Clinical significance of the reversed mitral annular motion velocity wave at the beginning of the mitral valve closure.

BACKGROUND: Pulsed tissue Doppler imaging is increasingly used to record mitral annular motion (MAM) velocity pattern. A reversed MAM velocity wave (Cm) is commonly seen at the beginning of the mitral valve closure in timing, whereas the underlying mechanism and clinical significance have not been studied. METHODS: Conventional, pulsed Doppler, pulsed tissue Doppler, and two-dimensional strain echocardiography were performed in 100 consecutive patients with cardiovascular risk factors. RESULTS: There were no correlations between the peak Cm and the ratio of peak early diastolic transmitral f

Takotsubo cardiomyopathy presenting as postoperative atrial fibrillation.

Takotsubo cardiomyopathy (TC) is a condition which was first acknowledged in Japan and is characterized by a reversible systolic dysfunction of the apical or mid segments of the left ventricle. Typically affecting women in the post-menopausal population, it is triggered by intense emotional, physical or medical stress. Also known as apical ballooning syndrome or stress cardiomyopathy, TC derives its name from the left ventricular angiographic appearance of a 'Takotsubo', literally translated as an 'octopus fishing trap' in Japanese. Patients often describe chest pain, have ischemic electrocard

Optimization techniques in cardiac resynchronization therapy.

Cardiac resynchronization therapy improves symptoms and cardiac function, as well as reduces mortality in patients with progressive congestive heart failure, reduced left ventricular ejection fraction and a left bundle branch block on the surface electrocardiogram. As many as 30% of patients fail to have an adequate response. The interplay between the atrioventricular delay and the contribution of a properly timed atrial contraction to ventricular filling along with a properly timed sequence of activation of the right and left ventricular is crucial to maximizing the benefits of cardiac resync

A modified echocardiographic protocol with intrinsic plausibility control to determine intraventricular asynchrony based on TDI and TSI.

BACKGROUND: Established methods to determine asynchrony suffer from high intra- and interobserver variability and failed to improve patient selection for cardiac resynchronization therapy (CRT). Thus, there is a need for easy and robust approaches to reliably assess cardiac asynchrony. METHODS AND RESULTS: We performed echocardiography in 100 healthy subjects and 33 patients with left bundle branch block (LBBB). To detect intraventricular asynchrony, we combined two established methods, i.e., tissue synchronization imaging (TSI) and tissue Doppler imaging (TDI). The time intervals from the on

Cardiac sympathetic activity is associated with inflammation and neurohumoral activation in patients with idiopathic dilated cardiomyopathy.

BACKGROUND: Idiopathic dilated cardiomyopathy (IDC) is characterized by sympathetic nervous overactivity, inflammation and neurohumoral activation; however, their interrelationships are poorly understood. METHODS AND RESULTS: We studied 99 patients with IDC (age 54 +/- 1 years, left ventricular ejection fraction (EF) 40 +/- 1%, maximum oxygen uptake (VO(2)max) 20 +/- 1 ml kg(-1) min(-2), mean +/- SEM) by using (123)I-metaiodobenzylguanidine (MIBG) imaging. MIBG washout and MIBG heart/mediastinum (H/M)-ratio at 4 h postinjection were calculated. In addition, the plasma levels of interleukin (I

Left ventricular cardiac function in fetuses with congenital diaphragmatic hernia and the effect of fetal endoscopic tracheal occlusion.

OBJECTIVES: The pre-existing compression of the left ventricle in congenital diaphragmatic hernia (CDH) could be aggravated by the amplified lung growth after fetoscopic endoluminal tracheal occlusion (FETO). Our aim was to document left ventricular (LV) size and function in fetuses with isolated left-sided CDH and to document the effect of FETO on the fetal heart. METHODS: We determined cardiac axis, LV diameters, ejection fraction, shortening fraction, mitral E/A index and myocardial performance index (MPI) in 27 fetuses with isolated left-sided CDH, and compared these with values in a refe

Left ventricular noncompaction in children.

