Transient exacerbation of tics in treatment of Tourette's syndrome with clonidine.

An 11-year-old boy with a history of behavioral problems was admitted to the inpatient Child Development Program of St. Louis University Medical Center. A diagnosis of Tourette's syndrome was made, and when family objected to a trial of haloperidol for fear of tardive dyskinesia, clonidine was initiated. A temporary period of worsening of motor and phonic tics ensued, while dose adjustments were made. When a maintenance level was achieved, approximately 2 weeks after initiating treatment, remission of symptoms was observed.

Neurobiological approaches in human behavior genetics.

An attempt should be made to base analysis of problems in human behavior genetics on existing knowledge of human biochemical genetics and neurobiology. Examples for this approach are studies showing HY antigen patterns of the opposite sex in transsexuality, slight psychological deviations in heterozygotes of recessive metabolic diseases such as phenylketonuria and lipid storage diseases, and psychological studies in healthy individuals with various genetic variants of the normal human electroencephalogram (EEG). Results of such studies will help gradually to replace emotional controversy by ra

Malignant fibrous histiocytoma of the head and neck.

Eight cases, the largest series of malignant fibrous histiocytomas of the head and neck to date, are reported. In over one-half of the cases, the initial problems encountered in the case made the true diagnosis misleading. For correct diagnosis multiple biopsies may be required. The diagnosis is further confirmed by the clinical behavior of the tumor and its site, size, and depth of involvement. Four specific histopathologic variants are described. Wide surgical excision is the preferrred treatment. The potential role of adjunctive irradiation should be considered.

Long-term impact of necrotizing enterocolitis on somatosensory function in preterm born children.

BACKGROUND: Necrotizing enterocolitis (NEC) is a painful intestinal disease in preterm infants that causes severe prolonged pain requiring high amounts of analgesics. In a critical stage of neurodevelopment, pain and analgesics potentially impact sensory development. This study compared somatosensory function and sensory processing in preterm-born children with and without a history of NEC, as well as key self-/parent-reported outcomes. METHODS: In this cross-sectional cohort study, preterm-born children (6-15 years old) with NEC were matched with controls. Each child underwent quantitative s

Algorithm-Based Mobile Texting Platform for the Self-Management of Interstitial Cystitis/Bladder Pain Syndrome: Pilot Study Evaluating Feasibility, Usability, and Potential Utility.

INTRODUCTION: We developed an algorithm-based mobile texting platform for promoting self-management in patients with interstitial cystitis/bladder pain syndrome (IC/BPS). Our aim was to measure the feasibility, usability, and potential utility of the platform. METHODS: A texting platform that delivered 4 treatment modules (education and behavioral modification, cognitive behavioral therapy, pelvic floor physical therapy, and guided mindfulness practices) and an automated weekly message over 6 weeks was developed. Feasibility and usability were determined using patient engagement (proportion o

Externalizing psychopathology from childhood to early adolescence: Psychometric evaluation using latent variable and network modeling.

Applying both latent variable and network frameworks, we conducted a comprehensive psychometric evaluation of the diverse array of symptoms from three externalizing dimensions, including attention problems, aggressive behavior, and delinquency/rule-breaking of the Child Behavior Checklist (Achenbach, 1991) across six time points from childhood to early adolescence. We also examined sex differences. Participants (N = 1,339) were drawn from two multisite longitudinal studies: Fast Track and the Child Development Project. Parents reported on externalizing psychopathology in kindergarten and Grade

Testing the efficacy of a brief sexual risk reduction intervention among high-risk American Indian adults: study protocol for a randomized controlled trial.

BACKGROUND: American Indian adults are more likely to experience co-occurring mental health and substance use disorders than adults of other racial/ethnic groups and are disproportionately burdened by the most common sexually transmitted infections, namely chlamydia and gonorrhea. Several behavioral interventions are proven efficacious in lowering risk for sexually transmitted infection in various populations and, if adapted to address barriers experienced by American Indian adults who suffer from mental health and substance use problems, may be useful for dissemination in American Indian comm

Variation in brain regions associated with fear and learning in contrasting climates.

In environments where resources are difficult to obtain and enhanced cognitive capabilities might be adaptive, brain structures associated with cognitive traits may also be enhanced. In our previous studies, we documented a clear and significant relationship among environmental conditions, memory and hippocampal structure using ten populations of black-capped chickadees (Poecile atricapillus) over a large geographic range. In addition, focusing on just the two populations from the geographical extremes of our large-scale comparison, Alaska and Kansas, we found enhanced problem-solving capabili

Gender and racial/ethnic differences in addiction severity, HIV risk, and quality of life among adults in opioid detoxification: results from the National Drug Abuse Treatment Clinical Trials Network.

