A Case of Refractory Panuveitis with Coexisting Spondyloarthritis Successfully Treated with Tofacitinib.

INTRODUCTION: Non-infectious uveitis is commonly treated with corticosteroids, immunosuppressants, and, recently, TNF inhibitors. However, some patients do not achieve adequate control of uveitis with these drugs. This report presents a case of panuveitis with coexisting spondyloarthritis (SpA) in which both TNF and interleukin-17 (IL-17) inhibitors failed to control uveitis, but tofacitinib, a Janus kinase (JAK) inhibitor, was effective in treating uveitis. CASE PRESENTATION: A 54-year-old woman visited our hospital with a complaint of blurred vision in her left eye. A small retinal exudate

Concurrent infections in children with Kawasaki disease: lessons learned over 26 years.

Etiology of Kawasaki disease (KD) remains an enigma despite more than 50 years of extensive research. There is evidence that concurrent infections may play a role in the pathogenesis of KD. The present study reports various infections identified in a large cohort of patients with KD in Northwest India. We reviewed case records of patients with KD from January 1994 to February 2020. Patients with KD identified to have concurrent infection at presentation were analyzed in detail. Of 878 cases of KD during this period, 88 (60 boys, 28 girls; 64 incomplete KD, 24 complete KD) had evidence of concu

Intrinsic T cell glutaminolysis promotes autoimmunity in lupus-prone mice.

BACKGROUND: Surgical pathology specimens from patients with hypertrophic pachymeningitis are infrequently encountered. After excluding infectious and neoplastic causes, autoimmune conditions should be considered, especially neurosarcoidosis, rheumatoid arthritis, granulomatosis with polyangiitis (GPA), and IgG4-related disease (IgG4-RD) before the case is designated "idiopathic". However, even if histological findings strongly favor one of these conditions, clinical and serological correlation is mandatory. Further complicating the issue is evolving thinking that suggests there may be overlap

Assessment and management for latent tuberculosis before advanced therapies for immune-mediated inflammatory diseases: A comprehensive review.

Tuberculosis (TB), caused by Mycobacterium tuberculosis , is the most significant infectious cause of mortality across the globe. While TB disease can prey on immunocompetent individuals, it is more likely to occur in immunocompromised individuals. Immune-mediated inflammatory diseases (IMIDs) are a group of diseases (rheumatoid arthritis, inflammatory bowel disease, ankylosing spondylitis, psoriasis, hidradenitis suppurativa, autoimmune blistering diseases, and others) where there may be a need for systemic immunosuppression to control the disease manifestations, treat symptoms and improve lo

Simultaneous tibio-talar fusion and tibial lengthening for end-stage ankle arthritis.

PURPOSE: Arthrodesis of the ankle joint is an accepted treatment option in patients with end-stage ankle arthritis. The goal is to achieve fusion between the tibia and the talus, thereby stabilizing the joint and alleviating pain. There might be associated limb length discrepancy, especially in post-traumatic and post-infectious cases. These patients require limb lengthening and arthrodesis. The purpose of this study is to report our experience with simultaneous ankle arthrodesis and lengthening using external fixation in adolescent and young adult patients. METHODS: This retrospective case s

Non-invasive and accurate readout of superoxide anion in biological systems by near-infrared light.

Infectious and inflammatory diseases involve superoxide anion (O2•-) production. Real-time and non-invasive evaluation of O2•- in intact biological systems has been a significant challenge in biology and medicine. Here, I report that an advanced near-infrared chemiluminescent probe, MCLA-800, enables reliable non-invasive optical readout of O2•-ex vivo and in vivo. MCLA-800 allowed highly selective and sensitive monitoring of O2•- in undiluted human whole blood ex vivo. For the first time, the use of MCLA-800 revealed two reproducible types of O2•- production in response to stimulation by unop

Epstein-Barr Virus and Systemic Autoimmune Diseases.

Epstein-Barr Virus (EBV) is an extremely successful human herpes virus, which infects essentially all human beings at some time during their life span. EBV infection and the associated immune response results in production of antibodies (seroconversion), which occurs mainly during the first years of life, but may also happen during adolescence or later in life. Infection of adolescents can result in infectious mononucleosis, an acute serious condition characterized by massive lymphocytosis. Transmission of EBV mainly occurs through saliva but can rarely be spread through semen or blood, e.g. t

Advances in Disease Mechanisms and Translational Technologies: Clinicopathologic Significance of Inflammasome Activation in Autoimmune Diseases.

Autoimmune diseases are characterized by dysregulated immune tolerance to self and inflammatory damage to tissues and organs. The development of inflammation involves multiple innate and adaptive immune pathways. Inflammasomes are multimeric cytosolic protein complexes that form to mediate host immune responses upon recognizing pathogen- or damage-associated molecular patterns via pattern-recognition receptors (PRRs). The accelerating pace of inflammasome research has demonstrated important roles for inflammasome activation in many pathologic conditions, including infectious, metabolic, autoin
