Up-regulated expression of Fas antigen in peripheral T cell subsets in patients with myasthenia gravis.

PURPOSE: Recent reports have linked various autoimmune diseases to defective Fas-mediated apoptosis or Fas expression. Here we aimed to determine whether Fas-mediated apoptosis is involved in the pathogenesis of myasthenia gravis (MG). METHODS: The expression of Fas antigen in peripheral T cell subsets from 17 Chinese patients with MG and 13 healthy individuals was determined by flow cytometry, and its associations with clinical classification, thymus pathology, the concomitance with hyperthyroidism (HT) and corticosteroid treatment were investigated. RESULTS: Compared with normal controls, …

Ocular myasthenia gravis.

PURPOSE OF REVIEW: To review ocular myasthenia gravis (OMG), a localized form of myasthenia gravis clinically involving only the extraocular, levator palpebrae superioris, and orbicularis oculi muscles. RECENT FINDINGS: Ocular manifestations can masquerade as a variety of ocular motility disorders, including central nervous system disorders and peripheral cranial nerve palsies. While sparing the pupils, the diagnosis and management can be challenging. SUMMARY: Because several diagnostic and treatment options are available for OMG, clinicians must decide the sequence and combination based on …

Genetics of myasthenia gravis: a case-control association study in the Hellenic population.

Myasthenia gravis (MG) is an heterogeneous autoimmune disease characterized by the production of autoantibodies against proteins of the postsynaptic membrane, in the neuromuscular junction. The contribution of genetic factors to MG susceptibility has been evaluated through family and twin studies however, the precise genetic background of the disease remains elusive. We conducted a case-control association study in 101 unrelated MG patients of Hellenic origin and 101 healthy volunteers in order to assess the involvement of common genetic variants in susceptibility to MG. We focused on three ca…

Asthma as a cause of persistent dyspnea in treated myasthenia gravis patients.

AIMS: The aim of this study was to evaluate the proportion of patients with treated myasthenia gravis (MG) who present with dyspnea not related to MG. METHODS: We analyzed the files of 63 consecutive adult patients with treated MG and persistent dyspnea who had been referred to our Pulmonary Function Test (PFT) Department between 2000 and 2010. RESULTS: We observed that asthma was the first cause of MG-unrelated dyspnea in MG patients, with 9 patients (14%) presenting with asthma-related PFT abnormalities. Six patients had asthma for several years before developing MG, and 3 patients (4%) de…

Risk for myasthenia gravis maps to a (151) Pro→Ala change in TNIP1 and to human leukocyte antigen-B*08.

OBJECTIVE: The objective of this study is to comprehensively define the genetic basis of early onset myasthenia gravis (EOMG). METHODS: We have carried out a 2-stage genome-wide association study on a total of 649 North European EOMG patients. Cases were matched 1:4 with controls of European ancestry. We performed imputation and conditional analyses across the major histocompatibility complex, as well as in the top regions of association outside the human leukocyte antigen (HLA) region. RESULTS: We observed the strongest association in the HLA class I region at rs7750641 (p = 1.2 × 10(-92) ;…

Profound olfactory dysfunction in myasthenia gravis.

In this study we demonstrate that myasthenia gravis, an autoimmune disease strongly identified with deficient acetylcholine receptor transmission at the post-synaptic neuromuscular junction, is accompanied by a profound loss of olfactory function. Twenty-seven MG patients, 27 matched healthy controls, and 11 patients with polymiositis, a disease with peripheral neuromuscular symptoms analogous to myasthenia gravis with no known central nervous system involvement, were tested. All were administered the University of Pennsylvania Smell Identification Test (UPSIT) and the Picture Identification T…

Improvement in ocular myasthenia gravis during CPAP therapy for sleep apnea.

We describe the case of a 73-year-old male with a history of ocular myasthenia gravis (OMG) who was followed through diagnosis and treatment of sleep-disordered breathing (SDB). Therapeutic use of continuous positive airway pressure (CPAP) led to improvement of not only SDB but also OMG. To our knowledge, this is the first paper describing this treatment benefit. This case report emphasizes the need to obtain a sleep history in patients with ocular myasthenia gravis in order to screen for SDB.

Impaired regulatory function in circulating CD4(+)CD25(high)CD127(low/-) T cells in patients with myasthenia gravis.

Previous studies have reported alterations in numbers or function of regulatory T (Treg) cells in myasthenia gravis (MG) patients, but published results have been inconsistent, likely due to the isolation of heterogenous "Treg" populations. In this study, we used surface CD4, CD25(high), and CD127(low/-) expression to isolate a relatively pure population of Tregs, and established that there was no alteration in the relative numbers of Tregs within the peripheral T cell pool in MG patients. In vitro proliferation assays, however, demonstrated that Treg-mediated suppression of responder T (Tresp…

An ocular form of myasthenia gravis with a high titer of anti-MuSK antibodies during a long-term follow-up.

We herein report a case of ocular myasthenia gravis (MG) that was highly positive for anti-muscle-specific tyrosine kinase (MuSK) antibodies. The examined patient exhibited bilateral ptosis and lateral gaze palsy without any generalized symptoms and was diagnosed with ocular MG with anti-MuSK antibodies. She responded to treatment with prednisolone and immunosuppressants and experienced only ocular symptoms for four years and eight months after onset. Ocular MG with anti-MuSK antibodies lasting for a long term has rarely been described. Our findings suggest that it may be reasonable to test fo…

Epidemiology of myasthenia gravis in Austria: rising prevalence in an ageing society.

OBJECTIVE: The purpose of this study was to investigate recent epidemiological trends of myasthenia gravis (MG) in Austria. METHODS: We used the national hospital discharge register, which records the discharge diagnoses of all inpatient stays in Austria to calculate the yearly inpatient prevalence of MG from 1992 to 2009 (main or secondary diagnosis of MG). The population prevalence was indirectly estimated. The temporal and geographical variability of the inpatient prevalence was correlated with the number of practicing neurologists. RESULTS: The inpatient prevalence of 2009 was calculated…

Changes in quality of life scores with intravenous immunoglobulin or plasmapheresis in patients with myasthenia gravis.

BACKGROUND: Intravenous immunoglobulin (IVIG) and plasmapheresis (plasma exchange (PLEX)) have comparable efficacy in reducing the Quantitative Myasthenia Gravis Score for disease severity (QMGS) in patients with moderate to severe myasthenia gravis (MG). OBJECTIVE: To determine if the improvement in the quality of life (QOL) after immunomodulation is comparable with either IVIG or PLEX. METHODS: 62 patients participated in the MG-QOL-60 study, completing the questionnaire at baseline and at day 14 after treatment. The MG-QOL-15 scores were computed from the MG-QOL-60 questionnaire responses…

[Investigation of preoperative respiratory evaluation in four cases of myasthenia gravis undergoing extended thymectomy in the past two years].

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disease. The MG patients undergoing extended thymectomy under general anesthesia are at risk for postoperative complications, such as respiratory insufficiency and crisis (cholinergic and myasthenic). We evaluated the preoperative predictive factors, which are important for postoperative respiratory control. METHODS: Four patients undergoing extended thymectomy under general anesthesia in our hospital within the last two years (2008-2010) were studied. All patients were graded with Ossermann Classification (I, IIa, IIb, IIb), MGFA Clinical C…
