[Uterus duplex with hematometrocolpos and ipsilateral agenesis of the kidney].

BACKGROUND: Malformations of the mullerian-duct-system occur with an incidence of about 0.001-0.005% of the population, and they are frequently associated with unilateral agenesis of the kidney. CASE: A 12-year-old girl presented to our emergency department with cyclic abdominal pain which increased with each of her menses. Ultrasound investigation showed a uterus duplex with obstructed hemivagina and hematometrocolpos as well as an ipsilateral absence of the kidney. Additionally, a cystic expansion in the small pelvis was detected which required further investigations (MRT and MR-angiography…

Safety of kidney biopsy in elderly: a prospective study.

Kidney biopsy plays an important role in the diagnosis and management of several renal diseases. There is a general reluctance to perform kidney biopsy in elderly due to fear of complications. There is no prospective head to head trial comparing complications of percutaneous kidney biopsy in elderly versus young. This prospective study was undertaken to know the frequency and type of biopsy related complications in elderly. Biopsy was performed using a spring loaded automatic 16 G biopsy gun. Post-biopsy, patients were confined to bed rest for 24 h. A record of intraprocedural problems and pos…

[A rare cause of anemia in a kidney-transplant recipient].

BACKGROUND: Anemia is a phenomenon frequently observed after kidney transplantation and differential diagnosis is broad. CASE REPORT: A 39-year-old woman who had been transplanted a kidney of her father 11 months ago was admitted to the hospital because of severe and worsening anemia (hematocrit [Hct] 0.24). She was under a standard posttransplant immunosuppressive protocol consisting of tacrolimus, mycophenolate mofetil (MMF) and prednisolone. Kidney function was excellent (serum creatinine 118 micromol/l), clinical symptoms of anemia included vertigo, fatigue and low blood pressure. Strikin…

Ureteroscopic management of renal calculi in anomalous kidneys.

OBJECTIVES: To review our experience with ureteroscopy in treating patients with renal calculi in anomalous kidneys and to evaluate the efficacy of this approach. METHODS: Eight patients with renal calculi in anomalous kidneys who were managed by ureteroscopic procedures were identified. Demographic information, preoperative stone burden, operative information (ureteroscope size, lithotrite used, instruments used, duration of surgery, complications, stenting), follow-up imaging, and complications were obtained from the medical record. This information was analyzed to determine the most freque…

[Surgical evaluation of candidates for donation. Selection of the kidney].

Living donation for kidney transplantation is being promoted due to the shortage of organs, the improved outcomes of living donor transplants and the evolution of immunosuppression regimens. The process of organ donation from a living donor affects not only medical-surgical features but also emotional, social and economic. Using kidneys from living donors involves a great responsibility in evaluation and selection. Candidates for donation undergo an extensive set of examinations in order to optimize selection and to plan surgery. Radiological evaluation is one of the most important features of…

Biomarkers of inflammation and progression of chronic kidney disease.

BACKGROUND: Chronic kidney disease is associated with higher levels of inflammatory biomarkers. Statins have anti-inflammatory properties and may attenuate loss of kidney function. Although inflammation may mediate progressive renal injury, the relation between statin use, markers of inflammation, and the rate of kidney function loss has not been elucidated. We examined the association between pravastatin use, levels of C-reactive protein (CRP), soluble tumor necrosis factor receptor II (sTNFrii), and the rate of kidney function loss. METHODS: We performed a post hoc analysis of data from a r…

New developments in the field of cystic kidney diseases.

For quite some time the field of polycystic kidney disease has led a life at the fringe of kidney research, but with the cloning of the PKD1 and many other genes this situation has dramatically changed. Polycystic kidney disease often is a syndromic disease affecting a variety of organs in addition to the kidney. Most of the proteins involved in polycystic kidney disease have been localized to the primary cilium, an extension at the apical membrane of renal tubular epithelial cells, which may serve chemo- and mechanosensory functions. It is speculated that primary cilia and their associated pr…

Vitamin D for health and in chronic kidney disease.

