Trends in postneonatal mortality in the United States. 1962 through 1978.

Trends in postneonatal mortality (PNM) rates in the United States were analyzed for the period 1962 through 1978 using National Center for Health Statistics birth and death certificate data. The PNM rates declined from 5.5 to 3.6 per 1,000 live births for whites and from 15.6 to 7.6 per 1,000 live births for blacks. Most of the decline in PNM rates could be accounted for by a drop in mortality from infectious diseases. A dramatic increase occurred in the reported rates of unexplained sudden infant death (SID), which emerged as the leading reported cause of PNM. The second leading cause of PNM

Stem cell-based interventions for the prevention and treatment of germinal matrix-intraventricular haemorrhage in preterm infants.

BACKGROUND: Germinal matrix-intraventricular haemorrhage (GMH-IVH) remains a substantial issue in neonatal intensive care units worldwide. Current therapies to prevent or treat GMH-IVH are limited. Stem cell-based therapies offer a potential therapeutic approach to repair, restore, and/or regenerate injured brain tissue. These preclinical findings have now culminated in ongoing human neonatal studies. OBJECTIVES: To determine the benefits and harms of stem cell-based interventions for prevention or treatment of germinal matrix-intraventricular haemorrhage (GM-IVH) in preterm infants. SEARCH

Leading causes of child mortality in Brazil, in 1990 and 2015: estimates from the Global Burden of Disease study.

OBJECTIVE:: To analyze under-5 mortality rates and leading causes in Brazil and states in 1990 and 2015, using the Global Burden of Disease Study (GBD) 2015 estimates. METHODS:: The main sources of data for all-causes under-5 mortality and live births estimates were the mortality information system, surveys, and censuses. Proportions and rates per 1,000 live births (LB) were calculated for total deaths and leading causes. RESULTS:: Estimates of under-5 deaths in Brazil were 191,505 in 1990, and 51,226 in 2015, 90% of which were infant deaths. The rates per 1,000 LB showed a reduction of 67.6

The natural history of pregnancy: diseases of early and late gestation.

OBJECTIVE: To identify the disease processes underlying the increasing rate of gestational age-specific perinatal mortality observed under the fetuses-at-risk model. DESIGN: Retrospective cohort study. SETTING: USA and Nova Scotia, Canada. POPULATION: Births in the USA (1995 and 2005) and Nova Scotia, Canada (1988-2007). METHODS: Incidence rates of perinatal death and serious neonatal morbidity were calculated using the fetuses-at-risk approach (e.g. cumulative incidence of stillbirth during any gestational week per 1000 fetuses at risk of stillbirth). MAIN OUTCOME MEASURES: Perinatal mor

Health and social inequities in Ireland.

This paper attempts to discuss the shape of inequalities in health in the Republic of Ireland by focusing on social class, gender and regional inequalities in health outcomes as shown in annual publications of vital statistics and in various research studies. The Republic of Ireland has a demographic profile of rapid population increase, unique in Europe. While the birth rate is the highest in Europe, the infant mortality rate is relatively low, yet the perinatal mortality rate is relatively high. Attempts are made to analyse social class variations in mortality and morbidity rates but, except…

Persistent pulmonary hypertension of the newborn.

Persistent pulmonary hypertension of the newborn (PPHN) is a significant clinical problem characterized by refractory and severe hypoxemia secondary to elevated pulmonary vascular resistance resulting in right-to-left extrapulmonary shunting of deoxygenated blood. PPHN is associated with diverse cardiopulmonary disorders and a high early mortality rate for infants with severe PPHN. Surviving infants with PPHN have an increased risk of long-term morbidities. PPHN physiology can be categorized by (1) maladaptation: pulmonary vessels have normal structure and number but have abnormal vasoreactivi

Thoracic circumference: A new outcome measure in spinal muscular atrophy type 1?

