We report a case of a 58-year-old patient who sustained multiple finger injuries on the right hand. After thorough debridement, two dorsal defects of the middle and ring fingers were covered simultaneously with a single arterialized venous free flap from the right forearm. The flap was used to create a dorsally syndactylized digit which survived completely and was subsequently divided longitudinally. A Cochrane systematic review of 30 randomised clinical trials assessed the effects of current treatments for adults with mild-to-severe changes in facial and forearm skin that occurred as a result of prolonged exposure to the sun ('photodamage'). OBJECTIVE: To study the diagnosis and treatment of Colles fracture with dorsally dislocated ulna.
METHODS: From July 2004 to November 2004, 20 cases of Colles fracture with dorsally dislocated ulna treated with supination-cast were evaluated.
RESULTS: Fractures were healed in all cases, and no recurrent dislocation was found in duration of 6 - 10 months follow up. Evaluation with Gartland and Werley score system showed that 100% of patient restored excellent function of wrist and forearm.

Malignant transformation of eccrine spiradenoma is extremely rare. We describe the case of a 70-year-old man with malignant eccrine spiradenoma of the forearm and metastases to the axillary lymph nodes. Surgical excision with adequate margins and lymph node dissection was performed. Tamoxifen therapy was instituted after obtaining positive immunostaining results for estrogen receptor. Three patients who continued to have distal radioulnar joint pain following Bowers' hemiresection interposition technique were treated by converting the resection arthroplasty into a Sauvé-Kapandji procedure. The distal radio-ulnar ligaments (DRUL) are key components of the triangular fibrocartilage complex (TFCC). A patient with sickle cell anemia and recurrent ankle ulceration is presented. Multiple previous skin grafts did not resolve the painful ulcers. A radial forearm free flap was attempted. Despite preoperative transfusions to reduce her hemoglobin S levels, postoperative thrombosis occurred. The true risk of free flap failure in sickle cell anemia is unknown and there is still debate about the exact nature of the hypercoagulable state that exists in this disease. External fixation of forearm and arm fractures is an excellent treatment option, particularly in compound, comminuted fractures that result from high-energy trauma. We report a case of severe idiopathic orthostatic hypotension (IOH) documented by direct, continuous, ambulatory blood pressure monitoring. Failure of vasoconstriction and more significantly of venoconstriction in response to upright posture was demonstrated, indicating an autonomic nervous system disorder. The venoconstrictive agent dihydroergotamine (DHE) had little effect in preventing a fall in forearm venous tone with head up tilting but caused a recumbent pressor response leading to side effects. These were noted at a time of low plasma DHE levels indicating denervation hypersensitivity.

The contribution of autonomic nervous system activity to the cardiovascular effects of delta-9-tetrahydrocannabinol (THC) was evaluated in 4 normal subjects. The peak heart rate rise after THC was attenuated by atropine and by propranolol, and nearly abolished by atropine-propranolol pretreatment.
