Results of Dome-shaped Proximal Tibial Osteotomy in Varus and Axial Malalignment of Tibia.

BACKGROUND: Anterior knee pain, with varus deformity and excessive external tibial torsion, can markedly impair function and quality of life. Although various surgical techniques address these deformities, dome-shaped osteotomy offers a promising approach for correcting malalignment while preserving joint integrity. This study evaluated clinical and radiological outcomes of dome-shaped osteotomy in patients with these combined deformities. METHODS: Twenty-eight knees from 21 patients with anterior knee pain, varus deformity (more than 5°), and excessive external tibial torsion (more than 30°)

Medial closing wedge high tibial osteotomy for lateral osteoarthritis in valgus knee offers good long-term outcomes in patients with normal body mass index.

PURPOSE: This study aimed to evaluate the long-term functional outcomes and survivorship of medial closing wedge high tibial osteotomy (MCWHTO) in treating lateral femorotibial osteoarthritis (OA) and factors associated. METHODS: Between 1997 and 2018, 65 patients (67 knees) who underwent an MCWHTO were longitudinally assessed. Inclusion criteria were symptomatic lateral femorotibial OA in valgus knees with no signs of medial compartment OA. The primary outcome was osteotomy survivorship with a failure endpoint defined by the necessity for total knee arthroplasty (TKA). Clinical and functiona

Association between history of cannabis use and outcomes after total hip or knee arthroplasty: a systematic review and meta-analysis.

BACKGROUND: Cannabis use may be increasing as countries legalize it and it becomes socially acceptable. A history of cannabis use may increase risk of complications after various kinds of surgery and compromise functional recovery. Here we systematically reviewed and meta-analyzed available evidence on how history of cannabis use affects recovery after hip or knee arthroplasty (THA/TKA). METHODS: The PubMed, EMBASE, and Web of Science databases were comprehensively searched and studies were selected and analyzed in accordance with the PRISMA guidelines. The methodological quality of included

Radiological Outcomes According to the Matta Score After the Surgical Fixation of Acetabular Fractures.

Background Acetabular fractures, a rising concern in developing countries, pose a significant challenge due to their complexity and association with post-operative complications. Often caused by high-energy mechanisms like falls and motor vehicle accidents, these fractures require accurate reduction to prevent long-term issues and the potential need for hip replacement. This study investigates the radiological outcomes of acetabular fracture surgery at six months, focusing on the effectiveness of achieving anatomical reduction using the Matta criteria in a low-and middle-income country (LMIC)

Medial Tibial Condylar Valgus Osteotomy improve clinical outcomes and effectively corrects significant Tibial varus deformities without compromising joint line obliquity or patellar heigh.

BACKGROUND: Tibial condylar valgus osteotomy (TCVO), or Chiba osteotomy, is a recognized procedure for treating advanced knee osteoarthritis in middle-aged individuals. Although its effectiveness is established, limited literature exists on its outcomes for specific conditions such as post-traumatic deformities, Blount disease (BD), and Pagoda-like proximal tibia varus deformities. HYPOTHESIS: We hypothesized that TCVO could improve both clinical and radiographic outcomes in patients with severe varus deformities, correcting lower-limb variances while preserving joint line obliquity (JLO) and

Clinical outcomes for minimally invasive sacroiliac joint fusion with allograft using a posterior approach.

BACKGROUND: Sacroiliac joint (SIJ) dysfunction can occur as a result of injury, degeneration, or inflammation. This dysfunction presents symptoms of pain at various locations, including the low back, hips, buttocks, and legs. The diagnosis of SIJ dysfunction is challenging and cannot be achieved solely with imaging studies such as X-rays, MRI, or CT. The current gold standard diagnostic modality is intra-articular SIJ blocks using two differing local anesthetics. Current treatments for SIJ dysfunction may be beneficial for short-term relief but lack long-term efficacy. The purpose of our study

Heterogeneous Spatial and Strength Adaptation of the Proximal Femur to Physical Activity: A Within-Subject Controlled Cross-Sectional Study.

Physical activity (PA) enhances proximal femur bone mass, as assessed using projectional imaging techniques. However, these techniques average data over large volumes, obscuring spatially heterogeneous adaptations. The current study used quantitative computed tomography, statistical parameter mapping, and subject-specific finite element (FE) modeling to explore spatial adaptation of the proximal femur to PA. In particular, we were interested in adaptation occurring at the superior femoral neck and improving strength under loading from a fall onto the greater trochanter. High/long jump athletes

The effects of a strength and neuromuscular exercise programme for the lower extremity on knee load, pain and function in obese children and adolescents: study protocol for a randomised controlled trial.

BACKGROUND: Childhood obesity is one of the most critical and accelerating health challenges throughout the world. It is a major risk factor for developing varus/valgus misalignments of the knee joint. The combination of misalignment at the knee and excess body mass may result in increased joint stresses and damage to articular cartilage. A training programme, which aims at developing a more neutral alignment of the trunk and lower limbs during movement tasks may be able to reduce knee loading during locomotion. Despite the large number of guidelines for muscle strength training and neuromuscu

The effects of poliomyelitis on motor unit behavior during repetitive muscle actions: a case report.

BACKGROUND: Acute paralytic poliomyelitis is caused by the poliovirus and usually results in muscle atrophy and weakness occurring in the lower limbs. Indwelling electromyography has been used frequently to investigate the denervation and innervation characteristics of the affected muscle. Recently developed technology allows the decomposition of the raw surface electromyography signals into the firing instances of single motor units. There is limited information regarding this electromyographic decomposition in clinical populations. In addition, regardless of electromyographic methods, no stu

Knee extensor strength does not protect against incident knee symptoms at 30 months in the multicenter knee osteoarthritis (MOST) cohort.

OBJECTIVE: Knee extensor weakness has not been associated consistently with the risk for incident knee pain. Additionally, the balance of hamstring-to-quadriceps strength (H:Q ratio) may affect risk and has not been studied. The authors determined whether knee extensor weakness or muscle imbalance is a risk factor for development of frequent knee pain or stiffness and whether the effect is modified by lower limb alignment. DESIGN: Observational study. SETTING: Community. PARTICIPANTS: Community-dwelling adults ages 50-79 years with or at risk of knee osteoarthritis based on obesity, knee in