Vitamin D is taken for granted and is not appreciated for its importance in overall health and well-being. Vitamin D, known as the sunshine … ion that vitamin D is biologically inactive and requires sequential hydroxylations in the liver and kidney to form 1,25-dihydroxyvitamin D helps explain why patients with renal failure are often resistant to vitamin D and suffer from secondary hyperparathyroidism and renal osteodystrophy. In addition to its role in maintaining calcium and phosphorus home …

Management of anemia in patients with chronic kidney disease.

The prevalence of chronic kidney disease (CKD) is increasing at an alarming rate in the United States and other Western countries, due in part to an increased incidence of diabetes, which itself appears to be a direct consequence of the obesity epidemic in modern society. Hypertension, a condition that also results from or is exacerbated by excess body weight, remains an important cause of CKD as well. In patients with CKD, anemia is both a common occurrence and a significant risk factor for increased morbidity and mortality, especially from cardiac complications such as coronary heart disease…

Impact of kidney transplantation on the progression of cardiovascular disease.

Kidney transplantation, of all the treatment modalities for end-stage renal disease, affords the greatest potential for prolonged survival and improved quality of life. Great strides in immunosuppressant therapy have improved graft survival and forced clinicians to consider other health-care needs of kidney transplant recipients. Chief among these needs is the prevention and treatment of cardiovascular disease. Cardiovascular disease is the most common cause of death among patients with a working renal allograft. Because therapies for primary and secondary prevention are successful in the gene…

Death with a functioning graft in kidney transplant recipients.

The percentage of deceased donor kidney transplant recipients who died with a functioning graft (DWF) within one year after transplantation increased by 1% between 1993-1995, while DWF between 2-5 years after transplant has decreased since 1992, after a 3-year of increase (1989-1992). Deaths due to infection or hemorrhage within the first post-transplant year have increased since 1995 and deaths from cardiovascular disease between 2-5 years after transplantation have decreased. Deaths from malignancies among recipients of living donor kidneys increased significantly from 0.5% to 0.7% after 199…

Symptomatic kidney involvement in a child with tuberous sclerosis.

Tuberous sclerosis complex is an autosomal dominant disorder of cellular proliferation and differentiation with variable penetrance and a high spontaneous mutation rate that affects multiple organs, including the kidney. Kidney involvement is commonly asymptomatic and bilateral, and rare in childhood, especially under 10 years. Herein, we report a case of unilateral renal angiomyolipoma in a nine-year-old girl with tuberous sclerosis who had symptoms of pain and macroscopic hematuria.

Percutaneous management of calculi within horseshoe kidneys.

PURPOSE: Percutaneous treatment of patients with calculi in a horseshoe kidney can be challenging due to the altered anatomical relationship in the retroperitoneum. Therefore, we performed a multi-institutional review to assess the safety and efficacy of this minimally invasive technique. MATERIALS AND METHODS: Of 37 patients identified with calculi in a horseshoe kidney at 3 institutions 24 (65%) underwent percutaneous nephrolithotripsy as primary treatment. Average patient age was 48.4 years and 75% of the patients were male. In 3 patients with staghorn calculi mean stone size as measured b…

[Management of isolated fetal dilatations of the kidney pelvis].

The most common sonographic abnormality found in the fetal urinary tract is collecting system dilatation, accounting for 4.5% of examinations. Numerous renal pelvis measurements and various gestational age cut-off points have been recommended in the assessment of obstructive fetal uropathy. As a result, renal pelvic measurements are not well standardized. The first measurements of the renal pelvis diameter in the 80's suggested that a threshold value of 10 mm was significant in order to distinguish between physiologic and pathologic dilatations. The routine use of prenatal ultrasound and the i

Programming the cardiovascular system, kidney and the brain--a review.

The concept that 'life before birth' or the 'first environment' is important in determining subsequent risk for the development of cardiovascular/metabolic disease is now gaining acceptance. There are substantial data from animal experiments that complement and enhance the epidemiological data from human studies. We argue that any factor which disrupts nephrogenesis, and lowers nephron number, during the period of active nephrogenesis, will induce malapadaptive changes in the future functioning of that kidney and predispose to the onset of adult hypertension. Such factors include exposure of t…
