Disseminated histoplasmosis partially mimicking a dermatomyositis in a patient with rheumatoid arthritis.

Histoplasma capsulatum var. capsulatum is a dimorphic fungus endemic to America and subtropical regions. Several cases of this opportunist mycosis have been reported in immunocompromised patients. We report the case of a patient treated with methotrexate and corticosteroid therapy for rheumatoid arthritis and who presented with disseminated histoplasmosis that partially mimicked a dermatomyositis.

Rickettsia rickettsii transmission by a lone star tick, North Carolina.

Only indirect or circumstantial evidence has been published to support transmission of Rickettsia rickettsii by Amblyomma americanum (lone star) ticks in North America. This study provides molecular evidence that A. americanum ticks can function, although most likely infrequently, as vectors of Rocky Mountain spotted fever for humans.

Lyme disease: an unusual case of peripheral nerve palsy.

Lyme disease is a vector-borne multisystem inflammatory disease caused by the spirochete Borrelia burgdorferi sensu lato. This disease is frequently seen in North America and to a lesser degree in Europe. However, its presence in England is uncommon and we present a case in which the patient developed a palsy of the common peroneal nerve.

Genetic stasis of dominant West Nile virus genotype, Houston, Texas.

The accumulation and fixation of mutations in West Nile virus (WNV) led to the emergence of a dominant genotype throughout North America. Subsequent analysis of 44 isolates, including 19 new sequences, from Houston, Texas, suggests that WNV has reached relative genetic stasis at the local level in recent years.

Intracranial tuberculoma.

In North America, central nervous system involvement by tuberculosis is uncommon. This patient review describes the clinical and radiological features of this unusual neurologic lesion. Special emphasis is given to the methods of arriving at the correct diagnosis, to the efficacy of combination antituberculous therapy, and to the possibility of paradoxical expansion of these lesions during the course of successful treatment.

Coeliac disease in Cuban children.

Coeliac disease is generally considered to be a disease of Europe, North America, and Australasia. A series of well-authenticated cases from Cuba is presented. One of the factors responsible for the presence of this disease in Cuba may be the increase in wheat consumption in the last few years. It is likely that coeliac disease exists in other tropical countries.

Spatio-temporal trends in COVID-19 morbidity and mortality due to elderly: a global perspective.

COVID-19 spread worldwide after emerging at the end of 2020 with a higher spread and death among the elderly and varies across the continent and country. This study aimed to investigate the spatio-temporal trends in COVID-19 morbidity and mortality due to the elderly. COVID-19 infected cases and death data were converted into morbidity and mortality rates per 1,000 population. Thematic maps were used for spatial illustration over time. Linear and Theil-Sen regression models were used to estimate the rate of change in COVID-19 morbidity and mortality due to the elderly. The results revealed a s

Diversity and clinical correlations of SARS-CoV-2 variant during the introduction of the Delta variant in Guatemala.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) genomic surveillance is crucial for understanding viral evolution and guiding public health responses. However, many countries, particularly in Central America, have limited sequencing capacity, resulting in scarce and delayed data. This study addresses this gap by analysing 320 SARS-CoV-2 genomes sequenced from a major diagnostic centre in Guatemala City, Guatemala, between April and August 2021. Clade 21J (Delta) was predominant (46.2%), followed by 19B (29.4%) and 20J (Gamma, 6.6%). The most reported symptoms were cough, headache,

First Detection of Hepatitis E Virus in Wastewater in Costa Rica.

Hepatitis E virus (HEV) is an important cause of acute viral hepatitis worldwide, with both human-restricted (genotypes 1 and 2) and zoonotic lineages (genotypes 3 and 4). However, information regarding HEV circulation in Central America remains limited. In this study, wastewater-based epidemiological surveillance was conducted in San José, Costa Rica, during 2025 to investigate the presence of HEV in the main wastewater treatment plant serving approximately one million inhabitants. A total of 54 influent and effluent samples were analyzed. HEV RNA was not detected by quantitative RT-PCR; howe

Direct evidence of trophic interaction between a crocodyliform and a large terror bird in the Middle Miocene of La Venta, Colombia.

Direct evidence of predation and other trophic relationships provide valuable information about trophic interactions between species in palaeo-communities. Data on ecological interactions amongst extant apex predators open a unique opportunity to better understand how sympatric apex predators coexisted or interacted with each other in the past. Here, we describe direct evidence of a predation or scavenging event in which we hypothesize that a medium-sized caiman (possibly Purussaurus neivensis) consumed (either through scavenging or through direct predation) a large terror bird. The distal par

Pregnancy and Childbearing for Orthopaedic Surgeons: Challenges and Successful Support Initiatives.

While female representation within surgical specialties is increasing, the field of orthopaedic surgery remains male-dominated. Residency, fellowship, and early career coincide with the childbearing years of female surgeons. Given the overlap between these critical career stages and years of childbearing, there has been a rise in articles characterizing the experiences and perceptions around childbearing and its impact on surgeons and their careers. Multiple studies have reported the alarmingly high rates of pregnancy complications, infertility, pregnancy loss, voluntary delay in childbearing,

Evidence of Morphological and Morphometric Differences in the Sella Turcica of Pteronotus mesoamericanus and P. mexicanus.

