Converging global health agendas and universal health coverage: financing whole-of-government action through UHC.

UN member states have committed to universal health coverage (UHC) to ensure all individuals and communities receive the health services they need without suffering financial hardship. Although the pursuit of UHC should unify disparate global health challenges, it is too commonly seen as another standalone initiative with a singular focus on the health sector. Despite constituting the cornerstone of the health-related Sustainable Development Goals, UHC-related commitments, actions, and metrics do not engage with the major drivers and determinants of health, such as poverty, gender inequality, …

Self-perceived healthcare needs for African American women in street-level prostitution: strategies for interventions.

Prostituted women have a unique combination of overall healthcare needs. This qualitative study explored the general health and perceived healthcare needs for eleven women. Lack of access to affordable culturally sensitive health clinics, individual mental health counseling, and healthy nutrition were unmet needs. Cultural sensitivity training and formation of partnerships among health care providers at Federally Qualified Health Centers with those in the social service and behavioral health fields could facilitate the provision of integrated healthcare services.

Supplier-induced demand: re-examining identification and misspecification in cross-sectional analysis.

This paper re-examines criticisms of cross-sectional methods used to test for supplier-induced demand (SID) and re-evaluates the empirical evidence using data from Australian medical services. Cross-sectional studies of SID have been criticised on two grounds. First, and most important, the inclusion of the doctor supply in the demand equation leads to an identification problem. This criticism is shown to be invalid, as the doctor supply variable is stochastic and depends upon a variety of other variables including the desirability of the location. Second, cross-sectional studies of SID fail d

[Emergency obstetrical care in Benin referral hospitals: 'near miss' patients' views].

OBJECTIVE: To evaluate emergency obstetric care and the perceptions and expectations of women who experienced 'near miss' events to improve maternal health in Benin. METHODS: Qualitative survey in seven hospitals at the three referral levels of the health pyramid from July to October 2003. We used two methods: 557 women with near miss events were interviewed in hospital and a standard questionnaire completed; then semi-structured individual interviews were conducted at home with 42 of these 557 women. RESULTS: Provided care, accommodation, facilities, costs and modalities of recovery, hygien

"When you first walk out the gates…where do [you] go?": Barriers and opportunities to achieving continuity of health care at the time of release from a provincial jail in Ontario.

We aimed to explore continuity of health care and health barriers, facilitators, and opportunities for people at the time of release from a provincial correctional facility in Ontario, Canada. We conducted focus groups in community-based organizations in a city in Ontario, Canada: a men's homeless shelter, a mental health service organization, and a social service agency with programs for people with substance use disorders. We included adults who spoke English well enough to participate in the discussion and who had been released from the provincial correctional facility in the previous year.

Women's health custom made: building on the 40 days postpartum for Arab women.

The 40-day postpartum period is characterised in the Middle East and elsewhere by an observance of seclusion, congratulatory visiting, the reciprocal exchange of gifts and money, and a special diet. Based on primary data from in-depth interviews among the Negev Bedouin in Israel, health enhancing practices are reviewed. The data are a subset from a larger study carried out in this setting. Often postnatal checkups, family planning counselling, and immunization services may not be routinely available or used. It is argued that these health services could be provided at the end of the 40-day per…

Last mile at the center of immunization delivery.

While advances in immunization led to major reductions in vaccine-preventable diseases in the past four decades, large gaps remain in the delivery of immunization services. In this perspective, we use illustrative examples to discuss gaps in global immunization and outline opportunities for evidence generation and intervention design to transform the last mile delivery of immunization services in global health. To address last mile gaps, technical limitations in global health surveillance infrastructure that result in imprecise estimates and overlapping obstacles that complicate immunization s

What are the "best-buys" for health promotion in pharmacy?

Chronic non-communicable diseases (NCDs) significantly contribute to global mortality and diminished quality of life, placing a growing burden on healthcare systems. The World Health Organization has developed cost-effective strategies to manage these diseases, entitled "best buys", by addressing major risk factors for NCDs, such as smoking, alcohol use, unhealthy diets, and physical inactivity. Pharmacies, as accessible healthcare facilities, are well-positioned to support preventive care and behaviour change, including tobacco and alcohol cessation, dietary modifications, and physical activi

Integration of testing for, prevention of, and access to treatment for HIV infection: state and local perspectives.

Individuals vary by socioeconomic status, race, geographic location, access to health care, risk for exposure to human immunodeficiency virus (HIV), and other co-occurring conditions, making it difficult to reach those who are at high risk for HIV infection. At the state and local levels, health departments often struggle to provide HIV prevention and care services, because of inadequate funding, fragmented systems, and a host of federal and state regulations. State and local health departments, however, have become adept at providing a patchwork of services despite these challenges. This repo

Contracting-out urban primary health care in Bangladesh: a qualitative exploration of implementation processes and experience.

BACKGROUND: Contracting-out (CO) to non-state providers is used widely to increase access to health care, but it entails many implementation challenges. Using Bangladesh's two decades of experience with contracting out Urban Primary Health Care (UPHC), this paper identifies contextual, contractual, and actor-related factors that require consideration when implementing CO in Low- and Middle- Income Countries. METHODS: This qualitative case-study is based on 42 in-depth interviews with past and present stakeholders working with the government and the UPHC project, as well as a desk review of ke

Policy, paperwork and 'postographs': Global indicators and maternity care documentation in rural Burkina Faso.

Targets and indicators set at the global level are powerful tools that govern health systems in low-income countries. Skilled birth attendance at a health facility is an important indicator for monitoring maternal mortality reduction worldwide. This paper examines how health workers negotiate policy implementation through the translation of clinical care into registries and reports. It does so by analysing the links between the global policy of institutional births and the role of documentation in the provision of birth care in primary health centres in Burkina Faso. Observations of health wor

Limited English proficient Asian Americans: Threshold language policy and access to mental health treatment.

The importance of providing timely, effective mental health services is increasingly recognized worldwide, and language barriers are a formidable obstacle to achieving this objective. Threshold language policy is one response implemented by California and other states within the U.S., in accordance with Title VI of the Civil Rights Act of 1964, which prohibits discrimination on the basis of race, color, and national origin in programs receiving federal funding. This policy mandates language assistance services for Medicaid enrollees whose primary language is other than English once their popul…
