The need for revised standards for skinfold thickness in infancy.

Skinfold standards provide a useful indication of subcutaneous fat. To evaluate skinfold thickness of 252 Cambridge infants over the first 2 years of age, SD scores relative to the Tanner standards were calculated, taking account of skewness in the standards. Cambridge SD scores were low, varying according to age from -1.2 to -1.8 for triceps and -0.6 to -1.2 for subscapular skinfolds. The Tanner skinfold standards were last revised 30 years ago, at a time of high prevalence of infantile obesity, and the present and other studies indicate that infants are now thinner. There is a need for new s

Prediction of individual response to postnatal dexamethasone in ventilator dependent preterm infants.

AIMS: To evaluate factors predictive of individual response to dexamethasone in preterm infants. METHODS: A cohort of 74 preterm infants born between January 1993 and February 1996 was studied retrospectively. All of them had received dexamethasone to facilitate weaning from artificial ventilation. Demographic factors, ventilation parameters, and details of dexamethasone administration were recorded from the medical and nursing notes. Radiographs were assessed by one observer who was unaware of the clinical condition of the infant or the outcome. Outcome variables examined included change in …

A one-way valved atrial septal patch: a new surgical technique and its clinical application.

Patients who undergo surgical repair of congenital heart defects, characterized by a hypoplastic right ventricle or high pulmonary vascular resistance, are at high risk for the development of postoperative right heart failure. This risk may discourage the surgical team from carrying out a biventricular or complete repair in such patients. To reduce the risk for right heart failure, we developed a one-way, valved, atrial septal patch to serve as an artificial one-way foramen ovale and tested it in an animal model. By permitting right-to-left shunt, this device decompresses the failing right ven

Tissue protein turnover is altered during catch-up growth following Escherichia coli infection in weanling rats.

Infection in young growing animals is manifested by poor tissue protein accretion; during subsequent catch-up growth this is reversed. To account for these changes, protein synthesis and degradation were measured in vivo in skeletal muscle, skin, liver and small intestine in weanling rats during catch-up growth after Escherichia coli infection. Observations were made at d 4, 6, 8, 11 and 14, when infected rats had elevated nitrogen balance. Liver protein mass and turnover were not affected by treatment. Although protein mass of small intestine fell during infection, catch-up was achieved befor

Abnormal flexion of the corono-pedal joint or "contracted tendons" in unweaned foals.

A condition frequently misnamed "contracted tendons" is described in unweaned foals. Various theories regarding its aetiology are examined. Overfeeding and lack of exercise are suggested as being the most likely causes, leading to excessive growth of the long bones. An effective method of treatment is described. "Contracted tendons" in yearlings are also discussed.

Operating Room-Complete Venovenous Extracorporeal Membrane Oxygenation for Thoracoscopic Surgery Without Repositioning.

Patients who cannot sustain gas exchange during one-lung ventilation may still require a brief thoracoscopic procedure. Venovenous extracorporeal membrane oxygenation can support them, but conventional practice involves cannulating the supine patient and then turning them laterally while they are circuit-dependent. We describe an operating-room-complete strategy in which the semi-supine position is the single operative position: femoral access and video-assisted thoracoscopic surgery proceed without repositioning, with exposure adjusted by rotating the table. The vein is cannulated with a purs

Left Main Spontaneous Coronary Artery Dissection Complicated by Cardiogenic Shock.

OBJECTIVE: To review a case of left main spontaneous coronary artery dissection (SCAD) complicated by pulseless ventricular tachycardia arrest and cardiogenic shock in a 58-year-old woman without pre-existing cardiovascular risk factors, presenting as non-ST-segment elevation myocardial infarction. KEY STEPS: Catheterization with pulmonary artery catheter placement was crucial in revealing left main SCAD with severe cardiogenic shock (Society for Cardiovascular Angiography & Interventions stage D). The patient required initiation of mechanical support with Impella-supported percutaneous coron

Management of neonatal atlantoaxial rotatory subluxation (AARS) using a rigid spinal immobiliser.

We present a very rare case of neonatal atlantoaxial rotatory subluxation (AARS) and its management.A term female neonate born via vacuum-assisted delivery presented with distress immediately after birth. On examination, she had right-sided torticollis and an audible cervical spine 'click'. Imaging confirmed AARS. Spinal precautions were initiated, and she was transferred to a surgical tertiary children's hospital. A rigid cervical collar was applied initially with limited success for immobilisation. Hence, after a multidisciplinary team meeting, it was changed to a rigid spinal immobiliser. M

Rapid Treatment With Veno-Venous Extracorporeal Membrane Oxygenation in a Patient Who Developed Acute Respiratory Distress Syndrome After Drowning.

