Ten years of transitional pain service research and practice: where are we and where do we go from here?

Chronic postsurgical pain (CPSP) is a prevalent yet unintended consequence of surgery with substantial burdens to the individual and their family, the healthcare system, and society at large. The present article briefly reviews the evidence for transitional pain services (TPSs) that have arisen in an effort to prevent and mange CPSP and persistent opioid use, and provides an update on recent novel risk factors for CPSP. Available evidence from one randomized controlled trial (RCT) and three non-randomized cohort studies suggests that TPS treatment is associated with better opioid use outcomes,

Group-Based Psychological Interventions for Autistic Youth with Chronic Pain: Narrative Review and Practice Considerations for Intensive Interdisciplinary Pediatric Pain Treatment.

Emerging work characterizing youth with chronic pain increasingly recognizes a large cohort of youth with co-occurring chronic pain and autism. This development has prompted questions about how to adapt Intensive Interdisciplinary Pediatric Pain Treatment (IIPTs) and the group-based treatments commonly used in these settings to improve accessibility, acceptability, and utility for autistic participants. There is a need for clinically oriented literature that IIPT programs and clinical trialists can use to guide adaptation efforts. Given long-term risks of inadequately treated pediatric pain, w

The Evolving Role of Peripheral Nerve Stimulation in Postoperative Analgesia and Recovery.

PURPOSE OF REVIEW: Acute postoperative pain has traditionally been managed with opioids and regional nerve blocks, both of which are associated with limitations including drug-related adverse effects and motor blockade. This narrative review evaluates temporary percutaneous peripheral nerve stimulation (PNS) as an advanced, opioid- and motor-sparing neuromodulation strategy for postoperative pain management. RECENT FINDINGS: Temporary PNS selectively targets sensory afferent fibers to provide immediate nociceptive inhibition while promoting neuroplastic changes that reverse central sensitizat…

Epidural and Intrathecal Catheter Use at the End of Life for Cancer Pain.

PURPOSE: More than 80% of patients with advanced malignancy experience pain as they approach death, and up to 30% fail to achieve adequate relief from systemic analgesics or cannot tolerate their side effects. For these patients, externalized epidural or intrathecal catheters offer a targeted alternative, delivering analgesic agents directly to the neuraxis. This narrative review examines patient selection, technical approaches, pharmacological regimens, clinical outcomes, and complications associated with externalized epidural and intrathecal catheter systems for refractory cancer pain at end

Pain, Analgesia, Sedation, and the Developing Brain.

Robust evidence informs our understanding of the impacts of neonatal pain on brain growth and development and outcomes in childhood and adult life. However, evidence regarding optimal assessment and treatment in common clinical scenarios remains incomplete. In this setting, clinicians must synthesize available evidence regarding efficacy, and prioritize safety. Nonpharmacologic interventions should be systematic, safely implemented, and incorporate parents, when possible. Pharmacologic interventions should be protocolized for painful procedures, reflecting the magnitude and duration of the pai