OBJECTIVE: Left ventricular noncompaction (LVNC) is an uncommon type of cardiomyopathy in children. We sought to determine the clinical presentations and outcomes of children diagnosed to have LVNC. DESIGN: The case records of children diagnosed to have LVNC between 1999 and 2007 were reviewed. The diagnosis was based on echocardiographic finding of a thick noncompacted myocardium layer characterized by a trabecular meshwork with deep endomyocardial spaces. RESULTS: Ten patients (seven males) were diagnosed to have LVNC at a median age of 2 years (range, 7 days to 12 years). Seven patients h

Valvular heart disease: a perspective on the asymptomatic patient with severe valvular aortic stenosis.

Early in 2006, the European Society of Cardiology (ESC) assigned the 2006 Denolin lecturer to provide a broad perspective on the asymptomatic patient with severe calcific valvular aortic stenosis (AS) in the older patient. Subsequently, in late 2006, American College of Cardiology/American Heart Association Guidelines and in 2007 ESC Guidelines have given the Class I recommendation to 'primary' aortic valve replacement (AVR) for asymptomatic patients with AS only to those with left ventricular dysfunction at rest and to those who only become symptomatic on exercise testing. Indications for AVR

Evaluation of left ventricular long-axis function in cases of rheumatic pure mitral stenosis with atrial fibrillation.

Using tissue Doppler echocardiography, we evaluated left ventricular long-axis function in 65 mitral stenosis patients, 30 of whom were in sinus rhythm and 35 of whom were in chronic atrial fibrillation. There were 35 healthy control subjects. Conventional echocardiography was used to evaluate left ventricular diameters, left atrial diameters, left ventricular ejection fractions, and mitral valve areas. Tissue Doppler echocardiography was used to evaluate isovolumetric contraction time (ICT), isovolumetric relaxation time (IRT), ejection time (ET), and peak systolic myocardial velocities. The

Orthotopic cardiac transplantation in a patient with pectus excavatum: approach via left anterior thoracotomy.

Severe pectus excavatum, although a rare condition, is likely to be encountered by large transplant centers. The main concerns for transplant centers are the associated cardiopulmonary abnormalities along with the technical details of the transplant. We describe our experience managing a patient with severe pectus excavatum with associated cardiorespiratory abnormalities. This case report demonstrates that orthotopic cardiac transplantation through a left anterior thoracotomy is a feasible method in patients with significant displacement of the heart into the left chest.

Left ventricular noncompaction with a single coronary artery of anomalous origin.

Noncompaction of ventricular myocardium (NVM) is a morphogenetic anomaly of ventricular myocardium that leads to the development of cardiomyopathy. It is frequently associated with other congenital cardiac malformations. A 75-year-old woman was admitted with resting dyspnea lasting for several days. Two-dimensional echocardiography demonstrated an enlarged left ventricle with global impairment of systolic function. The myocardial trabeculations, with multiple recesses, were observed at the apex and mid-ventricular segment of the left ventricle. Contrast echocardiography showed filling of the r

Left bundle branch block in chronic heart failure-impact on diastolic function, filling pressures, and B-type natriuretic peptide levels.

BACKGROUND: For patients with chronic heart failure (CHF), left bundle branch block (LBBB) is associated with impaired systolic function and increased morbidity and mortality, but data on diastolic function are scarce. In this patient population, we attempted to define the impact of LBBB on diastolic function, filling pressures, and brain natriuretic peptide and its circulating N-terminal precursor (NT-proBNP) levels. METHODS: A total of 94 patients with stable CHF (48 with complete LBBB, 46 without intraventricular conduction delay and normal QRS duration) underwent conventional 2-dimensiona

Reduced high-frequency QRS components in electrocardiogram leads facing an area of the heart with intraventricular conduction delay due to bundle branch block.

BACKGROUND: The mechanisms underlying high-frequency QRS components (HF-QRS) are incompletely understood. One theory is that HF-QRS are related to the conduction velocity of the heart. The purpose was to test this hypothesis by comparing HF-QRS in patients with left or right bundle branch block (LBBB and RBBB, respectively) to those in healthy subjects and in patients with ischemic heart disease (IHD). METHODS: Twenty-two patients with LBBB, 19 patients with RBBB, 63 normal subjects, and 64 patients with IHD were included. Twelve-lead electrocardiograms were analyzed in the frequency interval

Prognostic value of preoperative atrial fibrillation in patients with aortic stenosis and low ejection fraction having aortic valve replacement.

