Patterns of insomnia and its treatment in North Central London: using primary care data to establish unmet needs and health inequalities.

BACKGROUND: Existing research demonstrates that insomnia is common, with significant negative impacts on health and quality of life. Cognitive-behavioural therapy for insomnia (CBT-I), the first-line treatment, is highly cost-effective. However, healthcare records have not been used in the UK to establish real-world insomnia prevalence, inequalities or unmet need. AIMS: This study's aim was to establish the above in North Central London. METHOD: Data were extracted from primary care records across three London boroughs for 765 035 patients. Prevalence was determined by identifying those with

Interdisciplinary management of vaginismus: Case series on psycho-behavioral interventions by mental health and gynecological teams.

BACKGROUND: Vaginismus, categorized under ICD-11 as a sexual pain-penetration disorder, is often overlooked in clinical practice, particularly in culturally conservative societies where stigma, lack of awareness, and gendered misconceptions delay help-seeking. This case series examines the interdisciplinary management of vaginismus. CASES: Five women (ages 22-35) with lifelong or acquired vaginismus, diverse comorbidities (e.g., dysthymia, childhood trauma, infertility distress), and delayed help-seeking due to the sociocultural barriers were managed through an interdisciplinary model. Gyneco

Predicting Alzheimer's progression in MCI: a DTI-based white matter network model.

OBJECTIVE: This study aimed to identify features of white matter network attributes based on diffusion tensor imaging (DTI) that might lead to progression from mild cognitive impairment (MCI) and construct a comprehensive model based on these features for predicting the population at high risk of progression to Alzheimer's disease (AD) in MCI patients. METHODS: This study enrolled 121 MCI patients from the Alzheimer's Disease Neuroimaging Initiative (ADNI). Among them, 36 progressed to AD after four years of follow-up. A brain network was constructed for each patient based on white matter fib

Ketamine-enhanced prolonged exposure therapy in veterans with PTSD: A randomized controlled trial protocol.

BACKGROUND: The 2023 VA/DoD Clinical Practice Guideline for the Management of PTSD recommends individual, manualized trauma-focused such as Prolonged Exposure (PE) over pharmacologic interventions for the primary treatment of PTSD. Unfortunately, clinical trials of trauma-based therapies in the military and veteran population showed that 30% to 50% of patients did not demonstrate clinically meaningful symptom change. Ketamine, an FDA-approved anesthetic with potent non-competitive glutamatergic N-methyl-d-aspartate antagonistic properties, has demonstrated to enhance the recall of extinction l

Multimodal-based machine learning approach to classify features of internet gaming disorder and alcohol use disorder: A sensor-level and source-level resting-state electroencephalography activity and neuropsychological study.

OBJECTIVES: Addictions have recently been classified as substance use disorder (SUD) and behavioral addiction (BA), but the concept of BA is still debatable. Therefore, it is necessary to conduct further neuroscientific research to understand the mechanisms of BA to the same extent as SUD. The present study used machine learning (ML) algorithms to investigate the neuropsychological and neurophysiological aspects of addictions in individuals with internet gaming disorder (IGD) and alcohol use disorder (AUD). METHODS: We developed three models for distinguishing individuals with IGD from those

Neurofeedback training of executive function in autism spectrum disorder: distinct effects on brain activity levels and compensatory connectivity changes.

BACKGROUND: Deficits in executive function (EF) are consistently reported in autism spectrum disorders (ASD). Tailored cognitive training tools, such as neurofeedback, focused on executive function enhancement might have a significant impact on the daily life functioning of individuals with ASD. We report the first real-time fMRI neurofeedback (rt-fMRI NF) study targeting the left dorsolateral prefrontal cortex (DLPFC) in ASD. METHODS: Thirteen individuals with autism without intellectual disability and seventeen neurotypical individuals completed a rt-fMRI working memory NF paradigm, consist…

Effect of neurofeedback training on psychological features and plantar pressure distribution symmetry in patients with patellofemoral pain: A ransdomized controlled trial.

OBJECTIVE: The evidence shows changes in plantar pressure asymmetry are associated with patellofemoral joint overloading and may be effective in patellofemoral pain (PFP) etiology. Psychological factors such as catastrophizing and kinesiophobia causes a change in movement behavior which can lead to a plantar pressure distribution asymmetry. This study evaluated the effect of neurofeedback training (NFBT) on psychological variables and plantar pressure distribution symmetry in patients with PFP. METHOD: Thirty males with PFP were recruited for clinical trials. They were randomly divided into e

Mindfulness-Oriented Recovery Enhancement: Implementing an evidence-based intervention for chronic pain, opioid use, and opioid addiction in clinical settings.

The opioid crisis emerged in part due to the overprescribing of opioid analgesics for chronic pain. Although not the only source of the current epidemic of opioid use disorder (OUD), the prescription of opioids for chronic pain remains one vector for the development of opioid misuse and OUD. However, opioid tapering is not appropriate for all patients, and some patients may need to remain on opioid therapy for the long term. To reduce the risk of opioid-related harm among people with chronic pain and to treat incipient or entrenched addictive behaviours, new interventions are needed. This revi

Structural brain changes associated with cocaine use and digital cognitive behavioral therapy in cocaine use disorder treatment.

BACKGROUND: Few studies have investigated changes in brain structure and function associated with recovery from cocaine use disorder (CUD), and fewer still have identified brain changes associated with specific CUD treatments, which could inform treatment development and optimization. METHODS: In this longitudinal study, T1-weighted magnetic resonance imaging scans were acquired from 41 methadone-maintained individuals with CUD (15 women) at the beginning of and after 12 weeks of outpatient treatment. As part of a larger randomized controlled trial, these participants were randomly assigned t
