Using routinely reported tuberculosis genotyping and surveillance data to predict tuberculosis outbreaks.

We combined routinely reported tuberculosis (TB) patient characteristics with genotyping data and measures of geospatial concentration to predict which small clusters (i.e., consisting of only 3 TB patients) in the United States were most likely to become outbreaks of at least 6 TB cases. Of 146 clusters analyzed, 16 (11.0%) grew into outbreaks. Clusters most likely to become outbreaks were those in which at least 1 of the first 3 patients reported homelessness or excess alcohol or illicit drug use or was incarcerated at the time of TB diagnosis and in which the cluster grew rapidly (i.e., the

Short review: field recovery and potential information value of small elements of the skeleton.

The recovery of small elements of the skeleton (e.g. hyoid, carpals, and hand and foot phalanges) is one of the established tasks of the archaeologist and physical anthropologist when working in the field, whether in an archaeological or forensic context. In the present work, we illustrate the field location of ossified laryngeal cartilages, hand sesamoids, and the medial clavicular epiphyses. The potential information offered by these elements is briefly summarized. The frequency of these elements observed in a cemetery dating from 1943 indicates the possibility that these elements could be f

Predictors of drug treatment completion among parole violators.

This study examined the predictors of treatment completion among 380 state parole violators consecutively admitted to a comprehensive 12-month drug treatment program in lieu of reincarceration. Offenders were placed on intensive parole supervision throughout the 12-month treatment protocol and received three months of residential substance abuse treatment followed by nine months of outpatient counseling. Overall 123 (32.4%) of the offenders completed the 12-month treatment protocol. The primary reason for noncompletion was a positive drug screen. Bivariate analyses were performed to determine

Childhood abuse and aggression in girls: the contribution of borderline personality disorder.

The authors tested whether emerging borderline personality disorder (BPD) symptoms mediated the association between childhood physical abuse (CPA) and aggression among incarcerated girls. Participants were 121 incarcerated adolescent girls (13-19 years old). Three forms of aggression (relational, overt, and violent offending behavior) and exposure to CPA by a parental figure were assessed using self-report inventories, whereas BPD symptoms were evaluated using a structured interview. Mediation models, including tests of indirect effects, were conducted in which each form of aggression was pred

Drug courts: a primer for the family physician.

BACKGROUND: Drug courts are a judicial response to drug-related crimes. They combine the coercive powers of the judiciary with drug treatment. This article is intended to familiarize physicians with the reasons why drug courts exist, what they are, and how physicians can assist their patients who are clients of a drug court. METHODS: Methods for this article are based upon personal experience and a search of the literature using the key words "drug rehabilitation," "drug abuse," and "criminal justice system." RESULTS: Using a three-phase approach, drug courts emphasize urine drug testing, ra

Findings from STD screening of adolescents and adults entering corrections facilities: implications for STD control strategies.

BACKGROUND: Persons entering corrections facilities are at high risk for sexually transmitted diseases (STDs) because of risky sexual behavior and lack of access to routine screening. GOAL: The goal of the study was to develop a national picture of STD prevalence in this population. STUDY DESIGN: We analyzed information on age, race/ethnicity, urethral symptoms (men only), and test results for approximately 85,000 chlamydia, 157,000 gonorrhea, and 293,000 syphilis tests for persons entering 23 jails and 12 juvenile detention centers in 13 US counties from 1996 through 1999. RESULTS: At adul

Biology and immunology of human strongyloidiasis.

Strongyloidiasis is an important human parasitic infection primarily because of its potential for serious and even lethal disease in immunosuppressed patients. Certain features of the biology of Strongyloides stercoralis, such as the ability to autoinfect the host, help explain why the parasite is dangerous. Characteristics of the parasite can also be exploited in parasitological diagnosis of infection with the Baermann concentration procedure and by culture. Use of filariform larval antigens in ELISA or immunofluorescent-antibody tests now provide reliable serological tests for diagnosis, and

Mental health issues and associated factors amongst healthcare workers in US forensic-correctional settings: a systematic review of literature since the COVID-19 pandemic.

BACKGROUND: Healthcare professionals provide essential services to populations in the criminal justice system, often at the expense of their own well-being. This review synthesized literature findings on mental health challenges faced by healthcare professionals working in the US forensic-correctional settings since the COVID-19 pandemic. We investigated the prevalence of mental health conditions, their risk-protective factors, the impacts of these mental health issues on workplace retention, and highlighted relevant recommendations. METHODS: This study followed PRISMA guidelines. A comprehen

Parental incarceration and psychiatric disorders, suicidal behavior, risk-taking, and substance misuse events in offspring: A longitudinal within-individual study.