This is a case of a 76-year-old male with a past medical history of hypertension, chronic kidney disease (CKD), transient ischemic attack (TIA) status post tissue plasminogen activator (tPA), bilateral carotid stenosis, and seizure disorder who was brought into a South Florida emergency department by emergency medical services after a drowning incident secondary to a seizure. It was found that he had low oxygen saturation, a low Glasgow coma score (GCS), and was intubated and mechanically ventilated. CT of the chest and arterial blood gas (ABG) determined that he had acute respiratory distress

Kawasaki disease shock syndrome with pulmonary involvement in a child: a case report.

Kawasaki disease (KD) is an acute systemic vasculitis that can affect multiple organs. Kawasaki disease shock syndrome (KDSS) is a severe complication of KD. Pulmonary involvement is uncommon in KD, and there are very few reports of patients with KD who develop KDSS and pulmonary lesions. This report presents the case of a 9-year-old boy who was hospitalized for fever, neck mass and rash, and was subsequently diagnosed with KDSS. During the administration of intravenous immunoglobulin (IVIG), he experienced acute dyspnea, which required urgent invasive mechanical ventilation and bronchoscopy.

The effects of PRRS on the environmental impact of pig production: a life cycle assessment study.

The Porcine Reproductive and Respiratory Syndrome (PRRS) is a viral pig disease that increases mortality rates and reduces (re)production performance. This worsens a farm's whole-enterprise feed conversion ratio, which increases its greenhouse gas emission. PRRSV elimination should then result in mitigation of global warming. This study quantifies such mitigation of global warming and of other environmental impact categories such as eutrophication and land use by Life Cycle Assessment. The software package openLCA converted material and energy flows to impact categories of frameworks ReCiPe-20

Innovative use of EIT-guided prone positioning and inhaled nitric oxide therapy for refractory hypoxemia in primary graft dysfunction: a case report.

Primary graft dysfunction Grade 3 (PGD 3) following lung transplantation significantly increases the incidence of acute and chronic complications. These effects complicate clinical perioperative management and significantly increase mortality. Here, we report a case of PGD 3 and refractory hypoxemia after bilateral lung transplantation at our center. Despite ongoing extracorporeal membrane oxygenation (ECMO) support, the patient's partial pressure of oxygen (PaO₂) remained suboptimal at 71.7 mmHg on postoperative day 4, precluding safe discontinuation of ECMO support. Consequently, EIT-guided

Improving Welfare in Suckling Goat Kids During Winter: Impact of Heat-Retentive Kid Nest on Hiding, Playing, Suckling, and Nursing Behaviors.

The welfare of winter-born suckling goat kids is important, as their thermoregulatory systems are still developing. The pilot study investigated the impact of heat-retentive kid nest (HRKN) on hiding, playing and suckling behaviors of kids born during winter. Twelve newborn kids were divided into two groups along with their respective dams. The HRKN, designed to provide warmer microclimatic conditions, served as ancillary housing for the HRKN group, while the control group was housed intensively without HRKN. Given the lack of independent replicates in each treatment group (n = 1 per treatment

ECPELLA as a Bridge to Unexpected Myocardial Recovery in a Patient in Refractory Cardiogenic Shock.

Cardiogenic shock (CS) is a complex, multifactorial syndrome with substantial morbidity and mortality. Despite neutral results from randomized controlled trials, venoarterial extracorporeal membrane oxygenation (VA-ECMO) may be reasonable for select patients with refractory CS. A 37-year-old woman with morbid obesity and dilated nonischemic cardiomyopathy presented with Society of Cardiovascular Angiography and Intervention stage D biventricular CS, requiring vasopressor support and emergent escalation to ECPELLA, the combination of VA-ECMO and Impella (Abiomed), as a bridge to decision. She w

Multidisciplinary Management of Severe Maternal H1N1 Infection With Acute Respiratory Distress Syndrome and Ventilator-Associated Pneumonia Requiring Extracorporeal Membrane Oxygenation: A Case Report.

Pregnant women are at increased risk of severe H1N1 infection, which can lead to complications such as acute respiratory distress syndrome (ARDS) and secondary infections. We report a 29-year-old woman at 29 weeks of gestation who developed fever, cough, and rapidly worsening respiratory distress. Initial management included oxygen therapy, Bilevel Positive Airway Pressure (BiPAP), antivirals, antibiotics, and antenatal corticosteroids; however, worsening hypoxemia necessitated an emergency cesarean section, resulting in the delivery of a viable preterm infant. Postoperatively, she required me
