Methods other than tilt testing for diagnosing neurocardiogenic (neurally mediated) syncope.

The recording of spontaneous episodes of bradycardiac neurocardiogenic syncope (NCS) has shown that: a prolonged ventricular asystole seems … robably able to correctly identify most patients affected by NCS. Nevertheless, whether the type of reflex induced by the cardiovascular reflexivity maneuvers correlates with that of the spontaneous syncope is largely unknown. Our knowledge suggests that the correlation may be unsatisfactory, owing to the following: the variability of the mechanism of sp …

Modification of sexual behavior of Long-Evans male rats by drugs acting on the 5-HT1A receptor.

Modulation of the sexual behavior of male rats by the anxiolytic buspirone (S-20499) and its analog gepirone were compared to the effects of 8-OH-DPAT (or DPAT, a selective 5-HT1A reference agonist), and BMY-7378 (a selective 5-HT1A partial agonist). Long-Evans rats were used; modulation of copulatory behavior and alteration of penile reflexes were examined. Modulation of copulatory behavior was assessed by three indices: frequency and length of intromission, and latency of ejaculation. DPAT, at doses of 1-8 mg/kg, reduced these three indices in a time dependent manner such that the effects pe…

Ventriculophasic response in a horse with atrioventricular block.

Ventriculophasic response (VR) in atrioventricular nodal (AVN) block is characterized by a shortening of the sinoatrial cycle length when a QRS complex is interposed between two P waves. This has been well documented in humans and dogs with AVN blocks but not in horses. Herein, we describe a horse with second-degree AVN blocks and constant PR intervals that exhibited 5:3 conduction and suspected VR at rest but less consistently after submaximal exercise. Proposed VR mechanisms include (1) inhibition of the Bezold-Jarisch reflex from reduced end-systolic volume during ventricular contraction; (…

Transient Receptor Potential Channels in Sensory Mechanisms of the Lower Urinary Tract.

BACKGROUND: Urine storage and excretion require a network of interactions in the urinary tract and the central nervous system, which is mediated by a reservoir of water in the bladder and the outlet to the bladder neck, urethra, and external urethral sphincter. Through communicating and coordinating each other, micturition system eventually showed a switch-like activity pattern. SUMMARY: At cervicothoracic and lumbosacral spine, the spinal reflex pathway of the lower urinary tract (LUT) received mechanosensory input from the urothelium to regulate the bladder contraction activity, thereby con…

Teaching Video NeuroImage: Isolated Undulating Tongue Hyperkinesia Following a Basilar Stroke.

An 82-year-old man with a history of hypertension and coronary revascularization presented with sudden-onset right hemiparesis and disorientation lasting 5 hours. On admission, he was intubated because of gasping and a Glasgow Coma Scale of 3. Hemorrhagic stroke was suspected, but ruled out by the initial head CT, which revealed old cerebellar lacunae. The following day, the comatose, now unsedated patient exhibited tetraparesis; fixed, nonreactive pupils; and corneal reflex, but no oculocephalic reflex. Rhythmic undulating tongue movements without palatal or limb involvement were first observ…

The influence of late prematurity on the encephalopathy exam of infants with neonatal encephalopathy.

BACKGROUND: Late preterm (LPT) infants are increasingly treated for hypoxic-ischemic encephalopathy (HIE). However, neurodevelopmental differences of LPT infants may independently influence the neurologic exam and confound care. METHODS: Perinatal and outcome characteristics were extracted along with the worst autonomic and state/neuromuscular/reflex Sarnat components in a cross-section of infants with moderate/severe HIE. Infants were classified as late preterm (LPT, 34-36 weeks) or term (>36 weeks). RESULTS: 250 infants were identified, 55 were late preterm. LPT infants had lower mean gest…

Distinct forms of synaptic plasticity during ascending vs descending control of medial olivocochlear efferent neurons.

Activity in each brain region is shaped by the convergence of ascending and descending axonal pathways, and the balance and characteristics of these determine the neural output. The medial olivocochlear (MOC) efferent system is part of a reflex arc that critically controls auditory sensitivity. Multiple central pathways contact MOC neurons, raising the question of how a reflex arc could be engaged by diverse inputs. We examined functional properties of synapses onto brainstem MOC neurons from ascending (ventral cochlear nucleus, VCN) and descending (inferior colliculus, IC) sources in mice usi…

Afferent Visual Manifestations of Traumatic Brain Injury.