Morphological modifications are a potential mechanism for functional species and phylogenetic diversification. The sella turcica in mammals is a structure associated with the basisphenoid bone and serves as the receptacle for the pituitary gland; however, little is known about the morphological variation that may affect functionality in chiropterans. In this study, we provide morphological and morphometric evidence of differences between populations of Pteronotus mesoamericanus [the Gulf of Mexico] and P. mexicanus [the Pacific Coast] by describing variations in the dimensions of the dorsum se

Atypical Parkinsonism: An Uncommon Presentation of Disseminated Coccidioidomycosis.

Coccidioidomycosis is endemic in the southwestern United States, Central America, and South America. Coccidioidomycosis has a variety of clinical presentations. Coccoidal meningitis is a feared form of disseminated coccidioidomycosis with high mortality and mobility rates. We reported a case of a 64-year-old man who presented with a three-week history of gait abnormalities and back pain. The patient had atypical parkinsonism, signs of cogwheeling rigidity, a masked face, intention tremor, a shuffling gait, upgazed restriction, and long track signs of left Babinski. MRI of the brain and cervica

Human mpox co-infection with advanced HIV-1 and XDR-TB in a MSM patient previously vaccinated against smallpox: A case report.

Mpox is a zoonotic infectious disease caused by the mpox virus (MPXV). Historically, the majority of mpox cases have been documented in Central Africa. However, since May 2022, there has been a notable rise in reported cases from regions beyond Africa. Currently, over 110 countries spanning Europe, North America, South America, Asia, and other territories have reported mpox infections. This report details a case involving a patient who identifies as a man who has sex with men (MSM) and is concurrently infected with MPXV, human immunodeficiency virus type 1 (HIV-1), Pneumocystis jiroveci, as we

Understanding the global distribution of atypical porcine pestivirus (APPeV).

Atypical porcine pestivirus (APPeV) is a pestivirus affecting pigs, notably causing high mortality in piglets due to neurological issues that impair suckling. This study reviews global literature from 2015 to March 2024, assessing APPeV prevalence. Analysing 40 relevant articles, it finds APPeV widely distributed across Europe, South America, North America, and Asia, with minimal presence in Africa and Australia. The scarcity in these regions might be due to geographical isolation, environmental factors, limited surveillance, diagnostics, or under-reporting. China leads in APPeV prevalence rep

Molecular Detection of Dengue Virus, Zika Virus, and Chikungunya Virus Arboviruses in Neotropical Bats.

Background: The dengue, Zika, and Chikungunya arboviruses have spread in America in the past year, thus becoming global health issues. These viruses are maintained in nature in two transmission cycles: an urban cycle, transmitted from hematophagous mosquitoes to humans, and a wild cycle, recorded only in Africa and Asia, involving mosquitoes and nonhuman primates as natural hosts. The evidence shows that these arboviruses infect other wild mammals in America, such as rodents, marsupials, and bats. This study aimed to determine the potential natural infection of arboviruses in bats captured in

Disease Occurrence in- and the Transferal of Zoonotic Agents by North American Feedlot Cattle.

North America is a large producer of beef and contains approximately 12% of the world's cattle inventory. Feedlots are an integral part of modern cattle production in North America, producing a high-quality, wholesome protein food for humans. Cattle, during their final stage, are fed readily digestible high-energy density rations in feedlots. Cattle in feedlots are susceptible to certain zoonotic diseases that impact cattle health, growth performance, and carcass characteristics, as well as human health. Diseases are often transferred amongst pen-mates, but they can also originate from the env

Global distribution of prophylactic total gastrectomy in E-cadherin (CDH1) mutations.

Individuals with germline E-cadherin (CDH1) mutations are at high risk of developing diffuse gastric cancer (GC) and prophylactic total gastrectomy (PTG) represents the only life-saving treatment. We reviewed all PTGs reported in literature associated with CDH1 germline mutations. A total of 224 PTGs were reported. The majority were described in the United States of America (50%), the Netherlands (17.8%), and Canada (12.5%). GC was identified in 85.4% of cases after PTG, with a high rate of "no cancer" at histopathology identified in the United States of America (19.6%). Considering the mutati

Genome analysis suggests HTLV-1aA introduction in Chile related to migrations of ancestral indigenous populations.

The human T cell lymphotropic virus type 1 (HTLV-1), unlike other RNA viruses such as HIV, has a stable genome and has infected humans since remote times. Although the HTLV-1 infection is endemic in South America, there is scarce information about HTLV-1 in Chile and its history of introduction. This study assessed the genomic content of HTLV-1 from Chile and its relationship with HTLV-1 lineages circulating worldwide by phylogenetic reconstruction and dating analyses. A total of 30 HTLV-1 genomes collected from the four continents were used to conduct dating analyses, including the first HTLV
