Supracondylar fracture of the femur after total knee arthroplasty.

We reviewed the data on thirty-six supracondylar fractures of the femur (in thirty-four patients) that occurred after total knee arthroplasties that were done between April 1974 and December 1981. Patients who had osteoporosis, rheumatoid arthritis, one or more previous arthroplasties of the knee, or inadvertent breeching of the anterior aspect of the femoral cortex at operation appeared to be particularly at risk for a supracondylar femoral fracture. Malalignment of the component could not be implicated as a cause. Twenty-six fractures (in twenty-five patients) were treated by non-operative m

Bilateral capitellum humeri fracture; a case report.

This is the report of a 33-year-old female patient who was admitted to the Emergency Department with bilateral capitellum humerus fracture after a fall from a height. The patient was surgically treated with open reduction and internal fixation for both elbows, with 3 Kirschner wires on the right elbow and one mini-fragment screw on the left. Surgical intervention was carried out in emergency conditions. Following an early physical rehabilitation programme at the end of the 1st week, the roentgenograms in the 9th week showed acceptable bone union. However, in the 2nd year of follow-up the patie

Total dislocations of the navicular: are they ever isolated injuries?

Isolated dislocations of the navicular are rare injuries; we present our experience of six cases in which the navicular was dislocated without fracture. All patients had complex injuries, with considerable disruption of the midfoot. Five patients had open reduction and stabilisation with Kirschner wires. One developed subluxation and deformity of the midfoot because of inadequate stabilisation of the lateral column, and there was one patient with ischaemic necrosis. We believe that the navicular cannot dislocate in isolation because of the rigid bony supports around it; there has to be signifi

Evaluating the Safety of Limited Load Bearing for Activities of Daily Living Following Volar Locked Plating of Select Distal Radius Fractures.

PURPOSE: Distal radius fractures (DRFs) are the most common fractures worldwide. Treatment with open reduction and internal fixation (ORIF) is typically followed by a period of weight-bearing limitations. We hypothesized that (1) allowing patients to load bear for activities of daily living (ADLs) following ORIF of select fracture patterns would not lead to clinically relevant changes in radiographic alignment and (2) a low rate of complications including revision ORIF would be observed. METHODS: A retrospective review of adults (≥18 years old) with DRFs treated with ORIF was identified using

Treatment of a femoral neck fracture in a patient with dwarfism: a case report.

BACKGROUND: Femoral neck fractures in patients with pituitary dwarfism present significant surgical challenges due to anatomical variations, compromised bone quality, and limited therapeutic options. The management of such cases requires careful consideration of both the anatomical constraints of and the potential complications associated with various fixation methods. CASE PRESENTATION: This report presents the case of a 30-year-old female with congenital pituitary dwarfism who sustained a subcapital femoral neck fracture following trauma. After suffering a workplace injury due to a fall, th

Ogilvie's syndrome after orthopedic trauma surgery: A case report.

Ogilvie's syndrome, characterized by acute colonic pseudo-obstruction without mechanical blockage, typically affects the elderly or postoperative patients but is rarely reported after orthopedic trauma surgery. We described a 73-year-old man who developed persistent vomiting and abdominal distension following open reduction and internal fixation of tibial fractures and percutaneous pedicle screw fixation for thoracolumbar compression fractures. Imaging revealed diffuse intestinal dilation, predominantly involving the colon, without obstruction. Conservative management with bowel rest and suppo

Traumatic Globe Luxation and Optic Nerve Avulsion: A Case Report and Literature Review.

Traumatic globe luxation associated with optic nerve avulsion is rare. We describe a case of a 42-year-old Indian gentleman who was involved in a motor vehicle accident (MVA). He sustained a deep laceration wound from the right side of the nose extending to the left medial canthal region and left eyelid. The left globe was not visualized, and only the left optic nerve stump was seen. A computerized tomography (CT) scan showed a left globe dislocated inferotemporal with discontinuity of the left optic nerve, inferior rectus, and lateral rectus muscle. There were also comminuted fractures at the

The evolution and impact of the "damage control orthopedics" paradigm in combat surgery: a review.