Since respiratory insufficiency is the first cause of morbidity and mortality in spinal muscular atrophy type 1 (SMA 1), specific respiratory outcome measures are needed to evaluate changes and assess innovative therapies. In this study, thoracic circumference (TC) was used as a proxy for chest growth and an indirect measurement of respiratory function. The anthropometric parameters including TC and head-circumference (HC) were evaluated from birth to 13 months in 19 infants with SMA 1 and 124 control infants. TC was significantly decreased in the SMA 1 group from the first weeks of life. The

The role of surfactant and non-invasive mechanical ventilation in early management of respiratory distress syndrome in premature infants.

BACKGROUND: Surfactant replacement therapy has been used for few decades for the treatment of respiratory distress syndrome (RDS) and has significantly improved morbidity and mortality in premature infants. Non-invasive respiratory support has recently emerged as a strategy in the early management of RDS. In this review, we discuss the different strategies of early management of RDS. DATA SOURCES: A literature search of PubMed database was conducted to review the subject. The quality of evidence of key clinical studies was graded according to a modified grading system of the international GRA

A rare tuberculosis form: congenital tuberculosis.

Congenital tuberculosis is defined as infection developing as a result of the encounter between the infant and Mycobacterium tuberculosis bacilli during the intrauterine period or during normal birth. Although tuberculosis infection is very common all over the world congenital tuberculosis cases are rare and mortalities of 50% have been reported. Non-specific symptoms in congenital tuberculosis and difficulties encountered in the diagnosis of tuberculosis in general, make it difficult to reach a final diagnosis. Cases of congenital tuberculosis are generally known clinically during the first p

[Acute myocarditis due to Coxsackie virus B3 in two infants].

In the spring and summer of 2008 two seriously ill male infants were admitted to a paediatric intensive care unit. Initially, both had a fever, were drinking less and were pale complexioned. Physical examination revealed tachycardia, slow capillary filling and liver enlargement. Within a few hours, both infants developed circulatory and respiratory failure. A chest radiograph showed that the heart was enlarged and echocardiography revealed that the pump function of both ventricles was severely diminished. Myocarditis caused by Coxsackie virus B3 was diagnosed when the virus was demonstrated in

Antifungal therapy for neonatal candidiasis.

PURPOSE OF REVIEW: This review focuses on recent data regarding antifungal prophylaxis and antifungal treatment of neonatal candidiasis. RECENT FINDINGS: Candida species are the leading cause of invasive fungal infection in the neonatal intensive care unit, and are the third most common blood culture isolates recovered from cases of late-onset sepsis in the neonatal intensive care unit. Neonatal candidemia is associated with significant morbidity and mortality. Risk factors for neonatal candidiasis have been elucidated, and prophylaxis of high-risk infants has been found to be effective, part

Treatment of infantile hypertrophic pyloric stenosis in a district general hospital: a review of 160 cases.

BACKGROUND: Ramstedt's pyloromyotomy has long been the standard operation for the treatment of infantile hypertrophic pyloric stenosis. Controversy exists over whether this procedure can be performed safely in the district general hospital setting or whether it should be restricted to specialist pediatric units only. METHODS: Retrospective analysis was performed on the medical records of a series of 160 infants treated by Ramstedt's pyloromyotomy by 2 surgeons in a district general hospital over 16 years. RESULTS: There was no perioperative mortality. Oral feeding was achieved by 24 hours in

RSV infection in infants and young children. What's new in diagnosis, treatment, and prevention?

RSV is the most important respiratory pathogen in infants and young children. About 1% of primary RSV infections result in hospitalization. The virus is spread by large droplets of secretions or contact with contaminated secretions. Infants infected with RSV may demonstrate poor feeding, rhinorrhea, apnea, lethargy, wheezing, and respiratory distress. Diagnosis may be made by clinical signs and symptoms (especially those observed during epidemics), by chest radiographs showing hyperinflation, or by rapid antigen detection with immunofluorescence of nasopharyngeal aspirates. Risk factors for se
