This study investigated the role of alcohol expectancy as a moderator of the relationship between drinking habits and self-reported frequency of physical aggression. Questionnaires were administered to a sample of 114 American male college students. After controlling for the subjects' ages, hostility, and attitude toward aggression, the relationship between drinking habits and frequency of physical aggression was significantly stronger for those expecting alcohol to increase aggression than for those expecting either a decrease or no effect on aggression. This finding lends support to expectan

This paper outlines the evolution of ideas on the topic of alcoholic hallucinosis. From 1847 to 1987 numerous descriptive case-histories appeared in the literature. Analysis of these demonstrates that there were some phenomenological features of the illness about which authors were agreed. These were the acute onset of the illness, the predominance of auditory hallucinations and a history of heavy drinking. Protagonists of the delirium tremens school of though drew attention to the slight clouding of consciousness, the presence of the physical symptoms which may accompany an acute confusional

One of the European targets in the "Health for all by the year 2000" programme is to reduce alcohol consumption significantly by the turn of the century. This article describes how this target, and especially the 25% goal included in it, has been adopted in the Nordic countries. With the exception of Denmark, alcohol has for a long time been regarded as a serious public health problem, and the reduction of total consumption of alcohol has been held as one of the most important ways of combating alcohol problems. In the 1980s Sweden and Norway have accepted the European 25% goal with the least

The aim was to construct a questionnaire analyzing pathological features possibly present in alcoholic liver disease, to assess its interobserver variation and to determine the influence of technical data on this variation. A total of 764 inpatients drinking 90 g (median) of pure alcohol per day for 25 years was observed; 402 patients were excluded because of associated nonalcoholic disease, refusal or contraindication to biopsy, leaving 362 patients included. Two pathologists independently analyzed each biopsy and completed a questionnaire including 41 items. Coefficient of concordance betwee

After taking paracetamol regularly in therapeutic doses, a non-cirrhotic alcoholic subject developed hepatic necrosis and acute renal failure. This case is compared with 13 others found in the literature, and the clinical, biochemical and histological characteristics of such accidents are described. The severity of the acute renal failure is proven by the fact that 50% of the patients had to be put under dialysis. The potentiation of acetaminophen renal and hepatic toxicity by alcohol toxicity is discussed. The principal mechanism of enhancement is the activation of the cytochrome P 450 system

Morphometric analysis of postmortem brains from chronic ethyl alcohol abusers and controls was performed to determine the regional distribution and extent of atrophy in the cerebral hemispheres of alcoholics. This study was performed by digitizing photographs of coronal slices of the brains to compute the cross-sectional area of the cerebrum, cerebral cortex, subcortical nuclei, cerebral white matter, and the ventricular system at five standardized levels. Although the alcoholics and controls had similar demographic features and mean brain weights, brains from the alcoholic group showed mild b

Alcohol is hepatotoxic in both established alcoholics and normal volunteers. Good nutrition does not protect against this effect. Those who are admitted to the hospital with evidence of alcoholic liver disease are uniformly malnourished. The malnourished sick, including those with alcoholic liver disease, do not fare as well with their disease as do the well-nourished sick. The data are compatible with the concept that malnutrition is a complication of alcoholism and interferes with rapid repair of alcoholic liver injury. Only in this important sense does poor protein intake contribute to the

Seventy-nine undergraduate social drinkers were the subjects in two studies. Both studies had two experimental sessions and followed a blind design. The subjects in experiment I (expectancy manipulation) were informed they would certainly receive an alcoholic beverage in one session, while in the other one they were told the beverage possibly contained alcohol. Thus, the different expectancy on the beverage content was evaluated. The volunteers in experiment II (attention demanding tasks) were submitted to attention tests in one session and in another one they were not. Thus, the assumption th

The performance characteristics of 4 computerized psychological tests were studied using alcohol as a model compound. Subjects received alcohol (0.5 ml/kg) or placebo in a cross-over design and performance was assessed using the Simple Reaction Time test, the Switching Attention test, the Hand/Eye Coordination test and the Color-Word Vigilance test. Analyses indicated an overall significant effect of alcohol at blood-alcohol levels of 0.03% with response speed on the Color-Word Vigilance test being the most sensitive parameter.

