Günther Tulip filter: preliminary clinical experience with retrieval.

PURPOSE: The Günther Tulip filter is a permanent filter that has a hook to permit retrieval. The authors report their preliminary clinical evaluation of the filter with regard to feasibility and safety of retrieval. MATERIALS AND METHODS: Nine men and six women who ranged in age from 17 to 79 years (mean, 51 years) underwent treatment with use of the Günther Tulip filter. Patients judged to require caval interruption for < 14 days were selected to receive the filter and retrieval was planned for all patients. Indications for filter placement were: recent pulmonary embolism (PE) or proximal de

Acute clot-trapping efficiency in dogs with compacted versus elongated wires in bird's nest filters.

PURPOSE: The clot-trapping efficiency of the Bird's Nest filter was studied in dogs with the stainless steel wires in a compacted versus fully elongated configuration. MATERIALS AND METHODS: Bird's Nest filters were placed infrarenally with the wires compacted in 10 dogs and with the wires elongated in an additional 10 dogs. The dogs were separated into two groups, five dogs with each wire configuration in each group. In group 1, 10 unlabeled 6 x 30-mm blood clots were injected into the inferior vena cava. In group 2, six 6 x 10-mm, four 6 x 15-mm, and four 6 x 30-mm labeled clots were introd

Pulmonary valvotomy under normothermic caval inflow occlusion.

Pulmonary valve stenosis may require urgent surgical relief in infancy or elective valvotomy in childhood. A retrospective study has been made of 94 children who underwent pulmonary valvotomy for pulmonary valve stenosis between 1972 and 1983 using the technique of normothermic caval inflow occlusion. There were no early deaths nor late deaths. The group included 13 neonates less than 1 week old and 14 other infants under 1 year of age. The mean follow-up is 45 months. No child has had a second valvotomy for recurrent valvar stenosis. Two children have required re-operation for placement of a

[Superior and inferior cavography as well as azygography and retroperitoneal venography: methods, indications and results (author's transl)].

The diagnostic spectrum for venography of the trunk is broad. During the traditional superior and inferior cavography, which should be used for clinical bone marrow functional blockage in connection with a confirmed tumor or for thrombosis, the combined retroperitoneal venography and cavography which we developed for clinical use as well as direct azygography provide valuable supplementary information which can be significant especially for preoperative questions. In the case of carcinoma of the esophagus or stomach, the question whether or not the tumor can be surgically removed or whether th

Open and hybrid deep vein reconstructions: to do or not to do?

Endovascular reconstruction has become the standard treatment of chronic obstruction of large veins. Stenting is done with increasing frequency to treat iliac venous obstructions, with or without associated inferior vena caval or femoral vein occlusions. Open reconstruction with venous bypass is performed today in patients who fail attempts at venous stenting or who are not candidates for endovascular reconstructions. Patients with primary or secondary malignancies invading the vena cava undergo open caval reconstruction at the time of tumour excision. Open venous reconstructions are still pre

Cardiorespiratory response to cyanide of arterial chemoreceptors in fetal lambs.

Cardiorespiratory response to the stimulation of the carotid and aortic receptors by sodium cyanide was examined in fetal lambs in utero at 0.8 (120 days) gestation. Injections of 50-400 micrograms cyanide into the inferior vena cava or the carotid artery of intact fetuses elicited bradycardia and respiratory responses that varied from a single gasp to rhythmic respiratory movements but no significant change in arterial blood pressure. Carotid sinus denervation eliminated the cardiorespiratory response to intracarotid injection of cyanide and sinoaortic denervation abolished the response to in

Transparaumbilical Intravariceal Sclerotherapy for Duodenal Varices Using Outflow Embolization.

A 55-year-old patient was admitted for variceal treatment, a complication of chronic portal hypertension and liver cirrhosis. Imaging studies revealed prominent duodenal varices, the pancreaticoduodenal vein as its afferent pathway, a drainer vessel into the inferior vena cava, and a paraumbilical vein. We successfully performed complete obliteration of the varix, including its afferent and efferent vessels, via the paraumbilical vein approach.

Combined minimally invasive repair of the adult form of the scimitar syndrome and organic mitral regurgitation.

Scimitar syndrome is a rare congenital cardiac malformation defined as an anomalous pulmonary venous return from the right lung to the inferior vena cava. In adult patients, the surgical strategy chosen is very important because of concomitant diseases and conditions. Herein, we present a case in which a minimally invasive approach was successful for treating a combination of the adult form of the scimitar syndrome and severe degenerative mitral regurgitation.

The role of cardiac MRI in the diagnosis and management of sinus venosus atrial septal defect.

