To better define the risk of breast cancer in young patients, a retrospective review of all breast biopsies in women under age 40 years at Grady Memorial Hospital, Atlanta, from Dec 1, 1981, to Aug 15, 1987, was performed. During this time, 751 biopsies were performed on patients aged 9 to 40 years. None of the 128 patients aged 20 years or less had carcinoma. Of 150 patients aged 21 to 25 years, two had carcinoma. Pathologic discharge from the breast occurred in 308 out of 5000 (6.1%) apparently healthy women. Ductography was carried out in 237 such cases. The procedure revealed ductal ectasia in 96 patients (40.5%), ductal papillomatosis--38 (16.0%), intraductal papilloma--15 (6.8%) and cysts--in 16 cases. Cancer was suspected in 22 (9.3%) patients and in 14 (63.6%) of them tumor was diagnosed following excisional biopsy. On the whole, cancer was detected in 5.9% of the examined cases. In 13 out of 14 (92.9%), ductal carcinomas were either minimal (dia less than or equal to 1 cm) or noninvasive.

We describe a quantitative histological study of 34 breast biopsies using a marker for human macrophages, the monoclonal antibody EBM/11. Seventeen of the biopsies were of malignant tumours. Both benign and malignant breast tissue contained large numbers of macrophages with significantly higher numbers occurring in the malignant group. An analysis was made of macrophage counts according to stage, grade and prognostic index of the malignant tumours. The use of cytocentrifugation in the preparation of fine needle aspiration (FNA) specimens from the breast was evaluated. A total of 174 fine needle aspirates of breast masses were flushed into cytospin Collection Fluid, from which Papanicolaou-stained Cytospin preparations were made in the laboratory. For the purpose of defining the distribution of breast diseases in Nigeria, a review of 671 cases seen in Ife over a 10 year period was undertaken. Twenty-six (3.9 per cent) of the patients were males. In a retrospective histologic study of 1862 benign breast tissue specimens, 32 cases were found to have radial scar. The radial scar was multicentric in 44%, and found in breasts with fibrocystic disease and/or duct ectasia. During a follow-up period of 19.5 years (range, 15-24 years), one patient developed breast cancer compared with an expected rate of 0.94. Therefore, local excision of radial scar without further follow-up is sufficient.

Argyrophilic hormone storage granules were sought in the tissue specimens obtained from 52 benign breast lesions (13 normal breasts, 27 cases of fibrocystic disease, 9 fibroadenomas, 2 intraductal papillomas, and 3 cases of gynecomastia) and from 90 adenocarcinomas of the female breast.
