Bone health after lung transplantation has not been comprehensively reviewed in over 2 decades. This narrative review summarizes the available literature on bone health in the context of lung transplantation, including epidemiology, presentation, and postoperative management. Osteoporosis is reported in approximately 30% to 50% of lung transplant candidates, largely due to disease-related impact on bone and lifestyle, and corticosteroid-related effects during end-stage lung disease (interstitial lung diseases, chronic obstructive pulmonary disease, and historically cystic fibrosis). After lung

INTRODUCTION: Teriparatide (TPTD), a recombinant form of the biologically active Parathyroid hormone, has been shown to increase bone mass and prevent fractures by increasing serum calcium levels in response to systemic hypocalcemia. Studies have shown improvements in callus volume and mineralization, bone mineral content, rate of successful union, and strength at fracture sites, and produce a sustained anabolic effect throughout the remodeling phase of fracture healing. The present study was done to evaluate the efficacy of TPTD in hastening fracture healing in elderly patients by clinical, r

The effects of locally applied zinc chloride (ZnCl2 ) on early and late-stage parameters of fracture healing were evaluated in a diabetic rat model. Type 1 Diabetes has been shown to negatively impact mechanical parameters of bone as well as biologic markers associated with bone healing. Zinc treatments have been shown to reverse those outcomes in tests of nondiabetic and diabetic animals. This study is the first to assess the efficacy of a noncarrier mediated ZnCl2 on bony healing in diabetic animals. This is a promising basic science approach which may lead to benefits for diabetic patients

CONTEXT: Many individuals at high risk for osteoporosis and fragility fracture are never screened by traditional methods. Opportunistic use of imaging obtained for other clinical purposes is required to foster identification of these patients.
OBJECTIVE: The aim of this pilot study was to evaluate texture features as a measure of bone fragility, by comparing clinically acquired magnetic resonance imaging (MRI) scans from individuals with and without a history of fragility fracture.
METHODS: This study retrospectively investigated 100 subjects who had lumbar spine MRI performed at our institu

Treatment for breast cancer, including endocrine therapies, can contribute to bone loss and increase the risk of osteoporosis and fractures. Management of bone health in patients with cancer is often coordinated between oncologists, endocrinologists, and primary care physicians. In this article, we discuss the approach to screening for fracture risk among patients initiating treatments for breast cancer and recommendations for lifestyle modifications to optimize bone health. We will review 3 indications for pharmacologic bone-targeted therapies: prevention of cancer treatment-induced bone loss

BACKGROUND: Osteoporosis is a systemic skeletal disease characterised by reduced bone mass and a distortion of bone microarchitecture. It is clinically problematic as it leads to fragility fractures which confers excess morbidity and mortality on patients. Up to 32% of individuals will experience recurrent fragility fractures within two years of an initial fracture, yet existing risk models focus on the risk of having a single fragility fracture at a time. We aim to identify predictors of multiple fragility fractures to help improve risk stratification.
METHODS: 43,801 patients referred for t

Background/Objectives: Most metacarpal fractures are isolated, simple, closed, and stable fractures and located distally. They are often caused by accidental falls, strikes by humans, by objects or traffic accidents. The majority can be treated conservatively. When unstable, angulated, malrotated or shortened, a surgical fixation of these frequent fractures is needed. To treat simple, spiral, distal or shaft fractures, intramedullary Kirschner wiring (KW) or intramedullary compression screws (ISs) are used. We wanted to compare the outcomes of those two treatments. Methods: In a retrospective

INTRODUCTION: Japan has many patients with osteoporosis; however, only about one-fifth of these patients receive treatment. Although some treatment guidelines exist for osteoporosis, the number of newly diagnosed patients with osteoporotic compression fractures is increasing and protocols for treatment of osteoporotic compression fractures vary from one hospital to another. This study aims to investigate the availability of early balloon kyphoplasty(BKP)in relation to our treatment strategy for osteoporotic compression fractures.
METHODS: In our hospital, patients diagnosed with osteoporotic

