Early community context, genes, and youth body mass index trajectories: an investigation of gene-community interplay over early life course.

PURPOSE: To investigate additive and interactive influences of community adversity and cumulative genetic sensitivity on youth body mass index (BMI) trajectories over adolescence and young adulthood. METHODS: We used latent growth curve modeling to examine BMI trajectories over three waves (1995, 2001, and 2008) of the National Longitudinal Study of Adolescent Health (n = 14,563). We measured genetic sensitivity by a cumulative index of genes associated with serotonin and dopamine functions. RESULTS: Community adversity was positively associated with the initial level and rate of change in B

Clinical unity and community empowerment: the use of smartphone technology to empower community management of chronic venous ulcers through the support of a tertiary unit.

BACKGROUND: Chronic ulcers affect roughly 60,000 Irish people, at a total cost of €600,000,000, or €10,000 per patient annually. By virtue of their chronicity, these ulcers also contribute a significant burden to tertiary outpatient vascular clinics. OBJECTIVE: We propose utilizing mobile phone technology to decentralise care from tertiary centres to the community, improving efficiency and patient satisfaction, while maintaining patient safety. METHODS: Bespoke mobile software was developed for Apples iPhone 4 platform. This allowed for the remote collection of patient images prospectively a

Effect on postpartum hemorrhage of prophylactic oxytocin (10 IU) by injection by community health officers in Ghana: a community-based, cluster-randomized trial.

BACKGROUND: Oxytocin (10 IU) is the drug of choice for prevention of postpartum hemorrhage (PPH). Its use has generally been restricted to medically trained staff in health facilities. We assessed the effectiveness, safety, and feasibility of PPH prevention using oxytocin injected by peripheral health care providers without midwifery skills at home births. METHODS AND FINDINGS: This community-based, cluster-randomized trial was conducted in four rural districts in Ghana. We randomly allocated 54 community health officers (stratified on district and catchment area distance to a health facility

Rapid community identification, pain and distress associated with lymphoedema and adenolymphangitis due to lymphatic filariasis in resource-limited communities of North-eastern Nigeria.

Identification of communities with people that could benefit from adenolymphangitis (ADL) and lymphoedema morbidity management within Lymphatic Filariasis Elimination Programmes (NLFEP) in many African countries is a major challenge to programme managers. Another challenge is advocating for proportionate allocation of funds to alleviating the suffering that afflicted people bear. In this study we developed a rapid qualitative technique of identifying communities where morbidity management programme could be situated and documenting the pain and distress that afflicted persons endure. Estimates

Provocative poliomyelitis causing postpolio residual paralysis among select communities of two remote villages of North Karnataka in India: a community survey.

Intramuscular injections can provoke muscular paralysis especially, if the child has had exposure to polio virus. The purpose of the study was to determine the association with known risk factors for motor disabilities in two remote villages of North Karnataka (India), where an increased number of disabled people among select communities had been reported. A community based survey was conducted. The selection of study subjects was done through screening, history related with occurrence of musculoskeletal disability, screening and general examination of the affected joints and muscles. Data ana

Detection of prediabetes and undiagnosed type 2 diabetes in underserved communities using community pharmacy point-of-care testing: A prospective feasibility study.

AIMS: Early detection of type 2 diabetes (T2D) and prediabetes is critical to reduce disease burden, yet disadvantaged populations often remain undiagnosed. We assessed a pharmacy-based pathway using verified point-of-care HbA1c testing to identify prediabetes and enable engagement with a digital prevention programme and established the detection rates for undiagnosed T2D. METHODS: We conducted a prospective feasibility study across 16 community pharmacies in NHS Tayside, Scotland (Feb 2024-Jan 2025). Adults at high risk of T2D, identified using the Diabetes UK 'Know Your Risk' tool, were off

Integrative Profiling of Antimicrobial Resistance Risks in a Philippine Peri-Urban Community: Insights From Clinical Data, Wastewater Analysis and Community Engagement.

Antimicrobial resistance (AMR) is a growing global health threat, particularly in low- and middle-income countries such as the Philippines, where limited resources and regulatory challenges heighten the risk. Community-level assessments are critical for developing a context-specific evidence base that informs local decision-making. This study assessed the community AMR risk in a peri-urban community (Mandug, Davao City, Philippines) using an integrated approach that combined wastewater detection of antimicrobial resistance genes (ARGs), community survey and interviews on knowledge, practices,

Determinants of trust among ethnic minority community members towards community-based organisations and leaders during pandemics and other emergencies in Australia.

