This article describes the management of a 57-year-old female patient following a fracture and dislocation of the right humeral head. The treatment of the patient involved the use of thermal agents, manual therapy, continuous passive motion, and splinting of the arm in an elevated position. We describe an approach to treatment of limited shoulder motion that is focused on identifying and applying tension to restricting structures rather than restoration of translatory gliding movements of the humeral head. Our treatment approach is based on recent data from biomechanical studies that challenge

Sixteen cases with arthrogryposis multiplex congenita were examined clinically and electromyographically; three of them were re-examined later. Joint deformities were present in all extremities in 13 of the cases; in eight there was some degree of mental retardation. In two cases, there was clinical and electromyographic evidence of a myopathic disorder. In the majority, the appearances of the shoulder-neck region suggested a developmental defect. At the same time, selective weakness of muscles innervated by C5-C6 segments suggested a neuropathic disturbance. EMG revealed, in eight of 13 cases

BACKGROUND: Muscle imbalance about the shoulder in children with persistent brachial plexus birth palsy is thought to contribute to glenohumeral joint deformity. We quantified cross-sectional areas of the internal and external rotator muscles in the shoulder by magnetic resonance imaging in patients with chronic brachial plexopathy and the correlation between these muscle cross-sectional area ratios and glenohumeral deformity. The purposes of this investigation were to evaluate differences in the ratios between affected and unaffected shoulders in the same individual and to assess whether an i

PURPOSE OF THE STUDY: The reconstructions of Bankart lesions, carried out according to the procedure recommended by the Mitek company, showed a high rate of recurrent dislocation. Therefore, based on operative experience, anatomical studies and reconstructive surgery on cadaver shoulders, we developed a new surgical technique that provided good shoulder stability and better operative outcomes. The results are reported here.
MATERIAL: Between 1997 and 2005, arthroscopic Bankart repair with use of Mitek anchors was performed on 368 shoulders at the Department of Orthopedics, Faculty of Medicine

PURPOSE: To evaluate the role, the difficulties of rehabilitation and to diagnose the eventual surgical complications after shoulder prosthesis for 4-part fractures.
MATERIAL AND METHODS: Forty three patients (46 shoulders) who underwent shoulder arthroplasty after fracture of the proximal humerus underwent rehabilitation and follow-up at a special reeducation center for an average of 3 months (1 to 6). There were 42 four-part fractures (with 22 fracture-dislocation) and 4 three-part fractures. The patients were send by five different hospitals and have all been operated by senior surgeons. T

Calcium pyrophosphate dihydrate deposition (CPPD), commonly known as pseudogout, is an inflammatory arthropathy primarily affecting the knee, wrist, hip, and shoulder joints. However, it can occasionally deposit in various structures surrounding the spinal column, including the facet joints, ligamentum flavum, bursae, and intervertebral discs. Such occurrences are typically asymptomatic or associated with mild neck pain. Nonetheless, severe cases may lead to myeloradiculopathy, characterized by severe neck pain and upper extremity weakness. Conservative management with nonsteroidal anti-inflam…

Ehlers-Danlos syndrome (EDS) is a multifaceted debilitating disease. Affected patients are at risk for complications such as joint hypermobility and cardiac disease, but the prevalence, course, and management of these conditions are not well understood. The objective of this retrospective cohort study was to investigate the demographic characteristics and systemic manifestations in EDS. We performed a retrospective analysis of 98 EDS patients seen in a physical medicine and rehabilitation clinic between January 2015 and April 2019. Charts were reviewed for demographic information, subtype of E

This is the first report for a Japanese case of arthrochalasia type of Ehlers-Danlos syndrome (EDS). A 46-year-old woman consulted us for joint hypermobility and skin hyperextensibility that had been present soon after birth. There was no family history of a similar disease. She was diagnosed as having bilateral congenital hip dislocation and bilateral habitual shoulder dislocation at her childhood. Her skin was velvety, doughy and hyperextensible. She showed hypermobility of the joints of the hands and feet and generalized joint laxity, with no evidence of scoliosis. Electrophoretic analysis

The choice of the most appropriate procedure for surgical treatment of type IV-V and VI dislocations of the acromioclavicular joint according to Rockwood's classification has always been troublesome because of complications such as residual instability of the joint, delayed arthrosis of the A-C, delayed osteolysis of the clavicle, host intolerance towards artificial ligaments and because of the need of early mobilization of the affected limb. In our study, 17 male patients, ranging in age from 21 to 79 years and affected by A-C dislocation grade IV and V, both acute and chronic, underwent surg

