The low lumbar spine below Cotrel-Dubousset instrumentation: long-term findings.

STUDY DESIGN: Retrospective review of long instrumented fusions down to the low lumbar spine for the treatment of adolescent idiopathic scoliosis. OBJECTIVES: To evaluate whether the use of instrumentation systems that preserve the sagittal profile could reduce the incidence of early degenerative changes. SUMMARY OF BACKGROUND DATA: Long fusions and distractive Harrington instrumentation in the surgical treatment of adolescent idiopathic scoliosis (AIS) are known to produce pain and degenerative changes in the free lumbar segments. Reports on the use of Cotrel-Dubousett instrumentation (CDI)

[Ultrasonography-guided percutaneous ethanol injection in large an/or multiple liver metastasis].

INTRODUCTION: Percutaneous ethanol injection (PEI) under sonographic guidance is an effective therapy for hepatocellular carcinoma on cirrhosis, while less favorable results have been reported for liver metastases. Surgery and/or other new treatments (i.e., interstitial thermotherapy) are indicated only for small metastases (< 3 cm) and surgeons no longer perform the palliative debulking of neoplastic masses. PATIENTS AND METHODS: From March, 1994, to December, 1997, thirty-three patients with 62 large (> 3.5 cm) and/or multiple liver metastases, who were not eligible for surgery nor thermoth

Anesthetic Management for Carotid Endarterectomy Using Superficial and Intermediate Cervical Plexus Block: A Case Report.

Carotid endarterectomy (CEA) is a surgical procedure that is performed to reduce the risk of stroke by removing atherosclerotic plaque from the carotid artery. Regional anesthesia, mainly a combined superficial and intermediate cervical plexus block, is used as an alternative to general anesthesia for maintaining cerebral perfusion, hemodynamic control, cerebral monitoring, and rapid neurological assessment during surgery. Here, we present a case of a 69-year-old male with symptomatic right carotid artery stenosis who underwent a successful right CEA using a combined superficial and intermedia

Intraoperative hypercyanotic spells in a paediatric patient with uncorrected tetralogy of Fallot undergoing drainage of intracranial abscess in the prone position.

Patients with uncorrected cyanotic congenital heart disease undergoing non-cardiac surgery could be challenging to the anaesthesiologist as these patients are at a high risk for perioperative complications, including hypercyanotic spells. This case report describes the successful anaesthetic management of a paediatric patient with uncorrected tetralogy of Fallot who underwent emergency evacuation of intracranial abscess in the prone position under general anaesthesia. The intraoperative course was complicated by recurrent hypercyanotic spells. This report focuses on the unique considerations,

Infant Spinal Anesthesia for Urologic Surgery in a Patient With Epidermolysis Bullosa.

Infant spinal anesthesia presents a viable alternative to general anesthesia for short procedures below the umbilicus. This technique eliminates the need for airway instrumentation while preserving hemodynamic and respiratory parameters. Epidermolysis bullosa is a rare inherited genetic disorder marked by mucocutaneous fragility, erosions, ulcerations, and blister formation. Anesthesia management for patients with epidermolysis bullosa necessitates meticulous planning to minimize shearing stress on the skin and mucosa. This case report details the application of spinal anesthesia for an infant

Treatment of Chronic Locked Posterior Dislocation of the Shoulder: Is There a Time Limit to Try a Closed Reduction?: A Report of 3 Cases.

CASE: Three patients with chronic locked posterior shoulder dislocation diagnosed 4 to 10 weeks after injury, each with a reverse Hill-Sachs lesion involving >25% of the humeral head, underwent closed reduction under general anesthesia using the "2 thumbs" technique, followed by immobilization in external rotation. CONCLUSION: At a mean 4-year follow-up, all had excellent function without recurrent instability, avascular necrosis, or osteoarthritis. In carefully selected patients, closed reduction remains a safe and effective option beyond the 3-week time frame, when surgery is traditionally

Diagnostic Complexity of Spermatic Cord Liposarcoma: A Report of Two Cases.

Liposarcomas of the spermatic cord are a rare malignancy in the world of Urology and General Surgery - often developing in an insidious manner - and as such are often mistaken for more common differentials such as cysts or hernias. Here, we report two cases of liposarcoma of the spermatic cord in which the patients had delayed referrals to the urology service due to the unusual progression of the malignancies and inconclusive initial imaging. The details of both these cases are presented, and there is a discussion regarding the management of this malignancy.

A rare case of splenic hematoma after percutaneous drainage of splenic abscess with infective endocarditis.

