The Popliteofibular Ligament: A Narrative Review of Anatomical Variants and Surgical Relevance.

The popliteofibular ligament (PFL) plays a vital role in knee joint stability, particularly within the posterolateral corner (PLC) of the knee. Located between the femoral condyle and the fibular head, the PFL resists excessive external rotation and lateral translation of the tibia, thus preventing knee instability during dynamic activities. This ligament, although integral in maintaining knee integrity, has often been overlooked in clinical practice and research. This review synthesizes the current literature on the anatomy, biomechanics, and clinical relevance of the PFL, highlighting its mo

Acute Distal Biceps Tendon Repair With Cortical Button Offers Good Functional Outcomes: A Retrospective Study Focusing on Range of Motion, Muscle Strength and Pain.

Background Distal biceps tendon (DBT) rupture is not one of the most common upper limb injuries. Surgical intervention is recommended for these injuries to restore muscular strength and functionality. Multiple different techniques are documented in the literature, however there is no definitive consensus on the most effective surgical treatment. The objective of this study was to assess the functional results of patients who underwent repair of DBT utilizing cortical button fixation procedures. Methods This study is a retrospective single-unit case series consisting of 54 patients who underwen

Morphological study on the origin of the semitendinosus muscle in the long head of biceps femoris.

Hamstring muscle injuries are the most prevalent among athletes who engage in sprinting activities. Their most frequent location is where the long head of the biceps femoris joins with the semitendinosus muscle to form the conjoint hamstring tendon. Just distal to this area, an additional group of fibers of the semitendinosus originate from medial aspect of biceps femoris. The objective of this study was to analyze the morphological characteristics of this union and to discuss its potential role in hamstring tears. Anatomical dissection was performed on 35 thighs. Samples obtained from this re

The Volar Distal Radioulnar Portal in Wrist Arthroscopy: An Anatomical Study.

Background  Advances in wrist arthroscopy and the emergence of novel surgical techniques have created a need for new portals to the wrist. The aim of this study was to define and verify the safety of the volar distal radioulnar (VDRU) portal. Description of the Technique  The VDRU portal is located ∼5 to 10 mm proximal to the proximal wrist crease, just on the ulnar edge of flexor carpi ulnaris tendon and radial to the dorsal cutaneous branch of the ulnar nerve. The ulnar styloid marks the distal point of the portal. Methods  An anatomical study was performed on 12 upper extremity specimens of

Assessment of pisotriquetral misalignment with magnetic resonance imaging: Is it associated with trauma?

OBJECTIVE: Our objective was to determine whether there is an association between pisotriquetral (PT) malalignment and acute distal radius fracture by using magnetic resonance imaging (MRI). METHODS: We evaluated 138 patients who underwent 3-T MRI of the wrists. Group A comprised 85 patients with acute distal radius fracture, and group B comprised 53 patients without trauma. PT interval and angle and pisiform excursion were measured on oblique axial and sagittal multiplanar reformats. The presence of abnormalities in the flexor carpi ulnaris tendon (FCU), pisometacarpal ligament (PML), and pi

Teeth syndrome: diagnosis, complications and management.

Teeth syndrome or fight bite is a specific entity in hand surgery that is little known. It includes infectious complications of the hand following a fist against the teeth. Neglected or misdiagnosed this injury frequently leads to serious complications that could compromise the function of the hand. A retrospective study was performed on 20 patients treated for teeth syndrome at our department, during a period of 12 years (January 2003 to April 2015). All young adults with a mean age of 28 years and a significant male predominance. The dominant side was involved in 15 patients. Lesions were di

Anatomic deltoid ligament repair with anchor-to-post suture reinforcement: technique tip.

The deltoid ligament is the primary ligamentous stabilizer of the ankle joint. Both superficial and deep components of the ligament can be disrupted with a rotational ankle fracture, chronic ankle instability, or in late stage adult acquired flatfoot deformity. The role of deltoid ligament repair in these conditions has been limited and its contribution to arthritis is largely unknown. Neglect of the deltoid ligament in the treatment of ankle injuries may be due to difficulties in diagnosis and lack of an effective method for repair. Most acute repair techniques address the superficial deltoid

[Treatment of Monteggia fracture in children].