The present article describes a successful novel therapeutic intervention with Aredia with one child with Rett syndrome, after suffering from six pathological fractures within less than 3 years due to severe osteoporosis. Since the initiation of the treatment (3 years ago), the child has not suffered any fractures. Patients with chronic diseases and those with disabilities or on anticonvulsant medications are at risk for low bone density and possibly for the resultant pathologic fractures that define osteoporosis in children. Individuals with Rett syndrome (RS) have been shown to have low bone

To compare the effects of sodium fluoride and etidronate in severe postmenopausal osteoporosis, we conducted a 3 year, prospective, trial in 118 postmenopausal osteoporotic women with at least one vertebral fracture, who were randomly assigned to receive sodium fluoride (25 mg twice daily, as enteric-coated tablets) plus calcium (1000 mg/day) or intermittent etidronate (400 mg/day for 14 days) followed by calcium (1000 mg/day for 76 days). Lateral spine X-ray films and dual-energy X-ray absorptiometry (DXA) measurements of the lumbar spine and proximal femur were performed at enrollment and ye

Insufficiency fractures of the sacrum were diagnosed during the first year after successful transplantation in four (5.6%) of 71 lung and heart-lung transplant recipients. Each patient had development of low back pain after minor or no trauma; all had osteoporosis. In each instance, plain radiographs failed to demonstrate the fracture, and the diagnosis was established by radionuclide bone scanning that demonstrated the characteristic "butterfly" (bilateral sacral fracture) or "half-butterfly" appearance (unilateral sacral fracture). Sacral insufficiency fractures, a significant cause of low b

High-energy, open tibial shaft fractures may result in significant comminution, bone loss, and soft tissue injuries. Early, thorough debridement of all nonviable tissue is of critical importance in treating these fractures as an inadequate initial debridement increases the risk of infection and nonunion. Large iatrogenic bone and soft tissue defects can result from debridement and will require subsequent reconstruction by both orthopaedic and plastic surgeons. Although a variety of approaches exist to address these reconstructions, successful management of bone defects remains a considerable c

The facial nerve is frequently injured after head trauma with or without temporal bone fractures. Computed tomography (CT) is the best procedure for detecting the fracture line at the level of the facial nerve canal and for assessing any associated lesions within the temporal bone. Magnetic resonance (MR) is required if there is a facial nerve paralysis, unexplained by CT findings. We present five cases of delayed post-traumatic facial nerve palsy without evidence of temporal bone fractures on CT, thus studied on MR. MR was essential for diagnosing the nerve impairment. Osteoporosis, one of the most important disorders associated with aging, needs active prevention. Estrogen therapy prevents the early phase of bone loss and decreases the incidence of subsequent osteoporosis related fractures. All estrogens are probably not equally effective against post menopausal bone loss. In USA conjugated equine estrogens are the most used. In Europe estrogen therapy is usually applied with 17 beta estradiol with different ways of administration. If estrogen therapy cannot be used, there are limited datas about the use of bisphosphonates or calcitonins in the prevention o

Metatarsal fractures can present with a variety of situations. Ranging from the relatively benign, isolated central metatarsal fracture to the crush injury leading to extensive damage of the soft tissue and osseous components, these fractures can cause a significant inconvenience to the patient. With the exception of fifth metatarsal base fractures, little standardization is available for the treatment of metatarsal fractures. Controversy still exists regarding the proper treatment of various patient populations for junctional fifth metatarsal fractures. This article discusses the fractures of

Four cases of femoral neck fracture following avascular necrosis of the femoral head were studied histologically. All four patients were women who had received steroid therapy, three of them for systemic lupus erythematosus and the other for idiopathic thrombocytopenic purpura. Two types of fracture were found according to the site and the mechanism of fracture. One was at the junction between the necrotic bone and the repairing bone, and it can thus be regarded as a stress fracture. The other type of fracture commenced at the superior portion of the junction between the femoral head and neck,
