Cardiac arrhythmia and betel nut chewing--is there a causal effect?

Betel nuts are widely ingested in many countries, including Taiwan. They stimulate various autonomic and CNS reactions, but there have been no reports of cardiac toxicity. We treated 2 patients with cardiac dysrhythmias thought due to betel nut chewing. In case 1, a 44-y-old previously healthy male presented with chest tightness, dyspnea, diaphoresis and palpitation immediately after chewing 1 betel quid. He soon became breathless and died despite immediate cardiopulmonary resuscitation. In Case 2, a 28-y-old man suffered palpitations, epigastralgia, and chest distress following consumption of

Effect of alcohol on gastrointestinal cell regeneration as a possible mechanism in alcohol-associated carcinogenesis.

Chronic ethanol consumption is a major risk factor for oropharyngeal, esophageal, and rectal cancer. Because hyperregenerative gastrointestinal mucosa has an increased susceptibility towards chemical carcinogens and thus influences carcinogenesis, various studies have been performed to evaluate the effect of chronic ethanol consumption on mucosal cell turnover. In the rat, morphometric analysis showed that in chronically ethanol-fed rats the size of the basal cell nuclei of the oral mucosa from the floor of the mouth, the edge of the tongue, and the base of the tongue were significantly enlarg

[Intraoral cancer: 100 consecutive cases].

One hundred patients (43 women and 57 men) with intraoral squamous cell carcinoma were questioned about their tobacco and drinking habits and then subjected to a thorough intraoral clinical examination. Twelve of the women did not use tobacco and did not have increased alcohol consumption. Only three of the men did not use tobacco, and 70% had daily consumption of alcohol. Where location of the carcinoma was concerned, the men had two and a half times as many carcinomas of the floor of mouth and inferior surface of tongue as the women. An astonishing finding was that 15% of the patients did no

Motorcycle accidents and alcohol. A survey of fatalities in the Cape Peninsula.

Autopsy was performed on a total of 48 motorcycle accident victims (39 riders and 9 passengers) in Cape Town during 1982. Thirty-nine (81%) were under 30 years of age, while 61,3% of the riders tested were found to have alcohol in their blood, 42% having a blood alcohol level of more than 0,10 g/dl.

Alcohol associated with fatal light aircraft accidents, United Kingdom-1964-1973.

A total of 86 fatal light aircraft accidents in the United Kingdom from January 1964 to December 1973 are reviewed, in which tissue alcohol determinations were undertaken on 102 pilots. Alcohol was detected in 34 cases varying from small amounts of 313 mg/100 ml. The difficulties of interpretation of post mortem tissue alcohol levels are discussed with particular reference to those cases with measured alcohols in excess of 20 mg/100 ml a level taken as possibly significant in context of accident causation. There was 12 pilots, involved in 10 of the 86 fatal accidents, considered as having inge

Alcohol consumption among persons living with hypertension: evidence from a population-based study in Cape Verde.

BACKGROUND: Despite the well-documented effects of alcohol consumption on hypertension risk, there are persons living with hypertension who consume alcohol and may be at risk of alcohol use disorder (AUD). This paradox poses a critical public health concern and underscores a need to understand the alcohol consumption behavior of persons living with hypertension. This study investigates the prevalence and factors associated with alcohol consumption and AUD among persons living with hypertension in Cape Verde. METHODS: Using the 2020 WHO STEPS survey, we analyzed the observations of 1,136 adult

Alcohol-related encounter surveillance using emergency medical service records in Massachusetts, 2013-2023.

BACKGROUND: Alcohol is one of the most used substances in the United States. It can be hard to accurately assess alcohol use through self-report or medical records because it is a widespread mainstream legal substance. Identifying alcohol-related encounters in emergency medical services (EMS) records provides additional data on alcohol use. METHODS: We used the Massachusetts Ambulance Trip Record Information System (MATRIS) to develop a definition for alcohol-related encounters (AREs) in EMS encounters based on alcohol-related words and symptoms. An additional definition flagged AREs related

Sudden cardiac death after alcohol intake: classification and autopsy findings.