Aortic valve replacement in severe aortic stenosis (AS) with a low left ventricular ejection fraction (EF) is associated with high perioperative mortality. The aim of this study was to assess the prognostic value of preoperative atrial fibrillation (AF) in patients with AS and low EFs who undergo aortic valve replacement. Eighty-three consecutive patients with severe AS (area <1 cm2) and low EFs (< or =35%) were prospectively included. Perioperative mortality was 12%. Twenty-nine patients (35%) had preexisting paroxysmal or permanent AF. Perioperative mortality was higher in the AF group than

[Score for nutritional status evaluation: the role played in the prognostic stratification of dilated cardiomyopathy and advanced heart failure patients].

OBJECTIVE: Develop a method for the evaluation of patients nutritional status through a score that expresses universal nutritional status, as well as investigate if that score would be efficient for the prognostic stratification of advanced heart failure (HF) pts. METHODS: The score was reached by the selection of evaluation methods that would quantify nutritional status: ideal body weight percentage, thickness of tricipital skinfold, percentiles for arm muscular mass circumference, albumin serum level, lymphocyte total count. In order to be validated, the score was applied to a group of 95 p

Negative inotropic and chronotropic effects of delta-opioid receptor antagonists are mediated via non-opioid receptors.

Ten-minute perfusion of intact isolated rat heart with Krebs-Henseleit solution containing delta-opioid receptor agonists (DPDPE, (-)-TAN-67) or delta-opioid receptor antagonists (naltrindole, TIPP[psi], ICI 174,864) at a final concentration of 0.1 mg/liter decreased HR, blood pressure in the left ventricle, and the rates of myocardial contraction and relaxation. Intravenous injection of delta-agonists (DPDPE, (-)-TAN-67, deltorphin II) or delta-antagonists (naltrindole, TIPP[psi], ICI 174,864) decreased HR in narcotized rats. Naloxone and naltrexone produced no effect on contractility and HR

Hospital protocols and evidence-based therapies: the importance of integrating aldosterone blockade into the management of patients with post-acute myocardial infarction heart failure.

Left ventricular systolic dysfunction (LVSD) and clinical heart failure are common complications of acute myocardial infarction (AMI) and result in substantially increased mortality and morbidity. Evidence-based cardiovascular protective therapies, including angiotensin-converting enzyme inhibitors, beta blockers, antiplatelet agents, and lipid-lowering medications, improve outcomes for these patients. However, this population is significantly undertreated with these guideline-recommended agents. Critical pathways have been demonstrated to improve the quality and consistency of treatment; as s

[Chronic left heart failure. A focus on the pathogenetic basis of medical treatment].

Chronic left heart failure is the result of the activation of pathophysiological mechanisms which over time lead to progressive deterioration of the cardiac function. As pharmacotherapy aims at these mechanisms and as treatment possibilities are increasing, we find it relevant to provide a survey. Treatment evidence is based mainly on systolic dysfunction. It consists of angiotensin-converting enzyme inhibitor, aldosterone antagonist, beta-blocker, digoxin and diuretics. Incorporation of evidence-based medicine and treatment of diastolic dysfunction should be focussed on in the future.

Increase in creatinine and cardiovascular risk in patients with systolic dysfunction after myocardial infarction.

Baseline renal function is a potent independent risk factor for adverse events after acute myocardial infarction (MI). Worsening renal function (WRF) has been shown to influence outcomes in the heart failure population, but its impact on cardiovascular risk in the post-MI period has not been well defined. For assessment of the prognostic importance of WRF, 2231 patients who had left ventricular dysfunction and were enrolled in the Survival and Ventricular Enlargement (SAVE) trial were studied. Patients were randomly assigned between 3 and 16 d (average 11 d) after acute MI to receive captopril
