After performing selectively 25 laparoscopic cholecystectomies (LC) to determine the place of LC in the management of complicated gallstones, all patients presenting with gallstones were evaluated by the authors for LC. Eighty-six consecutive patients were evaluated and 84 were studied. Follow-up in every case exceeded 6 months. To define the significance of nonvisualization of the gallbladder by ultrasonography, we studied follow-up data on 31 cases in which the gallbladder could not be identified despite adequate fasting. Thirty one cases of gallbladder disease included 15 cases of chronic cholecystitis due to cholelithiasis (13 cases) and choledocholithiasis (2 cases), 4 cases of cholelithiasis which were not surgerized, and 3 cases of cancer. A prospective study to evaluate the accuracy of early diagnosis and the efficacy of early operation for biliary tract stone disease was performed. One hundred fifty-two consecutive patients with signs and symptoms compatible with biliary colic or acute cholecystitis were admitted to the hospital and promptly evaluated with ultrasonography and hepatobiliary (Pipida) scanning. The air crescent (AC) is a common radiological sign. Even if its commonest aetiology remains pulmonary aspergillosis, various other causes have been described. In this study, we report four rare causes of ACs seen on chest radiographs that haven't been described in the literature. Teaching point: The differential diagnosis of an air crescent sign on chest radiographs includes oesophageal bezoar, interstitial lung emphysema, central bronchial stenosis and perforated emphysematous cholecystitis.

Isolated pulmonary valve endocarditis (IPE) is a rare form of infectious endocarditis. This article reports the case of a 49-year-old patient with IPE who was initially admitted with suspected cholecystitis. After vegetations were detected by transthoracic (TTE) and transesophageal echocardiography (TEE), antibiotic therapy in accordance with the antibiogram was primarily attempted; however, due to persistently elevated infection parameters and structural valve damage a pulmonary valve replacement was eventually performed.

We present the case of 67 years-old man with unremarkable medical history. He was admitted to our department for abdominal pain compatible with choledocholithiasis associated with acute cholecystitis. ERCP was performed but direct papillary canulation attempts failed by means of conventional sphincterotome. So, pre- cut papillotomy was successfully attempted getting free access to distal choledochus and retrieving a small lithiasis. Unfortunately, the patient developed severe post-ERCP acute pancreatitis.

Acute biliary disease is a ubiquitous acute surgical complaint. General surgeons managing emergency surgical patients must be knowledgeable and capable of identifying and caring for common presentations. This article discusses the work-up, diagnosis, and management of the varying pathologies that make up biliary disease including cholelithiasis, cholecystitis, biliary dyskinesia, choledocholithiasis, cholangitis, gallstone pancreatitis, and gallstone ileus. Also addressed are more challenging and rare presentations including pregnancy and bariatric anatomy.

A study was conducted on 100 cases of Acute abdomen admitted in surgery department of Govt Medical college and Rajendra Hospital, Patiala, India. Study group included patients with different abdominal emergencies, e.g. gastrointestinal perforation, intestinal obstruction, acute appendicitis, acute cholecystitis, pancreatitis etc. Out of these, three cases were positive for HBsAg alone, one for anti Hepatitis C-Virus (HCV) alone and one was positive for both HBsAg and anti HCV.

Gallstone spillage during laparoscopic cholecystectomy is a relatively common occurrence. These intraperitoneal stones rarely give rise to complications. We present the case of a 68 female who presented with a 5 cm diameter epigastric mass two years after a laparoscopic cholecystectomy for acute-on-chronic cholecystitis with gallbladder stones. CT abdomen demonstrated an inflammatory mass with central calcification. Laparotomy and excision of the mass revealed the diagnosis as a gallstone granuloma.

In this report, 140 cases underwent laparoscopic cholecystectomy (LC) with preoperative diagnosis of cholelithiasis in 134, noncalculous cholecystitis, and gallbladder polypus in three each. Ten patients had a history of abdominal surgery. Ten patients were converted to open surgery because of severe adhesions and hemorrhage. Complications were found in 6 including subcutaneous emphysema in 2, subhepatic infection in 1, bile fistula in 2, and postoperative intra abdominal hemorrhage in 1.

A prospective computed tomography (CT) study of a group of patients suspected of having gallbladder disease was performed. Ten patients were evaluated who had gallbladder nonvisualization on two consecutive oral contrast medium administrations. Evaluation of the gallbladder and common bile duct by CT disclosed specific abnormalities including gallstones, unsuspected common bile duct obstruction, and acute and chronic cholecystitis. Additional information on the physiologic processes involved in gallbladder nonvisualization is offered.

The authors report a case of a 79-year-old female who presented with signs and symptoms of acute cholecystitis. She was taken to theatre within 24 h of acute admission to undergo laparoscopic cholecystectomy. The gallbladder was found to have undergone torsion upon its mesentery leading to its infarction and necrosis. Laparoscopic cholecystectomy was performed, and the patient made an uneventful recovery.

The purpose of the study was to evaluate efficacy of poliphepan and hepabene as components of complex therapy in patients with chronic cholecystitis (CC). The subjects were 33 CC patients with biliary tract (BT) dyskinesia and biliar sludge. Application of the drugs led to significant improvement of clinical symptoms and the parameters of BT functional condition, as well as to decrease of bile lithogenicity and improvement of life quality.

Intraoperative fibrocholangioscopy (FCS) was used in 446 patients with cholelithic disease and acute cholecystitis. 399 (89.5%) of the patients were hospitalized urgently. The use of FCS in patients with choledocholithiasis, obturative jaundice, cholangitis and pancreatitis is analysed. The safety and diagnostic accuracy of the method are proved. The use of FCS has provided significant improvement of the immediate and long-term results of the biliary tract surgery.

Based on the studies made in 84 patients with calculous cholecystitis and 12 patients with calcifying pancreatitis, an efficiency was shown of infusional administration of pyrencepine or ranitidine to prevent stress postoperative erosive-and-ulcerous lesions of gastroduodenal zone. Gastrocepine is shown to be superior to ranitidine and can be withdrawn without a maintenance dose, as well as becouse its cytoprotective effects with respect of the gastric and duodenal mucosa.

A prototype Doppler probe was used laparoscopically to identify the cystic artery in 30 consecutive patients undergoing laparoscopic cholecystectomy. This pilot study revealed that the probe provided valuable assistance in identifying the artery in 33.3% of the patients. In 10% of the patients, the Doppler identified the artery when standard dissection techniques failed to do so. The probe was particularly helpful in patients with acute cholecystitis.

It is reported that 12.5% of the Italian population suffer from cholecystopathy. The Authors carried out a retrospective analysis of the cases observed over an 18-month period. It was found that ultrasonography was the most appropriate screening method to reveal cholecystic pathologies and diseases of the biliary tract since it is specific and sensitive and also enables adjacent organs to be examined (liver, kidneys, aorta, pancreas, spleen).

The effect of pentagastrin (dose: 6 mkg/kg on the motor function and intracavitary pressure of the stomach and duodenum was studied in 28 healthy persons and 352 patients with duodenal ulcer, chronic gastritis, chronic pancreatitis, chronic cholecystitis). It was found that pentagastrin stimulates appearance of the III antroduodenal phase. It is recommended to use pentagastrin in patients with chronic antroduodenal disorders of passability and antroduodenal stasis.
