Results 28 months following transpupillary thermotherapy for classic and occult choroidal neovascularization in patients with age-related macular degeneration.

BACKGROUND AND OBJECTIVE: To report the outcome of patients 28 months following treatment with transpupillary thermotherapy (TTT) for classic and occult choroidal neovascularization (CNV) secondary to age-related macular degeneration. PATIENTS AND METHODS: A nonrandomized pilot trial of 36 eyes of 33 patients was performed. Eyes with angiographically defined CNV, 11 predominantly classical and 25 predominantly occult, were treated with large spot diode laser (810 nm) TTT for 1 minute, the end point being no or minimal visible change. Outcome was assessed with best-corrected LogMAR visual acui…

Delayed-onset expulsive choroidal hemorrhage attributed to an acute elevation in systemic blood pressure following traumatic globe rupture.

The authors describe a 78-year-old woman who suffered a traumatic partial dehiscence of a penetrating keratoplasty on the day prior to presentation. While awaiting surgical repair, the patient experienced an expulsive choroidal hemorrhage necessitating a primary evisceration of the eye. This case is unique because the hemorrhage can be largely attributed to the acute dramatic rise in systemic blood pressure that immediately preceded it. Management considerations for patients with open-globe injuries who have poorly controlled systemic hypertension should include close monitoring of vital signs…

Visual rehabilitation in a child with diffuse choroidal hemangioma by using aggressive amblyopia therapy with low-dose external beam irradiation.

Diffuse choroidal hemangioma is a congenital vascular hamartoma often associated with hemangiomatous lesions of the brain, orbit, and periocular skin (nevus flammeus) in the Sturge-Weber syndrome. Visual loss from diffuse choroidal hemangioma may result from chronic serous retinal detachments causing retinal pigment epithelial, photoreceptor or cystoid degeneration, and glaucomatous optic atrophy. Low-dose external beam irradiation has successfully resolved exudative retinal detachment and caused shrinkage of the choroidal hemangioma.(1-3) Visual loss in Sturge-Weber syndrome with diffuse chor…

A large anomalous anterior choroidal artery associated with internal carotid artery-posterior communicating artery aneurysm. Case report.

A case of a large anomalous anterior choroidal artery (AchA) associated with internal carotid artery-posterior communicating artery aneurysm is reported. At surgery, the large anomalous AchA mimicked the PcomA. Therefore, we initially mistook the large anomalous AchA for the PcomA, and the ICA-PcomA aneurysm for the dorsal ICA aneurysm arising at a location of other than an arterial division. This is a rare case of an anomaly of the AchA with an associated aneurysm found at surgery. Anatomical and surgical considerations are made.

Treatment failure after endoscopic third ventriculostomy with choroid plexus cauterization, endoscopic third ventriculostomy alone, and ventriculoperitoneal shunt in congenital hydrocephalus: a network and time-to-event meta-analysis.

PURPOSE: Congenital hydrocephalus is commonly treated with endoscopic third ventriculostomy (ETV), ETV with choroid plexus cauterization (ETV + CPC), or ventriculoperitoneal shunting (VPS), yet outcomes vary, and treatment failure is frequent. This study examines treatment failure between procedures in patients with congenital hydrocephalus. METHODS: We conducted a database search from inception to February 2025 for head-to-head studies comparing the primary treatments. A network meta-analysis (NMA) was employed to assess failure rates, and pseudo-individual patient data from published Kaplan…

Assessing glymphatic role in the association between choroid plexus enlargement and white matter lesions load: The uncertain path through diffusion tensor imaging along the perivascular space (DTI-ALPS).

BackgroundThe choroid plexus (CP) is involved in cerebrospinal fluid production, immune surveillance, and brain fluid homeostasis. CP enlargement has been linked to neuroinflammation, glymphatic dysfunction, and white matter lesion (WML) burden, but the mechanisms underlying these associations remain uncertain.ObjectiveTo investigate the relationships among CP volume, glymphatic function estimated using diffusion tensor imaging along the perivascular space (DTI-ALPS), WML burden, and global cognitive performance in individuals with suspected neurodegenerative conditions, and to assess whether …

Effects of kynurenine pathway metabolites on choroid plexus volume, hemodynamic response, and spontaneous neural activity: A new mechanism for disrupted neurovascular communication and impaired cognition in mood disorders.

Major Depressive Disorder (MDD) and Bipolar Disorder (BD) involve alterations of immune-inflammatory setpoints that activate the kynurenine pathway (KP), affecting serotoninergic and glutamatergic neurotransmission through indoleamine-2,3-dioxygenase (IDO) activity. This process produces metabolites like Kynurenine (Kyn), 3-Hydroxykynurenine (3-HK), Quinolinic acid (QuinA), and Kynurenic acid (KynA), these last two acting as agonist and antagonist at glutamatergic N-methyl-D-aspartate receptors (NMDARs), respectively. NMDARs, expressed in the choroid plexus (ChP) and arteriolar smooth muscle c…

Processing mechanism of guanidinoacetate in choroid plexus epithelial cells: conversion of guanidinoacetate to creatine via guanidinoacetate N-methyltransferase and monocarboxylate transporter 12-mediated creatine release into the CSF.

BACKGROUND: Guanidinoacetate (GAA) induces epileptogenesis and neurotoxicity in the brain. As epileptic animal models have been reported to show elevated cerebral GAA levels, the processing mechanism of GAA in the brain is important for maintaining brain homeostasis. We have revealed that GAA in the cerebrospinal fluid (CSF) is removed by incorporation into the choroid plexus epithelial cells (CPxEpic), which form the blood-CSF barrier (BCSFB). However, the processing mechanism of GAA incorporated into CPxEpic remains unknown. We have reported that monocarboxylate transporter 12 (MCT12) functi…

Post-injury ventricular enlargement associates with iron in choroid plexus but not with seizure susceptibility nor lesion atrophy-6-month MRI follow-up after experimental traumatic brain injury.

Ventricular enlargement is one long-term consequence of a traumatic brain injury, and a risk factor for memory disorders and epilepsy. One underlying mechanisms of the chronic ventricular enlargement is disturbed cerebrospinal-fluid secretion or absorption by choroid plexus. We set out to characterize the different aspects of ventricular enlargement in lateral fluid percussion injury (FPI) rat model by magnetic resonance imaging (MRI) and discovered choroid plexus injury in rats that later developed hydrocephalus. We followed the brain pathology progression for 6 months and studied how the ven…

Comparison of hydrocephalus metrics between infants successfully treated with endoscopic third ventriculostomy with choroid plexus cauterization and those treated with a ventriculoperitoneal shunt: a multicenter matched-cohort analysis.

OBJECTIVE It has been suggested that the treatment of infant hydrocephalus results in different craniometric changes depending upon whether ventriculoperitoneal shunt (VPS) placement or endoscopic third ventriculostomy with choroid plexus cauterization (ETV/CPC) is performed. Without an objective and quantitative description of expected changes to the infant cranium and ventricles following ETV/CPC, asserting successful treatment of hydrocephalus is difficult. By comparing infants successfully treated via ETV/CPC or VPS surgery, the authors of this study aimed to define the expected postoperat…
