There are no hard indications for splenectomy in children. The single most generally accepted indication is the more severe form of congenital spherocytosis. Idiopathic thrombocytopenia purpura is only an indication if there is a severe bleeding tendency. Other than local conditions involving the spleen, there seem to be no good indications for partial splenectomy. Ablation by means of embolisation has not gained popularity. We report on a case of a retroperitoneal accessory spleen with vascular supply directly from the aorta. A 47-year old woman presented with a six-month history of epigastric pain, intermittent nausea and vomiting. The CT scan of the abdomen revealed the presence of a retroperitoneal tumor between the spleen, left kidney and pancreas. Steroid-secreting neoplasms of the ovary and adrenal gland comprise a small group of tumors. A 76-year-old woman presented hair loss, facial hirsutism associated with increased serum total testosterone level. We report a rare case of a primary leiomyosarcoma of the adrenal gland. A 73-year-old woman presented with an inferior vena cava syndrome. MR imaging was suggestive of a large tumour originating from the right adrenal gland. Angiography revealed a tumour vascularised by the right adrenal artery. At explorative laparotomy a tumour of 27 cm in diameter was found which was completely fixed to the liver; the tumour was therefore considered unresectable. We report a case of a 56-year-old patient with an acute upper gastrointestinal bleeding following penetration of an aneurysm of the gastroduodenal artery in the duodenal bulb. The patient received an i.a. portsystem five month before the reported acute gastrointestinal bleeding. The portsystem was implanted for treatment of multiple liver metastases of an neuroendocrine tumor. This life-threatening situation could not controlled endoscopically. Authors present a case of twin intrauterine pregnancy combined with ectopic pregnancy. The simultaneous pregnancies occurred after the use of clomiphene citrate. In this case viable intrauterine twin gestation was observed by vaginal ultrasound examination, the simultaneous ectopic pregnancy was observed by laparotomy. Right salpingectomy was performed. In a 20-year period (1974-1993), 4230 patients underwent surgery of the gallbladder. Acute cholecystitis was the indication for emergency laparotomy in 869 patients (20.5%). Retrospective analysis demonstrated that after adjustment for age, sex, and mode of surgery--elective versus emergency--advanced age is not a risk factor contributing to mortality in uncomplicated cases of emergency surgery. Bladder injuries occur in approximately 1.6% of all laparoscopic procedures. Most often, these injuries are recognized intraoperatively or immediately postoperatively because of gross hematuria. We report two cases of bladder injury related to placement of suprapubic laparoscopic trocars that were not recognized at the time of surgery and in which no gross hematuria was evident. In each case, the patient had a history of abdominal surgery, and the diagnosis was difficult to make. BACKGROUND: Complete androgen insensitivity (CAI) is the most common cause of male pseudohermaphroditism. Affected individuals are advised to have bilateral gonadectomies to prevent the development of germ cell tumors.
CASE: A 19-year-old phenotypic female presented for the evaluation of primary amenorrhea and was diagnosed with CAI. Operative videolaparoscopy was performed, and the intraabdominal gonads were resected; the patient was discharged in < 24 hours.
