[Antimicrobial susceptibility of microorganisms isolated from urine of pediatric ward patients].

The study was an analysis of the frequency of urine bacterial isolation in hospitalized children as well as an evaluation of their susceptibility to antibiotics used in urinary tract infections (UTI). The analysis focused on microbiological urine tests carried out between January 2006 and December 2008. Altogether, 311 strains were obtained, of which E. coli (50.8%) and E. faecalis (13.5%) were the most frequently isolates. The highest percentage of Enterobacteriaceae were sensitive to ceftazidime (92%); to a lesser degree to amoxicillin/clavulanic acid (85%), to trimethoprim/sulfamethoxazole

Antibiotic prophylaxis in orthopedic prosthetic surgery.

Staphylococci account for approximately 70% of postoperative infections in orthopedic prosthetic surgery, with the leading organism being Staphylococcus epidermidis. Therefore, the antibiotics most widely used for prophylaxis are cefazolin, cefamandole and cefuroxime, by virtue of their excellent activity against these pathogens. However, methicillin-resistant coagulase-positive and -negative staphylococci are increasingly being reported as the causative agents of postoperative infection in clean prosthetic surgery, therefore prompting the use of glycopeptide antibiotics (vancomycin and teicop

Multidrug-resistant organism co-colonization in patients with discordant direct stool Xpert Carba-R and culture-based carbapenemase-producing Enterobacterales surveillance results.

Discrepancies between direct molecular surveillance and culture-based surveillance for carbapenemase-producing Enterobacterales (CPE) complicate infection control decision-making. This study investigated co-colonization patterns of multidrug-resistant organisms (MDROs) in patients with discordant results. From March to December 2023, two rectal swabs were simultaneously collected from high-risk patients at a tertiary hospital: one for direct Xpert Carba-R assay and one for culture-based molecular assay. Among 4120 screened patients, 142 (3.4%) tested positive by direct molecular assay, of whom

Polymicrobial Multidrug-Resistant Infection and Fatal Bowel Ischemic Perforation After Urgent Heart Transplantation in a VA-ECMO-Bridged Recipient: A Case Report.

Post-transplant infection caused by multidrug-resistant organisms (MDROs) is a major challenge in heart transplantation, especially in recipients requiring veno-arterial extracorporeal membrane oxygenation (VA-ECMO) before surgery. We describe a 52-year-old man with non-ST-elevation myocardial infarction and refractory cardiogenic shock who required VA-ECMO, intra-aortic balloon pump support, continuous renal replacement therapy, and mechanical ventilation before urgent heart transplantation. Before transplantation, he had active pneumonia. Donor respiratory culture grew Acinetobacter baumanni

Restoration of daptomycin sensitivity with adjunctive cefazolin is associated with C-terminal MprF mutations in MRSA bacteremia isolates.

Daptomycin (DAP) or vancomycin (VAN) resistance can result in methicillin-resistant Staphylococcus aureus (MRSA) bacteremia treatment failure. Clinical trials have not yielded a clear approach to MRSA combination therapy. Furthermore, there is not a protocol approved by the Clinical Laboratory Standards Institute (CLSI) for testing most possible antibiotic combinations, and methodology used in basic research laboratories for combination testing presents significant hurdles for implementation in clinical laboratories. In response to these challenges, we developed a high-throughput antibiotic co

Simultaneous inactivation of antibiotic-resistant bacteria and degradation of antibiotic-resistant genes in alkalised human urine.

The coexistence of pharmaceuticals and microorganisms in source separated urine poses a risk for the development of antimicrobial resistance (AMR), especially when urine-based fertilizers are applied to soils. While prior studies have investigated pathogen inactivation in source-separated wastewater matrices, few have evaluated the simultaneous fate of antibiotic-resistant bacteria (ARBs) and their corresponding resistance genes (ARGs) in real urine matrices, particularly under alkaline conditions. Here, we studied the inactivation of β-lactamase-producing Escherichia coli and vancomycin-resis

Silver Nanoparticles Synthesis From Bacillus subtilis and Its ROS-Mediated Staphylocidal Activity Against Methicillin-Resistant Staphylococcus aureus.

