Sports injuries in children and adolescents treated at a sports injury clinic.

This study presents the results of a three-year (1989-1991) retrospective survey of sports injuries to children and adolescents (5-17 years of age) treated at a sports injuries clinic (Crystal Palace, London). Of the total of 394 injuries, 178 (45.2%) were to females and 216 (54.8%) were to males. In both females and males the frequency of injuries showed a unimodal relationship with increasing age; the peak frequency in females occurred at age 13-14 years and in males at age 15-16 years. Fifty per cent of the injuries (197) were acute, 49.5% (195) were chronic and 0.5% (2) were inconclusive i

CTA-detected incidence and descriptive fracture patterns of arterial injury in 539 surgically treated tibial plateau fractures.

BACKGROUND: Tibial plateau fractures (TPF) are complex injuries associated with significant soft tissue and periarticular complications. Although vascular injuries are well described in lower limb trauma, their incidence in tibial plateau fractures has not been clearly established. METHODS: Retrospective cohort study including patients with surgically treated TPF at a level I trauma center between 2016 and 2024. CTA was selectively performed based on clinical judgment. Post hoc descriptive risk scenarios were constructed to organize observed fracture patterns. Analysis was primarily descripti

Kinematics of balance controls in people with chronic ankle instability during unilateral stance on a moving platform.

Balance control deficits resulting from ankle sprains are central to chronic ankle instability (CAI) and its persistent symptoms. This study aimed to identify differences in balance control between individuals with CAI and healthy controls (HC) using challenging single-leg balance tasks. Twenty-three CAI and 23 HC participants performed balance tasks on a force plate that either remained static or moved mediolaterally. Force and kinematic data were recorded to measure balance and joint movements. The CAI group showed significantly shorter time-to-boundary during static conditions but no signif

Planned Tunnel Convergence for Concomitant Posterior Cruciate Ligament Reconstruction and Meniscal Root Repair.

Meniscal root injuries frequently occur in the setting of multiligamentous knee injuries. These present a complicating factor for planned tunnels and fixation owing to their proximity to the posterior cruciate ligament (PCL) tibial tunnel. Techniques typically aim to avoid tunnel convergence to reduce the risk of damage to grafts and/or fixation devices and decrease the risk of osteolysis or loss of fixation. We describe a method for planned tunnel convergence for PCL reconstruction and meniscal root repair. The PCL tibial tunnel is first drilled in the typical trajectory. An anterior cruciate…

Augmented stress weightbearing CT for evaluation of subtle tibiofibular syndesmotic injuries in the elite athlete.

OBJECTIVE: We aim to present a novel imaging technique utilizing weight-bearing CT with syndesmotic stress to identify subtle, unstable syndesmotic injuries. We illustrate this with a case presentation of such an injury in an elite athlete that ultimately required operative fixation. MATERIALS AND METHODS: In order to perform an augmented stress weightbearing CT, the patient is in the standing position, feet facing forward, and with weight distributed equally. The patient is then coached to internally rotate the shin and knee. This places an external rotational moment on the TFS due to the pl

The effect of trunk flexion angle on lower limb mechanics during running.

Trunk flexion is an understudied biomechanical variable that potentially influences running performance and susceptibility to injury. We present and test a theoretical model relating trunk flexion angle to stride parameters, joint moments and ground reaction forces that have been implicated in repetitive stress injuries. Twenty-three participants (12 male, 11 female) ran at preferred trunk flexion and three more flexed trunk positions (moderate, intermediate and high) on a custom built Bertec™ instrumented treadmill while kinematic and kinetic data were simultaneously captured. Markers adhered

A Rare Case of Ipsilateral "Floating Ileum" with Floating Hip and Floating Knee.

