Understanding the complexity of surgical decision-making for individuals with symptomatic lumbar spinal stenosis: A qualitative study.

BACKGROUND: Understanding the factors influencing surgical decisions specific to symptomatic lumbar spinal stenosis (SLSS) can help healthcare providers support patients, set expectations and improve health literacy. Therefore, the purpose of this study is to explore the experiences and perspectives of Canadian patients who choose to undergo surgery for SLSS. METHODS: We used qualitative interpretive phenomenology to understand the surgical decision-making process from individuals with lived experience of SLSS. We conducted semi-structured qualitative interviews that lasted between 30 and 90

Supporting patient decision-making in non-invasive prenatal testing: a comparative study of professional values and practices in England and France.

BACKGROUND: Non-invasive prenatal testing (NIPT), which can screen for aneuploidies such as trisomy 21, is being implemented in several public healthcare systems across Europe. Comprehensive communication and information have been highlighted in the literature as important elements in supporting women's reproductive decision-making and addressing relevant ethical concerns such as routinisation. Countries such as England and France are adopting broadly similar implementation models, offering NIPT for pregnancies with high aneuploidy probability. However, we do not have a deeper understanding of

How information about overdetection changes breast cancer screening decisions: a mediation analysis within a randomised controlled trial.

OBJECTIVES: In a randomised controlled trial, we found that informing women about overdetection changed their breast screening decisions. We now present a mediation analysis exploring the psychological pathways through which study participants who received the intervention processed information about overdetection and how this influenced their decision-making. We examined a series of potential mediators in the causal chain between exposure to overdetection information and women's subsequently reported breast screening intentions. DESIGN: Serial multiple mediation analysis within a randomised

Evaluating multivariable clinical prediction models for Parkinson's disease prognosis: A scoping review.

Multivariable clinical prediction models combine two or more patient predictor variables to calculate the probability of a clinical outcome. In persons with Parkinson's disease (PD), clinical prognostic models can be used to predict disease progression or the occurrence of a clinical event. These models can aid in PD prognostication and complement clinical judgment by informing medical decision-making and resource allocation. In this scoping review of the literature from inception through May 2025, we present data from 74 multivariable prognostic models in PD, organized by clinical outcome. We

The patient's perspective on carpal tunnel surgery related to the type of anesthesia: a prospective cohort study.

CONTEXT: In Australia, rural clinical schools (RCSs) were developed to address the maldistribution of the rural medical workforce. Evidence demonstrates that medical students who attend an RCS, or have a rural background, are more likely to become rural doctors. To enhance the likelihood of our graduates from Deakin University becoming rural doctors, we strategically combined these two independent factors and created a dedicated rural training stream (RTS), which commenced in 2022. To support the introduction of the RTS and provide students with an authentic RCS experience, we developed a 3-da

Discordance between observer and patient-reported shared decision-making in inflammatory bowel disease: A multicenter cross-sectional study.

BACKGROUND:: Inflammatory bowel disease (IBD) is a chronic inflammatory disorder affecting the gastrointestinal tract. Optimal management of this condition requires collaboration between patients and physicians, a process known as shared decision-making (SDM). In this study, we aim to evaluate the quality of SDM in gastroenterology clinics in the Eastern Province of Saudi Arabia. METHODS:: This multicenter, cross-sectional study was conducted between September 2024 and December 2025. Audio recordings of gastroenterology visits were independently assessed by two reviewers using the validated O

[Visceral surgery in mass casualty incidents due to disasters, crises and war].

BACKGROUND: Mass casualty incidents with a focus on visceral surgery pose major medical and organizational challenges for hospitals. They are characterized by a large number of simultaneously admitted patients, limited personnel and material resources as well as the necessity to make time-critical surgical decisions. From a visceral surgical perspective, abdominal injuries involving severe bleeding, contamination and multiorgan involvement are particularly demanding as they require rapid surgical intervention and close interdisciplinary cooperation. This injury pattern is typically encountered

How Racism and Discrimination Impacts Black Young Women's Sexual Health: The Influence of Racial and Sexual Stereotypes on Educational Access.

PURPOSE: Structural racism and racial discrimination may increase adverse sexual health outcomes in Black adolescent girls. However, the influence of racism on sexual health has not been well-described in qualitative studies. The purpose of this study was to explore definitions of structural racism and determine how experiences of racial discrimination impact sexual health and decision-making in a sample of adolescent Black girls. METHODS: Black girls aged 13-19 years were recruited from an ongoing longitudinal study. Virtual focus groups and individual interviews were facilitated, inquiring

Decision-making and referral processes for patients with motor neurone disease: a qualitative study of GP experiences and evaluation of a new decision-support tool.