PURPOSE: Detoxification often serves as an initial contact for treatment and represents an opportunity for engaging patients in aftercare to prevent relapse. However, there is limited information concerning clinical profiles of individuals seeking detoxification, and the opportunity to engage patients in detoxification for aftercare often is missed. This study examined clinical profiles of a geographically diverse sample of opioid-dependent adults in detoxification to discern the treatment needs of a growing number of women and whites with opioid addiction and to inform interventions aimed at

Normal cognition and behavior in a Smith-Lemli-Opitz syndrome patient who presented with Hirschsprung disease.

Smith-Lemli-Opitz syndrome (SLOS) is an autosomal recessive disorder of cholesterol biosynthesis. It is caused by mutations in the gene encoding the enzyme 7-dehydrocholesterol Delta7-reductase (DHCR7), which catalyzes the final step in cholesterol biosynthesis, usually resulting in cholesterol deficiency. We report a 3.5-year-old girl who has cognition in the low average range and normal behavior, but in whom molecular studies identified two missense mutations in DHCR7: V326L and F284L. She was born at term following an uncomplicated pregnancy and delivery, and presented at 12 days of age wit

The Trajectory Statistics of Biological Exploratory Dynamics.

Biological processes, from molecules diffusing to their regulatory destinations to whales pursuing food or mates, are widely exploratory. Such behaviors are effective when some verifiable functional state or outcome can be reached regardless of initial conditions. In these cases, systems repeatedly undergo distinct and abortive trajectories; the process only ends when the system finds ``right'' outcomes. This dominance of the terminal condition (and indifference to the initial state) provokes a very different perspective than the conventional ``dynamical systems'' framework that has been a cen

The effectiveness of protective behavioral strategies for reducing alcohol use and alcohol-related harms among adults: A systematic review and meta-analysis of randomized controlled trials.

OBJECTIVES: Excessive alcohol consumption is a major public health concern. Protective behavioral strategies (PBS) are widely incorporated into alcohol interventions, but their effectiveness and role as mechanisms of behavior change remain unclear. This systematic review and meta-analysis evaluated the effects of PBS-based interventions on alcohol consumption, alcohol-related harms, and PBS use. METHODS: PubMed, Embase, Scopus, CINAHL, and ProQuest were searched from inception to December 2025 for randomized controlled trials (RCTs) of adults (≥18 years) delivering PBS as an active interventi

Association of whole-body and regional body fat mass indexes with depressive symptoms and suicidal behaviors in Korean adults: The moderating role of age.

BACKGROUND: The coexistence of obesity and mental health problems has been widely reported; however, the moderating role of age in these associations has been scarcely examined. We explored age-specific variations in the association of fat mass indexes (FMIs) with depressive symptoms and suicidal behaviors. METHODS: This study included a nationwide sample of 4185 Korean adults. Whole-body and regional body fat mass were measured using bioelectrical impedance analysis. Depressive symptoms were determined based on the Patient Health Questionnaire-9, and suicidal ideation and planning over the p

Noneigenstate Ab Initio Density Matrix Downfolding for Constructing Model Hamiltonians in Quantum Chemistry.

Model Hamiltonians represent a convenient way of reducing complex problems of many-electron quantum mechanics to much simpler problems: they can fully reproduce the core behaviors of a system of interest by encoding only the dominant physical interactions and using only a small number of associated parameters. Model Hamiltonians have been successfully applied to describe many chemical and physical phenomena. Density matrix downfolding (DMD) [ J. Chem. Phys. 2015, 143 (10), 102814] allows the derivation of model Hamiltonians of any form in a systematically improvable fashion by matching the ene

FMR1 Methylation Pattern and Repeat Expansion Screening in a Cohort of Boys with Autism Spectrum Disorders: Correlation of Genetic Findings with Clinical Presentations.

BACKGROUND/OBJECTIVES: Autism spectrum disorders (ASDs) are neurodevelopmental conditions with early onset of clinical manifestations. ASD etiology is highly heterogeneous, with genetic factors being strong determinants of the behavioral problems and neurodevelopmental deficits. Fragile X syndrome (FXS) (OMIM #300624), caused by the transcriptional silencing of the FMR1 gene, represents the most common monogenic cause of autism. Our study included 226 boys with a diagnosis of ASD, for a systematic screening of genetic and epigenetic defects in the FMR1 gene promoter in a Romanian pediatric coh