BACKGROUND: Parental incarceration (PI) is associated with mental and behavioral problems in offspring, but causality remains unclear. We conducted a quasi-experimental within-individual study on the associations of PI with offspring psychiatric disorders, suicidal behavior, risk-taking, and substance misuse events, aiming to test the effect of PI on offspring. METHODS: The study included 43,011 and 5,912 individuals born in Sweden 1973-2010 and exposed to paternal or maternal incarceration, respectively, between ages 10 and 30. Using data from nationwide registers available until the end of

Availability of medications for opioid use disorder and hepatitis C services in New York state jails: The New York state integrated jail survey.

OBJECTIVES: There is a significant knowledge gap regarding medication for opioid use disorder (MOUD) and hepatitis C virus (HCV) service provision in U.S. jails. To address this gap, characterized screening, treatment, and discharge planning capacity for OUD and HCV in New York State (NYS) jails outside of New York City. STUDY DESIGN: Cross-sectional study. METHODS: Between August and September 2022, 46 of 58 (80%) NYS jails completed a NYS Department of Health survey to determine jails' capacities to implement MOUD and capacity to screen, test and treat HCV. Surveys were completed prior to

Links among Parental Incarceration, Food Insecurity, Household Adverse Childhood Experiences, and Disordered Eating in United States Youth.

OBJECTIVE: To examine whether parental incarceration is related to increased risk for disordered eating (ie, fasting, binge eating, purging, and compulsive exercise) among children and adolescents in the US. STUDY DESIGN: We used data from the 2022 and 2023 National Survey of Children's Health, an annual cross-sectional, nationally representative survey of US children. Analyses were restricted to youth ages 6-17 with parental incarceration data (n = 65 321). Logistic regression models were conducted, adjusting for age, sex, race, ethnicity, parental education, and body mass index. RESULTS: B

Assessing the comparative effectiveness of ECHO and coaching implementation strategies in a jail/provider MOUD implementation trial.

BACKGROUND: For nearly two decades, it has been widely recognized that individuals in jail settings have a high prevalence of opioid use disorders (OUD) and are highly susceptible to fatal overdose upon their release. This setting provides a public health opportunity to address OUD with Medication for Opioid Use Disorders (MOUDs). Yet, 56% of jails do not provide MOUD, creating a pressing need for better implementation approaches in jail and the hand-off to the community. Two successful implementation strategies, NIATx external coaching and the Extension for Community Healthcare Outcomes (ECHO

Precarious perinatal care: experiences of incarcerated individuals in Ontario, Canada.

BACKGROUND: Incarcerated pregnant individuals face substantial barriers to timely and appropriate perinatal care, which may contribute to poorer maternal and newborn health outcomes compared to the general population. Their experiences, both during incarceration and post-release, offer critical insight into systemic obstacles affecting access to care. This study explores the perspectives of individuals who were pregnant while incarcerated in Ontario, Canada, examining their perinatal experiences and the factors contributing to adverse health outcomes for them and their newborns. METHODS: We u

Jail Characteristics and Availability of Opioid Treatment Services: Results from a Nationally Representative Survey.

The current wave of the opioid epidemic has contributed to a record number of drug-related overdoses and a significant proportion of people who experience opioid use disorder are admitted to local jails. These correctional facilities serve as the principal entry point to the criminal justice system as nearly every person who is taken into custody is admitted to a local detention center. Although jails are recognized as primary intervention points for people who may require treatment for opioid use disorder, services in these facilities remain deficient. The absence of jail-based treatment has

Cost-effectiveness and health impact of screening and treatment of Mycobacterium tuberculosis infection among formerly incarcerated individuals in Brazil: a Markov modelling study.

BACKGROUND: Individuals who were formerly incarcerated have high tuberculosis incidence, but are generally not considered among the risk groups eligible for tuberculosis prevention. We investigated the potential health impact and cost-effectiveness of Mycobacterium tuberculosis infection screening and tuberculosis preventive treatment (TPT) for individuals who were formerly incarcerated in Brazil. METHODS: Using published evidence for Brazil, we constructed a Markov state transition model estimating tuberculosis-related health outcomes and costs among individuals who were formerly incarcerate

Mediation of the Effect of Incarceration on Selling Sex Among Black Sexual Minority Men and Black Transgender Women in the HPTN 061 Study.

Incarceration among Black sexual minority men and Black transgender women (BSMM/BTW) is disproportionately high in the United States. Limited research has documented the disruptive effect of incarceration on sexual networks and sexual partnership exchange among BSMM/BTW. We estimate the influence of incarceration on selling sex and mediating pathways among 1169 BSMM/BTW enrolled in the HIV Prevention Trials Network (HPTN) 061 cohort to assess this relationship. Mediators investigated were social support, violence, illicit drug use, and distress due to experienced racism and homophobia. During