The idea of damage control (DC) is grounded on a sequential therapeutic strategy that supports physiological restoration over anatomic repair in critically injured patients. This concept is firstly described as damage control surgery (DCS) for war-wounded patients with abdominal exsanguinating trauma. The goal was to avoid prolonged operative times and prevent the outset of the lethal cycle of hypothermia, acidosis and coagulopathy. Damage control orthopedics (DCO) is also based on this concept and it is applied in the treatment of some polytrauma patients with pelvic and long bones fractures

Radial Head Resection versus Arthroplasty in Unrepairable Comminuted Fractures Mason Type III and Type IV: A Systematic Review.

Unrepairable comminuted fractures of the radial head Mason type III or type IV have poor outcomes when treated by open reduction and internal fixation. Radial head resection has been proposed as good option for surgical treatment, while in the last decades, the development of technology and design in radial head prosthesis has increased efficacy in prosthetic replacement. The present review was conducted to determine the best surgical treatment for comminuted radial head when ORIF is not possible. Better outcomes are reported for radial head arthroplasty in terms of elbow stability, range of m

Intercondylar Fractures of the Distal Humerus in Patients Younger Than 20 Years: Capsulectomy Results.

Few reports in the literature have been dedicated to young patients with distal fractures of the humerus, and few have addressed subsequent indications to improve range of motion (ROM). This article is a retrospective review of the elbow flexion-extension ROM in 31 patients (age, 12-19 years) who had been treated with open reduction and internal fixation of intercondylar fractures of the distal humerus at a level I trauma center from 1991 through 2013. The ROM of patients who underwent capsulectomy was compared with that of those who did not. Main outcome measures were ROM in degrees and need

Simultaneous bilateral femoral neck fractures secondary to epileptic seizures: treatment by bilateral total hip arthroplasty.

Simultaneous bilateral femoral neck fracture following an epileptic seizure attack are rare. Open reduction and internal fixation remains the most used therapeutic option. Arthroplasty, carrying a high risk of dislocation is less often recommended. We report the favourable evolution of a 49-year-old man who benefited from a single stage bilateral total hip arthroplasty operation for his simultaneous bilateral, femoral neck fractures secondary to a generalized seizure. This nonconsensual choice, in this case, was justified on multiple grounds: surgical care delay longer than 48 hours, substanti

Sagittal wrist motion of carpal bones following intraarticular fractures of the distal radius.

Forty patients (mean age, 37 years) with intraarticular C2 and C3 Colles fractures were treated by open reduction, internal fixation and bone grafting. At a mean follow-up of 8 years radiocarpal and midcarpal motion was evaluated, the depth of the articular surface of the distal radius in the sagittal plane was measured and the presence of arthritis was noted. The fractures healed with a mean palmar tilt of 6 degrees , a mean ulnar tilt of 18 degrees and ulna variance within 1 mm of the contralateral side. The depth of the articular surface of the distal radius was 1.3 mm greater than the unin

Management of acute fractures and nonunions of the proximal pole of the scaphoid.

It is my belief that all acute proximal pole fractures should be treated by open reduction and internal fixation, via a dorsal approach. There is no longer any place for conservative treatment of these fractures, because a lengthy period of plaster immobilization is required and there is an unacceptably high risk of nonunion with conservative management. Unfortunately, nonunion of the proximal pole remains a common and disabling problem which demands careful evaluation and treatment. Internal fixation combined with limited cancellous bone grafting produces very satisfactory results in terms of

Correlation of functional and radiological outcome after surgical treatment of acetabular fractures with quadrilateral plate involvement. A meta-analysis.

PURPOSE: Quadrilateral plate fractures are complex injuries often requiring surgical management. Nevertheless, postoperative reduction quality has not been consistently correlated with functional outcome. We conducted a meta-analysis evaluating the association between functional and radiological outcomes of patients with acetabular fracture involving the quadrilateral plate who were treated surgically with open reduction and internal fixation. METHODS: A comprehensive search of PubMed, Cochrane Library and Web of Science databases was performed to identify prospective and retrospective studie

Risk Factors for Aseptic Revision of Operatively Treated Ankle Fractures.

BACKGROUND: Early revision rates within 12 months after ankle fracture open reduction internal fixation (AF-ORIF) are fairly low; however, they remain relevant given the volume of ankle fractures occurring annually. Understanding these rates is complex because reoperation due to technical or mechanical complications is typically reported alongside soft tissue-related problems such as symptomatic hardware, wound dehiscence, or infection. There are limited data identifying risk factors specifically for revision of ankle fracture fixation in the absence of soft tissue complications. Understanding
