Since 1990, early surgery within 3 days following subarachnoid hemorrhage has been performed routinely in our hospital even for ruptured posterior circulation aneurysms. We report a new method for shoulder sonography (US): the patient lies supine and his/her arm is positioned so as to optimize tendon depiction. The supraspinatus tendon, which is the most frequently involved structure in rotator cuff tears, is studied with the patient's arm dropped, elbow extended and forearm prone. Eight cases of rupture of the posterior urethra were observed in children and treated via a trans-symphyseal approach. Repair was easy in the cases seen rapidly (2 to 10 days), urethral suture was effective and the result was excellent. On the other hand, in cases seen later (6 cases), between 6 months and 5 years, marked fibrous callus had to be resected. Urethral repair was not always possible (only one failure out of 6 late cases). This approach preserves genital functions.

Hydatic cysts of the liver can rupture into the thorax. The aim of this work was to demonstrate how abdominal access can be used in most cases. We report 44 cases of hydatic cysts which ruptured into the thorax among a series of 1411 hydatic cysts of the liver operated between 1974 and 1995. A 61-year-old man with hypertrophic obstructive cardiomyopathy and ruptured chordae tendineae but subsequent disappearance of the obstruction received mitral valve replacement 4 years after the diagnosis of ruptured chordae tendineae. Chordal rupture should be recognized as one of the important causes of mitral regurgitation in patients with hypertrophic obstructive cardiomyopathy.

Traumatic rupture of a splenic epidermoid cyst is a rare but recognised cause of "acute abdomen" in a child. We describe such a case occurring after very minor trauma, show the influence that imaging had in making an accurate diagnosis, and discuss how this influenced clinical management.

Although the etiology of PROM is multifactorial, increasing evidence regarding clinical risk factors, membrane histology, membrane culture, and amniotic fluid microbiology shows a strong association with infection. Sixty patients with acute rupture of the Achilles tendon were included in a prospective study and assigned randomly for operative treatment using a 4-string suture with either a new mobile cast (30) or a rigid below-knee cast (30). A case of pregnancy in a rudimentary uterine horn with rupture 15 weeks after the LMP is described. Ultrasonography at the beginning of such a pregnancy should allow the diagnosis of these uterine malformations. Closer monitoring of the pregnancy is necessary because of the seriousness of potential complications. The frequent finding of placenta accreta in such situations is pointed out. Management approaches are described.

BACKGROUND: Uterine septa may be responsible for spontaneous abortion. Hysteroscopic metroplasty may improve pregnancy outcome.
CASE: A patient with a history of hysteroscopic resection of a uterine septum presented in preterm labor. This was her second pregnancy following the procedure; the first resulted in a term vaginal delivery. This second pregnancy ended with a preterm cesarean delivery. A 3.5 x 3.5-cm fundal rupture was discovered during the cesarean. The metroplasty had been performed with the KTP laser under direct laparoscopic visualization. No surgical complications had occurred.

Report on a case of acute vital complication of a conservatively treated lumbar vertebra fracture. Nine days after traumatic fracture of the first lumbar vertebra a 62-year old patient suddenly suffered from haemorrhagic shock caused by a minor rupture of the abdominal aorta. Before this dramatic event there were no symptoms pointing to this severe complication. Aetiology, diagnosis and therapy are discussed.