BACKGROUND: The diagnosis of motor neurone disease (MND) is known to be challenging and there may be delay in patients receiving a correct diagnosis. This study investigated the referral process for patients who had been diagnosed with MND, and whether a newly-developed tool (The Red Flags checklist) might help General Practitioners (GPs) in making referral decisions. METHODS: We carried out interviews with GPs who had recently referred a patient diagnosed with MND, and interviews/surveys with GPs who had not recently referred a patient with suspected MND. We collected data before the Red Fla

A comparative analysis of health technology assessment decision-making for cancer drugs in Germany and England.

IntroductionThe rising cost of cancer drugs places a heavy burden on patients and public healthcare systems, posing an ongoing challenge of balancing access with efficient resource allocation. Health technology assessment (HTA) serves as a critical basis for reimbursement decisions, evaluating both clinical and economic value. Although the Federal Joint Committee (G-BA) and the National Institute for Health and Care Excellence (NICE) have established standardized HTA frameworks, differences in their assessment methods and decision-making processes may lead to divergent outcomes. This study com

Values and preferences towards the use of prophylactic low-molecular-weight heparin during pregnancy: a convergent mixed-methods secondary analysis of data from the decision analysis in shared decision making for thromboprophylaxis during pregnancy (DASH-TOP) study.

BACKGROUND: Venous thromboembolism (VTE) in pregnancy is a major cause of maternal morbidity and mortality, and the use of preventive low-molecular-weight heparin (LMWH) can be challenging. Clinical guidelines recommend eliciting pregnant individuals' preferences towards the use of daily injections of LMWH and discussing the best option through a shared decision-making (SDM) approach. Our aim was to identify individuals' preferences concerning each of the main clinical outcomes, and categorize attributes influencing the use of LMWH during pregnancy. METHODS: Design: Convergent mixed-methods.

Psychological mechanisms of loss aversion: A drift-diffusion decomposition.

Decision makers often reject mixed gambles offering equal probabilities of a larger gain and a smaller loss. This important phenomenon, referred to as loss aversion, is typically explained by prospect theory, which proposes that decision makers give losses higher utility weights than gains. In this paper we consider alternative psychological mechanisms capable of explaining loss aversion, such as a fixed utility bias favoring rejection, as well as a bias favoring rejection prior to gamble valuation. We use a drift diffusion model of decision making to conceptually distinguish, formally define,

Moving the needle towards the democratization of echocardiography: a case report.

BACKGROUND: Echocardiography has been traditionally performed in echo labs and the potential benefits of its use by primary care physicians (PCPs) are still unexplored. We present a case where POCUS (point-of-care ultrasound) was used as a complement of physical examination by a family doctor, allowing a prompt clinical decision in a heart failure (HF) patient. CASE SUMMARY: An 85-year-old woman, living independently, asks her family doctor for a home consultation due to increasing dyspnoea. On examination, severe dyspnoea and bilateral ankle oedema was noted and a point-of-care echocardiogra

Tough choices: dialysis, palliative care, or a third option for elderly ESRD.

Dialysis should not be presumed to be the treatment of choice for all elderly chronic kidney disease stage 5 patients. Nondialysis active medical management, as an alternative to dialysis or palliative care, is a reasonable alternative in select cases. Early referral of CKD 5 elderly patients may lead to early initiation of dialysis, which may not be advantageous; it also provides an opportunity to institute active management as a treatment alternative. The informed decision to proceed with dialysis must involve both an assessment of evidence-based outcomes applicable to the patient, and allow

Machine Learning Model for Readmission Prediction of Patients With Heart Failure Based on Electronic Health Records: Protocol for a Quasi-Experimental Study for Impact Assessment.

BACKGROUND: Care for patients with heart failure (HF) causes a substantial load on health care systems where a prominent challenge is the elevated rate of readmissions within 30 days following initial discharge. Clinical professionals face high levels of uncertainty and subjectivity in the decision-making process on the optimal timing of discharge. Unwanted hospital stays generate costs and cause stress to patients and potentially have an impact on care outcomes. Recent studies have aimed to mitigate the uncertainty by developing and testing risk assessment tools and predictive models to ident