INTRODUCTION: Concomitant ipsilateral floating hip with floating knee is a very rare injury pattern. Its co-occurrence with ipsilateral complete separation of ileum from pelvic girdle, which can be termed as "floating ileum," is even rarer. These are extremely high energy injuries associated with significant systemic insult adding to the dilemma and complexity in management. There is no mention in the literature about this potentially life-threatening injury combination; hence, an attempt has been made to provide a pathway of the management of this rare but complex injury pattern. CASE REPORT

Synovial fluid lubricin increases in spontaneous canine cruciate ligament rupture.

Lubricin is an important boundary lubricant and chondroprotective glycoprotein in synovial fluid. Both increased and decreased synovial fluid lubricin concentrations have been reported in experimental post-traumatic osteoarthritis (PTOA) animal models and in naturally occurring joint injuries in humans and animals, with no consensus about how lubricin is altered in different species or injury types. Increased synovial fluid lubricin has been observed following intra-articular fracture in humans and horses and in human late-stage osteoarthritis; however, it is unknown how synovial lubricin is a

Vascularized nerve grafts for lower extremity nerve reconstruction.

Vascularized nerve grafts (VNG) were introduced in 1976 but since then, there have been no reports of their usage in lower extremity reconstruction systematically. The factors influencing outcomes as well as a comparison with conventional nerve grafts will be presented.Since 1981, 14 lower extremity nerve injuries in 12 patients have been reconstructed with VNG. Common peroneal nerve was injured in 12 and posterior tibial nerve in 5 patients. The level of the injury was at the knee or thigh. Twelve sural nerves were used as VNG with or without concomitant vascularized posterior calf fascia. All

[Heterotopic replantation after bilateral lower leg amputation].

INTRODUCTION: Replantation is an established procedure in reconstructive surgery. In a bilateral lower leg amputation attempts should be made to rescue at least one extremity, if the patients' vital conditions are stable. PATIENT AND METHOD: We report on a patient who has suffered a bilateral lower leg amputation. Due to the complex injuries the left leg was heterotopically (cross-over) replanted to the right leg. At the left leg an above knee stump was created. In a second operation a soft tissue defect at the replanted extremity was covered by a free microvascular latissimus dorsi muscle fl

Ipsilateral femoral neck and shaft fractures.

Ipsilateral femoral neck and shaft fractures occur in 2.5% to 6% of femur fractures. The injury results from high energy trauma. Victims are usually young, with multiple associated injuries. The diagnosis of the neck fracture is delayed in 19% to 31% of patients. The neck fracture line is almost vertical and nondisplaced, or minimally displaced in 26% to 59% of cases. The shaft fracture is often midshaft and open, and/or comminuted in 47% to 67% of cases. Ipsilateral knee injuries occur in 20% to 40% of patients. Two major complications, osteonecrosis of the femoral head and nonunion of the ne…

Patient-Reported Knee Function Versus Star Excursion Balance Performance as Correlates of Psychological Return-to-Sport Readiness After Anterior Cruciate Ligament Reconstruction.

Background: Psychological readiness, quantified by the Anterior Cruciate Ligament Return to Sport after Injury (ACL-RSI) scale, is one of the most consistent correlates of return-to-sport success following ACL reconstruction. Whether psychological readiness reflects an independent construct or is related to measured functional performance is not known. Purpose: To evaluate the associations between patient-perceived knee function and objective performance on a functional task, as quantified by traditional scoring, kinematics, and motion quality, and psychological readiness 9 to 12 months after

Early ACL reconstruction shows an improved recovery of isokinetic thigh muscle strength compared to delayed or chronic cases.

INTRODUCTION: The recovery of periarticular strength is a major criterion in return-to-play testing. The rationale of the study was to assess the impact of the delay of surgery (∆ between injury and surgery) on knee extensor and knee flexor strength of anterior cruciate ligament (ACL)-deficient patients six months after reconstruction. MATERIALS AND METHODS: In a retrospective cohort study, all patients with ACL ruptures between 03/2015 and 12/2019 were analyzed. Inclusion criteria were isolated ACL rupture without any associated lesions undergoing a reconstruction using ipsilateral hamstring