Infective endocarditis with splenic abscesses remains a complex and unusual clinical scenario. We present a rare case of splenic hematoma caused by percutaneous drainage of splenic abscess with infective endocarditis. After antibiotic therapy, mitral valve replacement combined with splenectomy was performed simultaneously. When infective endocarditis is complicated with splenic abscess, splenectomy rather than percutaneous drainage of the splenic abscess is preferred to lower the risk of splenic haemorrhage and eradicate the source of infection. The order of surgery is controversial and should

Nodular Fasciitis of the Nose and External Auditory Canal: Two Rare Case Reports.

Nodular fasciitis(NF) is a proliferative disease of fibroblasts and myofibroblasts that generally affects subcutaneous tissue, muscle tissue, and fascia. NF usually occurs in young adults aged 20-40 and is more common in the upper extremities and relatively rare in the region of the head and neck. Here, we report on two patients with NF in the ear and nose. Under general anesthesia, the masses of NF were completely resected along the safety margin. The patients recovered well after surgery and there was no recurrence after more than half a year of follow-up.

Generalized Peritonitis Secondary to Perforated Uterine Pyometra.

Generalized peritonitis with sepsis is a common general surgery emergency. The most likely implicated structure for generalized peritonitis with pneumoperitoneum is the gastrointestinal tract with urgent explorative laparotomy being the most definitive treatment. In this particular case, perforated diverticulitis was suspected and upon an exploration of the abdomen, frank pus in the setting of normal colon was noted. Some common differential diagnoses for frank pus in the abdomen include viscus perforation, pancreatic necrosis, gangrenous cholecystitis, or penetrating abdominal trauma. Here, w…

Benefits of WALANT in palliative upper limb surgery after nerve injury.

Wide-Awake Local Anesthesia No Tourniquet (WALANT) may be a satisfactory anesthesia alternative for the management of upper limb peripheral nerve palsy sequelae. The main advantages are the possibility of active patient cooperation through intraoperative active mobilization, comfort and cost reduction. The legislation about WALANT in France remains unclear; the modalities of lidocaine epinephrine injection should be redefined. For palliative upper limb surgery, WALANT allows the surgeon to adjust the tension on the tendon transfer intraoperatively. Level 1 studies are needed to evaluate the ef

Ventilation Failure due to Endotracheal Tube Migration Into a Tracheal Pouch in a Toddler With a Repaired Tracheoesophageal Fistula: A Case Report.

A tracheal pouch is a rare complication of successful repair of a congenital tracheoesophageal fistula (TEF). An 18-month-old child with a repaired congenital TEF was scheduled for esophageal dilation to treat his esophageal stricture. Migration of the distal end of the endotracheal tube into a previously undetected tracheal pouch caused an abrupt failure to ventilate at the end of surgery. Given our experience, we recommend to screen the trachea of every patient with corrected TEF for a tracheal pouch when they are scheduled for another procedure requiring general anesthesia.

[Medical thoracoscopy in the elucidationof recurrent pleural effusion].

Medical thoracoscopy allows the respiratory physician access to inspection and biopsy of the pleura. The method has several names, such as pleuroscopy, non-intubated thoracoscopy and exploratory thoracoscopy, all of which cover the same procedure. The main indication for medical thoracoscopy is the diagnosis of recurrent pleural effusion, where pleural biopsy is needed. Medical thoracoscopy differs from thoracic surgical surgery as patients are sedated rather than in general anaesthesia. This is a review of the state of the art of pulmonological investigations of recurrent pleural effusion and

Neurosurgical Infections.

Patients in the neurointensive care unit often undergo life-saving neurosurgical interventions that can be associated with serious complications. Infection is a common and sometimes fatal complication of such procedures. Infection may occur not only in the setting of major cranial and spinal surgery, but also with common minor procedures that utilize neurosurgical devices, such as placement of external ventricular drains, ventriculoperitoneal shunts, and deep brain stimulators. In this article, we review the epidemiology and microbiology of these infections, and discuss their general and proce

[Concomitance of acute cholecystitis and acute appendicitis].

Acute appendicitis and acute cholecystitis are among the most common diagnoses that general surgeons operate. However, they are only rarely described simultaneously. Authors describe the course of disease, diagnostics and surgery solution of disease. They report, that there are many factors, which play important role in the process of diagnostics and treatment of patient. They emphasis individual access not only of patients view, but also individual acces and view of surgeon. Because of rare coincidence of both diseases, there are many questions, but answers miss.Key words: acute cholecystitis

Worth syndrome "mandibular osteosclerosis" as an incidental finding: a report of 2 cases.

This report presents two cases of Worth syndrome involving the mandible which were identified as an incidental finding on radiologic evaluation. Both patients were females who presented with enlarged mandibles. Radiologic evaluation revealed multiple bilateral mandibular enostoses, widened and thickened inferior cortical border of the mandible, with no other major clinical finding on examination. One of the patients received orthognathic surgery and healed uneventfully with no post-surgical complications. In this report, we reviewed possible differential diagnoses to this syndrome to facilitat
