGROUND: According to the US National Highway Traffic Safety Administration, in 2010, 10,228 people were killed in alcohol-impaired driving crashes. Daily, intoxicated drivers are seen in trauma centers across the country. At our trauma center, we sought to determine the number of drivers who had a documented elevation in their blood alcohol content (BAC) and compare this with county police records to evaluate how many charges for driving while intoxicated (DWI) were issued. METHODS: A retrospective chart review was performed for trauma admissions during a 3-year period. Patients with a BA

Analysis of interpeak latencies of brainstem auditory evoked potential waveforms during microvascular decompression of cranial nerve VII for hemifacial spasm.

OBJECT: Microvascular decompression (MVD) of the facial nerve is an effective treatment for patients with hemifacial spasm. Intraoperative monitoring of brainstem auditory evoked potentials (BAEPs) during MVD can reduce the incidence of hearing loss. In this study the authors' goal was to evaluate changes in interpeak latencies (IPLs) of Waves I-V, Waves III-V, and Waves I-III of BAEP Waveforms I, III, and V during MVD and correlate them with postoperative hearing loss. To date, no such study has been performed. Hearing loss is defined as nonuseful hearing (Class C/D), which is a pure tone ave

A one-page orofacial myofunctional assessment form: a proposal.

The author presents her own proposal of a one-page orofacial myofunctional assessment and for each item on the list a brief rationale is provided. The protocol is an easy but comprehensive form that can be faxed or emailed to referral sources as needed. As science provides more objective assessment and evaluation tools, this one-page form can be easily modified.

Re-activation of atrophic motor Schwann cells after hypoglossal-facial nerve anastomosis.

Facial nerve lesions are common in humans and often require surgical intervention. If repair is delayed, reinnervation can be facilitated by transposing the freshly cut hypoglossal nerve end-to-end directly to the distal facial nerve, allowing for uncompromised hypoglossal axons to reinnervate the denervated facial musculature (hypoglossal-facial anastomosis, HFA). Schwann cells (SCs) in the distal nerve stump have an important function in promoting axonal regeneration by expressing multiple regeneration-associated proteins. Chronically denervated SCs cease to express those factors, but it is