The symptoms and clinical management of alcohol, barbiturate and benzodiazepine withdrawal syndromes are discussed in this article. People who suffer alcohol withdrawal should be admitted to hospital if they have medical or surgical complications or severe symptoms; supportive care and pharmacotherapy, especially diazepam loading, are the essential components of treatment. Barbiturate withdrawal requires pharmacotherapy and admission to hospital for patients who have taken more than 0.4 g/d of secobarbital or an equivalent amount of another barbiturate for 90 days or longer, or 0.6 g/d or an e

To test for early evidence of alcoholic cardiomyopathy and to assess changes in exercise response after abstinence, 12 asymptomatic alcoholic men (group 1) underwent maximal upright bicycle exercise radionuclide ventriculography two to six days after alcohol withdrawal. Six of these patients (group 1A) had similar testing two to four weeks later. Six control subjects (group 2) had repeated exercise tests without isotope study. Group 1 left ventricular ejection fraction response (LVEF) was normal. LVEF at similar workloads did not differ in group 1A (p = NS). However, unlike group 2 results, th

The sensory, motor, and physical development of rats reared by dams, artificially reared on milk formula alone, or on milk formula combined with alcohol was examined. The rats given alcohol received 6.6 g/kg/day, which was administered either as 12 fractions distributed uniformly over 24 hours and resulted in stable blood alcohol concentrations (BACs) of 85 mg/dl, or condensed into 4 fractions over 8 hours and resulted in maximum and minimum BACs of 411 mg/dl and 45 mg/dl, respectively. Pups were reared artificially from postnatal days 4 to 12 with alcohol exposure occurring on days 4 to 10. A

Alcohol abuse in the State of Minnesota has an impact on health, health care resources, and the economy. Alcohol abuse was related to 3.3 per cent (1,150) of deaths in Minnesota in 1983; of these, almost one-third were the result of fatal injuries. Alcohol abuse contributed to 12 per cent (33,909) of all years of potential life lost, two-thirds of which were secondary to injury. The estimated cost of alcohol abuse ranged from $1.4 billion to $2.1 billion, representing from 2.8 per cent to 4.3 per cent of all personal income of Minnesotans, from 32 per cent to 50 per cent of State expenditures,

Malnutrition adversely effects hepatic function and structure. In certain instances, histological changes indistinguishable from alcoholic liver disease may be seen. Alcohol is clearly a direct hepatic toxin and its results are not prevented by normal nutritional intake. The majority of patients admitted to the hospital with alcoholic liver disease are moderately to severely malnourished but find it difficult or impossible to resume an intake sufficient to repair and maintain a normal nutritional state. Malnutrition may be considered a complication of alcohol abuse which occurs simultaneously

In 107 patients with radiologically confirmed calcifications of the pancreas (calcifications affecting the whole of the gland 57%, in one-third 27%, in two-thirds 16%), the incidence of accompanying liver damage was investigated. No relationship was to be found between the extent of pancreatic calcifications and the amount, nature or duration of alcoholic drinks consumed. Normal and insignificant findings were established in 40%, marked mesenchymal reactions in 32%, an advanced alcoholic hepatitis or incipient cirrhosis in 17%, and full-blown alcoholic cirrhosis in 11% of the cases. The etiology of malnutrition and the metabolic effect of aggressive nutritional support by enteral feeding were evaluated in patients with moderately severe alcoholic hepatitis. Among 21 patients presenting with jaundice, ascites, coagulopathy and low grade encephalopathy, the mean digestibilities (intake - fecal excretion/intake x 100) of total energy and fat were subnormal at 74.6 +/- 2.9 and 64.5 +/- 4.4%, respectively, and nitrogen balance was negative in half the patients with a mean value of +0.74 gm per day +/- 1.2. Based on initial ad libitum intake of hospital diet, patients were grou

Elevated levels of serum enzymes are frequently associated not only with alcohol-related organ damage but also with excessive alcohol consumption and alcoholism without significant tissue injury. However, both in the early detection of alcoholism as well as also in the diagnosis of alcohol-related diseases the sensitivities and specificities of these enzyme markers vary considerably. They may be influenced by nonalcohol-related diseases, enzyme-inducing drugs, nutritional factors, metabolic disorders, age, smoking, etc. Consequently, we have neither a single laboratory test--enzyme marker--nor

A quantitative histological study was made of the cerebellar vermis in 10 male alcoholic and 8 age-matched control cases. The mean Purkinje cell loss was 21% in the alcoholic group. The small rostral and caudal lobes were most severely affected. The mean Purkinje cell density per unit length of foliae showed similar changes. Histological measurements of the area of the molecular, granular and medullary layers of the cerebellar vermis showed that the molecular layer varied in the degree of shrinkage between lobes (from 11 to 39%) while the granular layer showed a consistent shrinkage (9 to 10%)