Sinus venosus atrial septal defects (SV-ASDs) are inter-atrial communications caused by a deficiency of the common wall between the superior or inferior vena cava and the right-sided pulmonary veins. The diagnosis can be challenging, especially in adults with delayed presentation. We present images that illustrate an example of the role of cardiac magnetic resonance imaging (CMRI) in the diagnosis and follow-up of a patient with SV-ASD.

Atrial septal defect with normal pulmonary arterial pressure in adult cyanotic patient.

A 22-year-old male presented with recurrent stroke, central cyanosis, and dyspnea. Transesophageal echocardiography and cardiac catheterization revealed bidirectional shunt flow through atrial septal defect (ASD) without pulmonary arterial hypertension. The orifice of inferior vena cava facing towards ASD opening led partially right to left shunt resulting in cyanosis with normal pulmonary arterial pressure.

Atrioventricular nodal reentrant tachycardia ablation in the setting of bilateral femoral vein occlusion.

Atrioventricular nodal reentrant tachycardia (AVNRT) is the most common supraventricular tachycardia. Ablation of the slow pathway is the treatment of choice for recurrent episodes despite adequate medical therapy. We present an uncommon case of AVNRT ablation using a superior vena cava approach in a patient with bilateral common femoral vein occlusion. This approach appears to be a safe alternative in patients with an absence of an inferior venous access.

[Lymphocytic gastritis with anasarca and venous thrombosis].

INTRODUCTION: Venous thrombosis has not previously been reported as a complication of lymphocytic gastritis. CASE: A 47-year-old patient was admitted for anasarca related to varioliform gastritis. The densitometric examination showed two partial thromboses, at the confluence of the splenic and mesenteric veins and in the postrenal vena cava. COMMENTS: In patients whose serum albumin levels are very low, the risk of thrombosis of exudative enteropathies should considered.

Late "adult form" scimitar syndrome presenting with "infant form" complications.

This case report describes an 85-year-old woman with complications resulting from scimitar syndrome, anomalous drainage of the right pulmonary veins into the inferior vena cava. In addition to being the oldest reported case of this congenital anomaly, this is the first report of pulmonary vascular complications from scimitar manifested in the adult. The magnitude of the vascular shunt and advanced nature of the disease precluded surgical or percutaneous intervention in this patient.

Osteosarcoma invading adrenal gland: rare cause of adrenal mass.

Adrenal gland metastasis from osteogenic sarcoma is extremely rare and is an atypical location for metastasis. Vascular hepatic exclusion techniques and the use of venovenous bypass with cardiac surgical techniques may be required to resect large adrenal masses that have invaded the inferior vena cava to achieve curative resection. The use of newer chemotherapeutic agents and aggressive surgical resection have prolonged the survival of patients with osteosarcoma.

Hepato-atrial anastomosis, the "other Senning operation" for treatment of Budd-Chiari syndrome.

A now 36-year-old woman developed a suprahepatic inferior vena cava stenosis, 9 years after liver transplantation for extensive liver echinococcosis. The lesion was treated by percutaneous angioplasty and stenting. Five years later, recurrence of echinococosis with intrastent stenosis together with clinical symptoms, prompted surgical treatment. Hepato-atrial anastomosis was performed under cardiopulmonary bypass with good result.

Congenital extrahepatic portocaval shunt associated with hepatic hyperplastic nodules in a patient with Dubin-Johnson syndrome.

We report a rare case of congenital extrahepatic portocaval shunt diagnosed during evaluation of hyperplastic nodules in the liver. Diagnostic imagings showed hypoplasia of the intrahepatic portal venous system and splanchnic portal venous return to the inferior vena cava through aberrant vessels. Altered hepatic blood flow dynamics due to this shunt may have been implicated in the etiology of the hepatic hyperplastic nodules.

Issues and controversies in venous thromboembolism.

This paper gives specific recommendations on a number of issues in venous thromboembolism: how to evaluate idiopathic deep vein thrombosis (DVT); how to treat calf vein thrombosis and upper-extremity DVT; how to use low-molecular-weight heparin, vena cava filters, catheter-directed thrombolytic therapy, and compression stockings; how long to continue anticoagulation therapy; and how to manage recurrent DVT.

[Determining the type of drowning based on the levels of blood in the thoracic duct lymph].

The degree of lymphogemy in persons who died of different causes (including drowning) was studied by histologic analysis of the thoracic lymphatic ducts. It was found that quantitative estimation of the presented feature makes it possible to estimate the specific type of drowning which depends on the level of venous hypertension in the system of superior vena cava. This phenomena is strongly marked in spastic and mixed types of drowning and is absent in aspiration and reflex types.