Rapidly emerging antimicrobial resistance (AMR) in Staphylococcus aureus is a global health issue that causes life-threatening infections in nosocomial and community-acquired settings. The prevalence of methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant S. aureus (VRSA) infections is higher in clinical practices, which causes a major hurdle in the treatment. The epidemiology of such systemic and invasive infections results in higher morbidity and mortality, especially in middle-income countries where hospitalization rates and improper drug use escalate the threat. Caught Between Stewardship and Resistance: How to Treat Acute Complicated Diverticulitis in Areas of Low Antimicrobial Susceptibility?

Antimicrobial resistance is one of the main threats to public health, with multidrug-resistant (MDR) pathogens on the rise across continents. Although treatment guidelines generally recommend antimicrobial therapy for acute complicated diverticulitis, they do not specify treatment pathways according to local or national resistance profiles. There is sparse data regarding specific pathogens involved in Hinchey II-IV patients who undergo surgery. This study seeks to address these issues and determine how often and what types of MDR bacteria occur in patients undergoing emergency surgery. We pros

Antibacterial activity of epsilon-poly-l-lysine produced by Stenotrophomonas maltophilia HS4 and Paenibacillus polymyxa HS5, alone and in combination with bacteriophages.

Over the past decades, antibiotic resistance has become a major clinical problem, and searching for new therapeutic strategies seems to be necessary. Using novel natural compounds, antimicrobial peptides, and bacteriophages is the most promising solution. In this study, various cationic metabolite-producer bacteria were isolated from different soil samples. Two isolates were identified as Stenotrophomonas maltophilia HS4 (accession number: MW791428) and Paenibacillus polymyxa HS5 (accession number: MW791430) based on biochemical characteristics and phylogenetic analysis using 16S rRNA gene seq

Understanding transmission pathways and integrated digital surveillance potential of antimicrobial resistance in Ethiopia in a One Health approach: a mixed-method study protocol.

INTRODUCTION: Antimicrobial resistance (AMR) has a critical global impact, mostly affecting low- and middle-income countries. A major knowledge gap exists in understanding the transmission pathway of the gut colonisation with AMR bacteria between healthy humans and their animals in addition to the presence of those AMR bacteria in the surrounding environment. A One Health (OH) approach is necessary to address this multisectoral problem. METHODS AND ANALYSIS: This cross-sectional, mixed-method OH study design will use both quantitative and qualitative methods of data collection. Quantitative m

β-Lactamase Suppression as a Strategy to Target Methicillin-Resistant Staphylococcus aureus: Proof of Concept.

β-Lactamase (penicillinase) renders early, natural β-lactams like penicillin G useless against methicillin-resistant Staphylococcus aureus (MRSA), which also expresses PBP2a, responsible for resistance to semisynthetic, penicillinase-insensitive β-lactams like oxacillin. Antimicrobial discovery is difficult, and resistance exists against most treatment options. Enhancing β-lactams against MRSA would revive its clinical utility. Most research on antimicrobial enhancement against MRSA focuses on oxacillin due to β-lactamase expression. Yet, Moreillon and others have demonstrated that penicillin

Pattern of acquisition of hospital-associated pathogens in the ICU of an academic tertiary care hospital.

BACKGROUND: Among hospitalized patients, a 48-hour window from time of hospitalization defines nosocomial infections and guides empiric antibiotic selection. This time frame may lead to overuse of broad-spectrum antibiotics. Our primary objective was to determine the earliest and median time since hospital admission to acquire antibiotic-resistant pathogens among patients admitted to the intensive care unit (ICU) of an academic, tertiary care hospital. METHODS: Retrospective chart review was conducted for adult patients admitted to the ICU from home or another hospital within the same health