Traumatic brain injury (TBI) causes structural and functional damage to the central nervous system including the visual pathway. Defects in the afferent visual pathways affect visual function and in severe cases cause complete visual loss. Visual dysfunction is detectable by structural and functional ophthalmic examinations that are routine in the eye clinic, including examination of the pupillary light reflex and optical coherence tomography (OCT). Assessment of pupillary light reflex is a non-invasive assessment combining afferent and efferent visual function. While a assessment using a flas…

Motor cortical excitability during voluntary inhibition of involuntary tic movements.

BACKGROUND: Tics can be voluntarily inhibited. However, the neurophysiology of voluntary tic inhibition remains underexplored. The objective of this study was to explore state-dependent effects of voluntary tic inhibition on M1 excitability. METHODS: Neurophysiological assessments (single motor-evoked potentials, corticospinal recruitment curves, short-interval intracortical inhibition, H-reflex) were performed in 14 adults with Tourette syndrome during voluntary tic inhibition and free ticcing. Regressions between behavioral performance and neurophysiological measures were also performed. R…

Effect of central lesions on a spinal circuit facilitating human wrist flexors.

A putative spinal circuit with convergent inputs facilitating human wrist flexors has been recently described. This study investigated how central nervous system lesions may affect this pathway. We measured the flexor carpi radialis H-reflex conditioned with stimulation above motor threshold to the extensor carpi radialis at different intervals in fifteen patients with stroke and nine with spinal cord injury. Measurements after stroke revealed a prolonged facilitation of the H-reflex, which replaced the later suppression seen in healthy subjects at longer intervals (30-60 ms). Measurements in …

Glutamatergic Receptor Activation in the Commisural Nucleus Tractus Solitarii (cNTS) Mediates Brain Glucose Retention (BGR) Response to Anoxic Carotid Chemoreceptor (CChr) Stimulation in Rats.

Glutamate, released from central terminals of glossopharyngeal nerve, is a major excitatory neurotransmitter of commissural nucleus tractus solitarii (cNTS) afferent terminals, and brain derived neurotrophic factor (BDNF) has been shown to attenuate glutamatergic AMPA currents in NTS neurons. To test the hypothesis that AMPA contributes to glucose regulation in vivo modulating the hyperglycemic reflex with brain glucose retention (BGR), we microinjected AMPA and NBQX (AMPA antagonist) into the cNTS before carotid chemoreceptor stimulation in anesthetized normal Wistar rats, while hyperglycemic…

Hypoxia-inducible factors and hypertension: lessons from sleep apnea syndrome.

Systemic hypertension is one of the most prevalent cardiovascular diseases. Sleep-disordered breathing (SDB) with recurrent apnea is a major risk factor for developing essential hypertension. Chronic intermittent hypoxia (CIH) is a hallmark manifestation of recurrent apnea. Rodent models patterned after the O2 profiles seen with SDB patients showed that CIH is the major stimulus for causing systemic hypertension. This article reviews the physiological and molecular basis of CIH-induced hypertension. Physiological studies have identified that augmented carotid body chemosensory reflex and the r…

V1 and v2b interneurons secure the alternating flexor-extensor motor activity mice require for limbed locomotion.

Reciprocal activation of flexor and extensor muscles constitutes the fundamental mechanism that tetrapod vertebrates use for locomotion and limb-driven reflex behaviors. This aspect of motor coordination is controlled by inhibitory neurons in the spinal cord; however, the identity of the spinal interneurons that serve this function is not known. Here, we show that the production of an alternating flexor-extensor motor rhythm depends on the composite activities of two classes of ventrally located inhibitory neurons, V1 and V2b interneurons (INs). Abrogating V1 and V2b IN-derived neurotransmissi…

Differential involvement of two cortical masticatory areas in submandibular salivary secretion in rats.

To evaluate the role of the masticatory area in the cerebral cortex in the masticatory-salivary reflex, we investigated submandibular salivary secretion, jaw-movement trajectory and electromyographic activity of the jaw-opener (digastric) and jaw-closer (masseter) muscles evoked by repetitive electrical stimulation of the cortical masticatory area in anesthetized rats. Rats have two cortical masticatory areas: the anterior area (A-area) in the orofacial motor cortex, and the posterior area (P-area) in the insular cortex. Our defined P-area extended more caudally than the previous reported one.…