Described in this paper is a therapeutic concept by which to cope with Monteggia's fracture in childhood. It is based on experience obtained from 72 cases with injuries of that kind and their evaluation in a group study. Therapeutic approach to the individual case was determined by the following criteria: age of infant, localisation and shape of ulnar fracture, reducibility of capitulum radii, and accompanying injuries requiring treatment on the same arm. The need for surgical stabilisation of ulnar fracture was found to increase along with growing age of the affected child. High-stability ana

Arthroscopic Latarjet Using a 3 Anterior Portal Technique.

Arthroscopic Latarjet is technically challenging with a steep learning curve. Several limitations include the risk of malposition of the coracoid bone block, the risk of screw fixation, such as hardware failure and irritation, and the risk of neurological injury. Initial descriptions of the technique also describe several anterior portals, especially for the subscapularis split. The learning curve is steep, and it is associated with increased risks of complications when compared to the open Latarjet procedure. This technique involves a simplified 3 anterior portal technique, with the portals l

Specific Overuse Injuries in Professional Athletes.

Overuse injuries are a major cause of pain, performance decline, and time loss in professional athletes. Unlike acute traumatic injuries, these conditions develop gradually because of repetitive submaximal loading and inadequate recovery. Recurrent mechanical stress may lead to microstructural damage involving tendons, bones, cartilage, ligaments, and surrounding soft tissues. Overhead athletes, runners, and jumping athletes are particularly susceptible to chronic overload syndromes, including rotator cuff tendinopathy, patellar tendinopathy, Achilles tendinopathy, and stress fractures. Restoration of Elbow Flexion with a Pedicled Latissimus Dorsi Myocutaneous Flap to a Brachial Plexus Injury at the Terminal Nerve Level.

A pedicled latissimus dorsi (LD) myocutaneous flap is a reliable reconstructive method for elbow flexion, though there are no reports regarding its application to a terminal nerve level injury of the brachial plexus. A 29-year-old man presented with dysfunction of elbow flexion, wrist extension, and finger extension. Physical examination and electromyography showed that the palsy was caused by an injury at the terminal nerve level of the brachial plexus without dysfunction of the axillary nerve. Bipolar transfer of LD for reconstruction of elbow flexion and subsequent tendon transfer for wrist

A preclinical rat model for bilateral phrenic nerve stimulation during mechanical ventilation.

Phrenic nerve stimulation (PNS) may preserve diaphragm activation and mitigate multiorgan injury during mechanical ventilation (MV); however, a minimal invasive rat model integrating PNS with MV is lacking. We established an omohyoid muscle-based PNS rat model combined with MV. Bilateral nerves were exposed within 20 ± 2 min by transection at the intermediate tendon of omohyoid muscle, minimizing trauma and bleeding. Threshold stimulation (0.6 ± 0.2 mA) correlated with body weight. Ventilator-synchronized stimulation increased compound muscle action potentials by ~30%, whereas histology confir

Reconstruction of an Amputated Thumb by Great Toe Harvested from a Simultaneously Amputated Lower Limb: Spare Part Surgery Concept Revisited.

Utilization of tissues from the amputated part to cover critical structures or enhance function in another area is a valuable technique in reconstructive surgery. For this "spare part surgery" to occur high level of awareness is to be present among surgeons who perform the primary procedure. We are presenting a case wherein the great toe was harvested for thumb reconstruction in a patient who had a nonsalvageable lower limb injury with a crush injury of the hand with amputation of multiple fingers including the thumb. The patient on follow-up is using his reconstructed thumb for all his activi

Role of Matrix Metalloproteinases in Musculoskeletal Diseases.

Musculoskeletal disorders include rheumatoid arthritis, osteoarthritis, sarcopenia, injury, stiffness, and bone loss. The prevalence of these conditions is frequent among elderly populations with significant mobility and mortality rates. This may lead to extreme discomfort and detrimental effect on the patient's health and socioeconomic situation. Muscles, ligaments, tendons, and soft tissue are vital for body function and movement. Matrix metalloproteinases (MMPs) are regulatory proteases involved in synthesizing, degrading, and remodeling extracellular matrix (ECM) components. By modulating

Surgical treatment of chronic mallet finger.

The literature to find the best approach to correct a chronic mallet finger deformity has been reviewed. All the evidence we found was type IV mallet finger injury, based on the CEBM classification. In the European literature, if correction of the proximal interphalangeal joint is not needed, and surgery is to be done on the distal interphalangeal joint only, then the most frequently reported technique involves the conversion of the chronic injury into an acute one by excising the scar and part of the joint capsule, and the extensor tendon is reattached with minor variations. An 80% to 100% su