BACKGROUND: Effective pandemic plans must be applicable to all members of our communities. However, people from multicultural communities with diverse language needs often engage with a wide range of organisations and individuals when seeking information during a pandemic or health emergency. Despite this, there is limited evidence on levels of trust and the influence of these sources on promoting community engagement with pandemic preparedness activities in high-income settings. METHODS: A cross-sectional survey was conducted among members of ethnic minority communities in Australia who self

Integrating Community Health System Activities and Participation into the Primary Healthcare System for Ensuring Effective Community Participation and Achieving Health Goals in Nigeria.

Introduction: In Nigeria, communities have long organised health and social actions that address local needs but remain weakly integrated into the formal primary healthcare (PHC) system. Understanding these approaches and how they interface with PHC is important for advancing universal health coverage (UHC). Objective: This study examined community-driven health and social actions and explored pathways for their integration into the formal PHC system. Method: A qualitative study was conducted in three purposively selected Nigerian states: Anambra, Akwa Ibom, and Kano. Data were collected throu

Early community reactions and acceptance of the Sliding-Scale Community Based Health Insurance Scheme in Ethiopia: Qualitative Findings from the treatment arm.

BACKGROUND: Ethiopia's Community-Based Health Insurance (CBHI) launched in 2011, has significantly improved access to healthcare. However, the program has faced several challenges, including concerns about equity, financial sustainability, and limited resource mobilization stemming from its flat-rate premium contribution system. To address these issues, a new sliding-scale premium scheme-based on households' socio-economic status-has been piloted in 13 districts across Oromia and Sidama regions in Ethiopia. However, empirical evidence on community responses to sliding-scale CBHI models remains

Longitudinal association between having stress-coping strategy and hypertension in a community-based Japanese study: the Circulatory Risk in Communities Study (CIRCS).

AIM: Hypertension is a leading contributor to the global disease burden and mortality. Although various risk factors have been identified, the roles of psychosocial stress and stress-coping strategies in the development of hypertension remain unclear. This study examined the longitudinal association between having a stress-coping strategy and the incidence of hypertension in Japanese communities. METHODS: A prospective cohort study, a part of the Circulatory Risk in Communities Study, included 3668 participants (1359 men and 2309 women), aged 40-74 years, from three rural areas in Japan. Bridging Evidence and Practice: A Community-Engaged Adaptation of Cognitive Behavioral Therapy for Autistic Youth With Anxiety Receiving Services in Community-Based Organizations.

Differences between research and community care settings contribute to a research-to-practice gap. Engaging community partners in intervention adaptation may better align mental health evidence-based practices (MH-EBPs) with community-based settings and intervention recipients. This study utilized a community-engaged intervention adaptation method (i.e., Adapted Method for Program Adaptation through Community Engagement [AM-PACE]) to develop a community-adapted version of Behavioral Interventions for Anxiety in Children with Autism (BIACA). A Community Advisory Board provided ongoing input abo

Community-Acquired Kodamaea ohmeri Fungemia in an Immunocompetent Host Coexisting With Severe Leptospirosis: A Case Report and Review of Community-Acquired Fungemia Cases.

Community-acquired fungemia in immunocompetent hosts remains exceptionally rare. Kodamaea ohmeri is an emerging opportunistic yeast pathogen increasingly recognised as a cause of invasive fungal infections, particularly among immunocompromised individuals and in healthcare settings. A 33-year-old previously healthy female farmer presented with severe leptospirosis with multiorgan dysfunction. Blood cultures obtained before central venous catheter insertion grew K. ohmeri, identified by the VITEK 2 YST card and matrix-assisted laser desorption ionisation-time of flight (MALDI-TOF) mass spectrom

Implementation and evaluation of a community health workers-led digital integrated diseases screening system to provide healthcare for patients at community level in Rwanda.

BACKGROUND: Community health workers (CHWs) play a vital role in identifying patients within the community. To enhance their decision-making and reduce unnecessary referrals, Rwanda introduced a digital integrated disease screening tool (d-IDS), embedded within the national community Electronic Medical Records (cEMR) system. This study aimed to design, integrate, and evaluate the d-IDS to support its broader national scale-up. METHODS: This study employed a pre-post effectiveness-implementation hybrid design to implement the d-IDS and evaluate its effectiveness in improving patient management

Patient-physician communication and shared decision-making in interstitial lung disease care: A mixed-method study based on a patient community survey.

BACKGROUND: Shared decision-making (SDM) is a core component of patient-centered care; however, its implementation in routine practice remains inconsistent, leading to communication gaps. In interstitial lung disease (ILD), where treatment decisions are complex, effective physician-patient communication may influence patient satisfaction and engagement. This study aimed to identify factors associated with patient satisfaction and patient-perceived involvement in SDM in ILD care. METHODS: This mixed-method study included a web-based questionnaire and qualitative analysis using the Marketing Re