Alcohol is known to have an immediate effect on cardiac rhythm, and previous studies have found that a notable proportion of sudden cardiac deaths (SCD) occur after alcohol intake. The objective of the present study was to investigate the association between the timing of alcohol intake and SCD. Our study population is drawn from the Fingesture study, which includes 5869 consecutive SCD cases from Northern Finland who underwent medicolegal autopsy 1998-2017. Toxicological analysis was performed if there was any suspicion of toxic exposure, or if there was no obvious immediate cause of SCD at a

Context, acute tolerance, and subjective response affect alcohol-impaired driving decisions.

RATIONALE: Alcohol intoxication produces effects that can impair judgment and increase engagement in risky behaviors, including alcohol-impaired driving (AID). Real-world AID decisions are informed by contextual circumstances and judgments of associated risk. How individuals vary in their AID decision-making across contexts and whether subjective alcohol responses (stimulation, sedation, acute tolerance) differentially affect AID decisions are critical, but under-studied research questions. OBJECTIVES: We systematically investigated predictors of AID decisions at different hypothetical drivin

Alcohol Use in Pregnancy.

Alcohol exposure during pregnancy results in impaired growth, stillbirth, and fetal alcohol spectrum disorder. Fetal alcohol deficits are lifelong issues with no current treatment or established diagnostic or therapeutic tools to prevent and/or ameliorate some of these adverse outcomes. Despite the recommendation to abstain, almost half of the women consume alcohol in pregnancy in the United States. This review focuses on the trends in prenatal alcohol exposure, implications for maternal and fetal health, and evidence suggesting that preconception and the prenatal period provide a window of op

Pancreas and liver injury are associated in individuals with increased alcohol consumption.

BACKGROUND & AIMS: Although chronic pancreatitis and liver cirrhosis are common sequelae of excess alcohol consumption, the 2 conditions are rarely associated. We studied the prevalence of simultaneous liver cirrhosis and chronic pancreatitis in alcoholics. METHODS: Postmortem autopsy data from 620 individuals with a history of excess alcohol consumption and 100 nonalcoholics (controls) were analyzed. The individuals were classified into groups based on macroscopic observations of pancreas (no injury, acute pancreatitis, fibrosis, and chronic pancreatitis) and liver (no injury, moderate steat

Comparison of DWI offenders with non-DWI individuals on the MMPI-2 and the Michigan Alcoholism Screening Test.

Two groups of driving-while-intoxicated (DWI) offenders with either one DWI offense or with repeat offenses were compared to a group of nonoffenders using the Minnesota Multiphasic Personality Inventory (MMPI-2) and the Michigan Alcoholism Screening Test (MAST). Demographic information was also collected regarding their prior legal history, family history, and blood alcohol level at the time of the DWI arrest. The results indicated both DWI groups had scored significantly higher than the comparison group on the K, Psychopathic Deviate (Pd) Scale, Over-Controlled Hostility (O-H) Scale, and MacA

[Health counseling about alcoholic beverage consumption].

This paper reviews the scientific evidence on the relationship between consumption of alcoholic beverages and health, at the individual and population levels. This evidence is used to inform the health counselling on such consumption. We propose that during the individual clinical encounter, the counselling should be adapted to clinical characteristics of the patient (mainly age and cardiovascular risk), consider the patient preferences (because there are alternatives to alcohol for cardiovascular prevention for those who do not like alcohol), and prudent ('primum non nocere'). For the populat

Alcohol Use and COVID-19 Outcomes.

BACKGROUND: There is increasing recognition that health behaviors may contribute to outcomes following COVID-19, but information on the role of alcohol use is limited. RESEARCH QUESTION: Are different severity levels of preexisting alcohol use disorder (AUD), alcohol consumption, and their combination associated with 30-day COVID-19 outcomes? STUDY DESIGN AND METHODS: Using data from a US national cohort (Veterans Health Administration), we determined the risk associated with AUD, based on diagnostic codes including alcohol abuse, alcohol dependence, and alcohol-related disease (ARD) for COV

Analyzing Questions About Alcohol in Pregnancy Using Web-Based Forum Topics: Qualitative Content Analysis.

BACKGROUND: Prenatal alcohol exposure represents a substantial public health concern as it may lead to detrimental outcomes, including pregnancy complications and fetal alcohol spectrum disorder. Although UK national guidance recommends abstaining from alcohol if pregnant or planning a pregnancy, evidence suggests that confusion remains on this topic among members of the public, and little is known about what questions people have about consumption of alcohol in pregnancy outside of health care settings. OBJECTIVE: This study aims to assess what questions and topics are raised on alcohol in p