Arthrogryposis of the upper extremity is easy to diagnose. The shoulders, when affected, are adducted and internally rotated; they are thin, and very little girdle muscle is noted. The elbows are usually straight, and extension contractures are present. The hand and wrist are clublike; the wrist is contracted in flexion, with slight ulnar deviation. The thumb is usually adducted and flexed in a palmar direction. The small joints of the fingers are stiff, and frequently the fingers are ulnar deviated. Early treatment consists of passive stretching of the contracted parts by either plaster casts

INTRODUCTION: Frozen shoulder (FS) is a chronic and painful joint disorder characterized by progressive stiffness and functional limitation. Its multifactorial pathophysiology remains poorly understood, although emerging evidence suggests a role for low-grade inflammation, metabolic dysregulation, and immune-endocrine imbalance. Nutrition and lifestyle factors may therefore represent modifiable contributors to disease development and progression.
METHODS: This cross-sectional observational study investigated the association between dietary profile, physical activity, pain intensity, and shoul

INTRODUCTION: Low-energy traumatic irreducible anterior shoulder dislocations with concomitant greater tuberosity fractures in elderly females are rarely reported. The difficulty in reduction is attributed to the interplay between osseous impaction and soft tissue interposition. Three-dimensional computed tomography (CT) was utilized to delineate this combined obstruction mechanism. An open surgical approach incorporating joystick reduction techniques and preservation of the long head of the biceps tendon was implemented. To our knowledge, no systematic therapeutic protocols have been establis

Proximal humeral fractures, predominantly affecting the elderly, pose significant treatment challenges due to the complex anatomy of the shoulder joint and variability in bone quality. MultiLoc nails (Synthes USA Products, West Chester, USA) are the latest construct, and PHILOS (Proximal Humerus Internal Locking System, (Synthes USA Products, West Chester, USA)) plates are the earlier construct used for the fixation of proximal humerus fractures. This systematic review aims to provide a comparison of MultiLoc nails and PHILOS plates, focusing on their effectiveness, safety, and patient outcome…

BACKGROUND: Adhesive capsulitis or contracted shoulder, known as frozen shoulder, is a persistent painful condition that may last for more than three months. It is a common disease-causing morbidity that causes pain and loss of shoulder range of motion. Physical therapy is advocated for the restoration of a pain-free state and normal use of the upper extremity, along with other interventions.
PURPOSE: This study aimed to explore the level of current clinical practice for managing chronic contracted frozen shoulder (CCFS) among physiotherapy professionals in the United Arab Emirates (UAE) comp

BACKGROUND: Adhesive capsulitis is a common shoulder disorder inducing joint capsule fibrosis and pain. When combined with rotator cuff tear (RCT), treatments can be more complex. Currently, targeted therapy is lacking. Since adhesive capsulitis is reported to be related to circulating materials, we analyzed the contents and biology of circulating exosomes from RCT patients with and without adhesive capsulitis, in an attempt to developing a targeting treatment.
METHODS: Samples from a consecutive cohort of patients with RCT for surgery were collected. Circulating exosomal miRNAs sequencing we

Periprosthetic joint infections of the shoulder (PJIS) are the major cause for revision within the first two post-operative years, and are challenging both to diagnose and treat. Success depends on early identification of microorganisms, appropriate surgical procedures and efficient antibiotic administration. The peculiar microbiology of the shoulder may render the criteria for hip/knee PJI management inappropriate. In addition, later cases with clinically subtle signs often present diagnostic challenges. In recent years, specific issues of PJIS have been managed through the use of new instrum

AIMS: Advances in arthroscopic techniques for rotator cuff repair have made the mini-open approach less popular. However, the mini-open approach remains an important technique for repair for many surgeons. The aims of this study were to compare the integrity of the repair, the function of the shoulder and satisfaction post-operatively using these two techniques in patients aged > 50 years.
PATIENTS AND METHODS: We identified 22 patients treated with mini-open and 128 patients treated with arthroscopic rotator cuff repair of July 2007 and June 2011. The mean follow-up was two years (1 to 5). O

CONTEXT: The role of the rotator cuff is to provide dynamic stability to the glenohumeral joint. Human and animal studies have identified sarcomerogenesis as an outcome of eccentric training indicated by more torque generation with the muscle in a lengthened position.
OBJECTIVE: The authors hypothesized that a home-based eccentric-exercise program could increase the shoulder external rotators' eccentric strength at terminal internal rotation (IR).
DESIGN: Prospective case series.
SETTING: Clinical laboratory and home exercising.
PARTICIPANTS: 10 healthy subjects (age 30 ± 10 y).
