Early hemodynamic response to angiotensin II defines a prognostic phenotype in bloodstream infection-associated septic shock.

BACKGROUND: Angiotensin II is increasingly used as an adjunctive vasopressor in septic shock, but the clinical significance and determinants of early hemodynamic response remain incompletely understood. METHODS: We performed a retrospective cohort study of adults with bloodstream infection-associated septic shock treated with angiotensin II between 2019 and 2025. Early hemodynamic response was quantified as the proportional reduction in background norepinephrine-equivalent dose (NED) 3 h after angiotensin II initiation and analyzed as a continuous variable. The primary outcome was 30-day mort

Off-Label Sotatercept Use in Pediatric Pulmonary Arterial Hypertension: A Single-Center Retrospective Hemodynamic Study.

Sotatercept, a first-in-class activin signaling inhibitor, has demonstrated disease-modifying efficacy in adults with pulmonary arterial hypertension (PAH). Pediatric data remain limited to ongoing clinical trials. We conducted an IRB-approved, single-center retrospective analysis of pediatric patients with WHO Group 1 or Group 3 PAH treated with off-label sotatercept at Rady Children's Hospital / UC San Diego. All patients had received maximal background triple therapy yet demonstrated persistent symptoms or adverse hemodynamics. Right heart catheterization was performed before sotatercept in…

PEGylation-associated hemodynamic effects in a rodent model of acute hypersensitivity reaction: Complement activation-mediated mechanisms.

Polyethylene glycol (PEG)-modified nanocomposites may induce acute hypersensitivity reactions (HSR), including complement activation and hypotension, followed by tachyphylaxis with an unknown mechanism. We tested a hypothesis that complement activation mediated hypotension induced by PEGylated magnetic nanoparticles (PEG-MNP) in a rodent model of acute HSR. Blood flow of renal/cremaster vasculature and aortic arch was measured in anesthetized rats using ultrasonic flowmetry or laser speckle contrast imaging. PEG-MNP (250 or 50 nm) induced a temporary reduction in arterial pressure by approxima

Local Hemodynamic Changes and Branch Outcomes Associated with Inner Branched versus Outer Branched Stent Grafts.

BACKGROUND: This study compared the hemodynamic performance and branch outcomes of outer branched (t-Branch) and inner branched (E-nside) st … y and reintervention, with CFD simulation analysis indicating no significant impact on local branch hemodynamics. However, the t-Branch may be associated with higher DF in the device's aortic segment compared to the E-nside.

Associations of wall shear stress and nitric oxide distribution with vertebral artery tortuosity: A morphological and hemodynamic study.

The vertebral artery (VA) exhibits substantial morphological variability and is prone to tortuosity and atherosclerosis at the V1 segment . This study aimed to systematically analyze sex-, laterality-, and age-related differences in the geometry and hemodynamics of the VA V1 segment, and to explore the potential role of hemodynamic factors in VA tortuosity and remodeling. Computed tomography angiography data from 53 subjects were used to reconstruct 3D models of the VA V1 segment. Geometric parameters of the subclavian artery and VA were quantified across sex, laterality, and age groups. Compu…

Distal vein ligation in radiocephalic arteriovenous fistula creation: A randomized trial of hemodynamic impact and early maturation.

BACKGROUND: The optimal anastomotic configuration for distal radiocephalic arteriovenous fistula (AVF) creation remains debated, particularly regarding the role of distal venous outflow preservation. This randomized controlled trial compared latero-lateral AVF without distal vein ligation (LL-AVF) versus latero-lateral AVF with distal vein ligation (LLT-AVF) to assess early maturation and hemodynamic performance. METHODS: We conducted a prospective randomized study including 60 patients with end-stage kidney disease undergoing first distal radiocephalic AVF. Patients were randomized 1:1 to LL

Clinical outcome prediction of Contour Neurovascular System treatment based on pre-treatment geometric and hemodynamic characteristics.

PURPOSE: The contour neurovascular system (CNS) is increasingly used for endovascular treatment of intracranial aneurysms, but outcome and post-treatment device behavior vary between patients. This study investigated whether pre-treatment geometric and hemodynamic characteristics from image-based blood flow simulations are associated with clinical outcome and follow-up CNS shape modifications. METHODS: Pre-treatment vascular geometries and blood flow simulations were analyzed in 39 CNS-treated cases. Geometric, hemodynamic, and ostium-flow clustering features were extracted, followed by globa

Atrial Fibrillation and Great Saphenous Vein Insufficiency: Exploration of Common Pathways in Hemodynamic Derangements and Systemic Inflammation.

Atrial fibrillation is the most prevalent type of arrhythmia globally, the incidence of atrial fibrillation continues to rise. Concurrently, as lifestyles modernize, the prevalence of saphenous varicose veins is also increasing significantly. Although both conditions have received considerable clinical attention, previous studies have primarily focused on independent investigations within their respective fields, leaving the potential association between these conditions relatively unexplored. In recent years, retrospective clinical analyses have shown that saphenous varicose veins may constit

Systolic Anterior Motion and Dynamic Mitral Regurgitation Following SAVR: A Case of Hemodynamic Optimization Over Intervention.

Mitral regurgitation (MR) frequently coexists with severe aortic stenosis and may persist or worsen after surgical aortic valve replacement (SAVR) despite afterload reduction. Following SAVR, abrupt ventricular unloading can unmask hyperdynamic ventricular function, promote systolic anterior motion and left ventricular outflow tract (LVOT) obstruction, resulting in dynamic MR and hemodynamic instability. A thorough preoperative assessment of ventricular geometry and mitral valve anatomy is crucial to identify patients at increased risk of post-SAVR LVOT obstruction. We describe a case of sever
